Job Summary

This position will contribute to these MGH efforts through collaboration with MassGeneral Hospital for Children (MGHfC) and the Department of General Internal Medicine (DGIM) Population Health Management teams.
MGHfC oversees a team of CommunityHealthWorkers. CHWs are trusted members of the community with the skills and experience to understand their patients' circumstances. By building trusting relationships and walking alongside their patients, CHWs help address medical and psychosocial needs in order to promote self-efficacy, help patients meet their goals, and improve health outcomes.
The CommunityHealthWorker (CHW) will be part of the pediatric CHAMPION Program's multi-disciplinary care team, which includes a physician and a dietitian. The evidence-based CHAMPION program and research study seeks to evaluate whether telehealth vs. in-person delivery of CHAMPION is equally effective at reducing child BMI and asthma control in diverse lower-income families. The CHAMPION program consists of individual and group visits guided by a set curriculum with a multi-disciplinary team (medical provider, dietitian, and communityhealthworker), and other tools to support behavioral change and reduce body mass index (BMI) and asthma, such as follow-up phone calls, educational materials, social determinants of health screening and referral and text messaging. The CHAMPION CHW will provide care to children and adolescents with overweight/obesity and asthma, through group and individual visits, both in-person and virtually, and will participate in a Virtual Learning Community and Quality Improvement calls and activities as part of the research project. The CHAMPION CHW will support this program at the MGH Chelsea HealthCare Center and MGH Revere HealthCare Center. Hours are expected to be roughly 10am-6:30pm Mondays, Tuesdays and Thursdays based on clinical schedules, and Wednesdays and Fridays, 8:30am-5pm.
The CHAMPION CHW will engage patients and their families in setting their own short-term goals and will track the benchmarks along the way toward the achievement of these goals. In addition, the Asthma -Healthy Weight Lifestyle CHAMPION CHW will work with patients to help decrease barriers to timely follow-up care and provide coaching to engage patients and families in identification and achievement of care goals.
This is not a clinical position, but the role is part of the clinical team and requires a good knowledge of (and willingness to learn) basic clinical concepts and an understanding of when a referral to a licensed clinician is appropriate.

Assist patients in organizing their records, making follow-up appointments, filling their prescriptions, and understanding past medical history.
Work with primary care providers to reinforce provider care plans and health education messages, the importance of follow-up care, medication adherence, routines of self-care, etc.
Provide advocacy, patient education, and support in accessing community-based and hospital-based programs.
Support timely scheduling, communication, and problem-solving across areas to ensure program excellence.
Refer to internal or external case management services within the practice when other issues are identified (i.e., food insecurity, domestic violence issues, etc.)
Collaboration and Documentation
Document each patient encounter in detail. Track benchmarks of progress in care, including short-term goal completion.
Maintain regular communication with the patient's providers (through clinical messages in EPIC, emails, phone calls, case review meetings, etc.)
Support practice staff to develop creative processes to proactively manage patients with asthma and/or obesity in a non-stigmatizing manner; help practice staff to develop patient-centered care goals.
Collaborate and communicate with interdisciplinary primary care team to identify care plan goals.
Additional Ad-hoc Responsibilities (generally on an as-needed basis):
Attend initial and continuing education training programs, including self-directed reading and in-person and online learning.
Complete an initial assessment with the patient and provider to identify the specific areas of focus for the asthma CHAMPION CHW role with particularly high-risk patients.
QUALIFICATIONS:
High school diploma or GED required.
Relevant experience in the community or a bachelor's degree preferred; preference for Psychology/Social Work/Public Health or related field.
3 years of experience in the field, pediatric healthy weight management training, or experience and motivational interviewing training is desired.

MENTAL HEALTH LITIGATION DIVISION OVERVIEW
The Mental Health Litigation Division provides individuals with attorneys for Civil Commitment proceedings in the District and Superior Courts and provides attorneys for appointment by the Probate and Family Courts to represent persons in Guardianships and Substituted Judgment proceedings, including cases involving Conservatorship, Elder Abuse, the Validation of Health Care Proxies, Nursing Home Admission and Do Not Resuscitate orders. Civil Commitments are handled by both staff and private attorneys, while most cases in the Probate Court are handled by private panel attorneys.
In addition to our staff offices in Brockton, Roxbury, Northampton, and Worcester, the MHLD has attorneys in our trial support and oversight office and our training department. All our attorneys receive training in issues relating to mental health and the statutory and constitutional rights of our clients. The Mental Health Litigation Division offers interns a variety of experiences and assignments under the supervision of highly experienced attorneys.

Interns in MHLD commitment offices will
* Shadow several attorneys during their internship,
* Develop a working understanding of G.L. c. 123 and the requirements for civil commitment, District and Municipal Court commitment proceedings, court-ordered medication proceedings, and the rules of evidence as applied in mental health cases,
* Visit clients and conduct interviews to obtain relevant information and ascertain the client's position on their pending petition(s)
* Work with independent medical experts including psychiatrists, psychologists, social service experts, and other mental health professionals as requested by staff attorneys,
* Work with attorneys to prepare motions for funds, motions for in person hearings, review medical records, direct examinations, cross-examination of Designated Forensic Psychologists and Psychiatrists and closing arguments,
* Develop research on race and equity issues in civil commitments and assist in developing a litigation guide for such issues, Conduct research into support for novel issues or arguments.
Mental Health Litigation Division Trial Panel Support Unit
The Mental Health Trial Panel Support Unit based in Boston supports and oversees the private panel attorneys handling civil commitments and probate court matters. TSOU seeks law students who have completed their 1L year to
* Conduct legal research on systemic and specific issues affecting clients with mental disabilities,
* Draft practice materials for attorneys representing clients in mental health matters,
* Handle intake line calls from patients and clients in mental health facilities and provide information about their legal status, assignment of counsel or referral,
* Trial Support interns also assist MHLD Training Unit attorneys to create and edit training materials, and to develop and maintain Mental Health online training and resource initiatives.

Specifically, this position promotes healthy sexuality by delivering curricula to middle and high school girls in the Lynn Public Schools and community agencies. The salary range for this position is $45,000 - $50,000 annually. As a full-time role, the Health Educator is eligible to participate in the organization's benefits program.
POSITION OVERVIEW (What we're looking for)
As a member of the Middle and High School Programs department, the Health Educator advocates for healthy sexuality education through partnerships with the Lynn Public Schools, LynnCommunityHealth Center, North Shore Medical Center, Girls Inc. National, and other service providers. This role creates a more holistic approach to healthy sexuality education, including but not limited to trauma-informed care, cultural competency, LGBTQ+, racial justice, and social & emotional learning. Reporting to the Adolescent Sexuality Education Program Manager, the Health Educator is a vital resource in implementing healthy sexuality approaches to assist participants in leading productive and fulfilling lives. This position will be scheduled to work 9:00 am - 5:00 pm.
ESSENTIAL FUNCTIONS & DUTIES (What you'll be achieving)
At a minimum, the Health Educator will be responsible for successfully carrying out the following essential functions and duties:
Collaborate and coordinate with the Lynn Public School administration and health teachers.
Plan and implement programming and workshops, including Girls Inc. healthy sexuality programming, for the middle and high school program participants as requested.
Plan and co-facilitate special events, such as National Day to Prevent Teen Pregnancy, local health fairs, and parent workshops as requested.
Participate in the Massachusetts Department of Public Health provider meetings, trainings, and regular conference calls as requested.
Complete all reporting requirements for the Department of Public Health and Girls Inc., administer all surveys, and maintain accurate records regarding program delivery.
Analyze data from participant surveys to inform programming and grant reporting.
Monitor progress and inform the Adolescent Sexuality Education Program Manager about all problems, concerns, and needs of youth and collaborating organizations.
Provide ongoing technical assistance and support to the community around healthy sexuality strategies.
Assist in the facilitation of training for program staff.
Participate as a team member of the Middle School and Teens Program to assess the needs of girls and the greater community in relation to how Girls Inc. is/could be meeting such needs.
Work collaboratively with Girls Inc. staff to provide a safe, secure, pro-girl environment.
Travel to and from schools to implement healthy sexuality education.
Assist with program adaptations as needed.
REQUIRED QUALIFICATIONS (What you'll bring along)
Education: Bachelor's degree in a related field from an accredited university. The educational requirement may be substituted with four years of relevant experience.
Experience: Minimum of two years of direct service-related experience working with adolescents, including teaching or the implementation of programs for middle and/or high school youth.

Excellent management and organizational skills.
Experience working with youth and adults in a multicultural, urban setting.
