The Field Based CommunityHealthWorker is responsible for assessment, planning and implementing care strategies that are individualized by patient and directed toward the most appropriate, least restrictive level of care. They also Identify and initiate referrals for social service programs; including financial, psychosocial, community and state supportive services, and manage the care plan throughout the continuum of care as a single point of contact for the member. As a Field Based CommunityHealthWorker (CHW), you will act in a liaison role with Medicaid members to ensure appropriate care is accessed as well as to provide home and social assessments and member education. The coordinator also addresses social determinant of health such as transportation, housing, and food access.
Working Schedule: Monday through Friday any 8-hour shift to be determined by the hiring manager between the hours of 8am to 5pm. This position is a field-based position with a home-based office. You will work from home when not in the field.
Location: Southeastern Missouri -Cape Girardeau, Scott, Stoddard and surrounding communities in Missouri.
Local travel up to 50% and mileage is reimbursed at current government rate.

Primary Responsibilities:
Assess, plan, and implement care strategies that are individualized by patient and directed toward the most appropriate, lease restrictive level of care
Utilize both company and community-based resources to establish a safe and effective case management plan for members
Collaborate with patient, family, and healthcare providers
Identify and initiate referrals for social service programs, including financial, psychosocial, community, and state supportive services
Communicate with all stakeholders the required health-related information to ensure quality coordinated care and services are provided expeditiously to all members
Partner with care team (community, providers, internal staff)
Advocate for patients and families as needed to ensure the patient's needs and choices are fully represented and supported by the healthcare team
Document all member assessments, care plan and referrals provided
You'll need to be flexible, adaptable and, above all, patient in all types of situations
Engage members either face to face or telephonically
Help member set person-centered SMART goals and develop a care plan to achieve those goals with regular follow up calls and ongoing documentation of progress towards goals met
Adhere to detailed, specific documentation requirements in the member's health record
Proactively engage the member to manage their own health and healthcare using Motivational Interviewing Skills
Provide member education on community resources and benefits
Utilize strong skill sets of managing multiple tasks at a time, being self-motivated, driven toward quality results, managing time well, being very detailed oriented and organized, work well in a team and on your own, and ability to manage multiple deadlines
Knowledge and continued learning of community cultures and values
Performs other duties as assigned

The essential functions include, but are not limited to the following:
• Liaison with senior living facilities and senior centers in Ray, Platte, and Clay County. Missouri.
• Responsible for tracking Older Adult community activities as dictated by grants in place.
• Acts as a knowledge expert and educates the community regarding Older Adult Programming.
• Assists the Manager in seeking program funding and outcome data for Older Adult Programming.
• Conducts support groups to older adults with the focus of increasing coping strategies.
• Conducts support groups for caregivers and kinship care (Grandfamilies).
• Offers education and support to older adults on topics of grief, depression, and other mental health topics.
• Organizes and leads programming and classes in the community.
• Leads the planning of a yearly conference, and other special events, with the focus on aging community and caregivers.
• Creates monthly newsletter/calendar and other email blasts about events occurring in the community.
• Works with Beacon social media staff to create flyers and other media information about events.
• Attends monthly coalition meetings to network with other professionals in the older adult community.
• Provides referrals and provides care coordination, in conjunction with the Older Adult Case Manager, to individuals and families seeking support and resources.
MINIMUM QUALIFICATIONS (KNOWLEDGE, SKILLS, AND ABILITIES)
Education:
• Bachelor's degree in a related social services field required. Master's degree preferred.
• Active driver's license and minimum state-required auto-insurance.
• CPR/First Aid Certification required.
Experience Required:
• 1 - 2 years in a similar related position or worked with older adults and/or caretakers OR
demonstration of training, leading education groups or liaison work.
Required Knowledge:
• Knowledge of behavioral health in the older adult population
• General knowledge of community resources and/or the willingness to research as needed
Skills/Abilities:
• Excellent communication skills, both written and verbal.

Attend community meetings and appropriate health fairs to understand community issues and provide information to community members. Identify the particular health care needs of individuals in a community.
Advocates for individual or communityhealth needs. Acts as a liaison between communities, individuals and other service providers.
Help clients understand their health conditions, develop strategies to improve their health conditions and provide informal counseling, support, and follow-up procedures.
Helps improve community resident's ability to effectively communicate with health care providers. Helps identify and reduce cultural and socio-economic barriers between patients and health providers.
Distribute posters, flyers, and brochures to YCHC service areas that are appropriate for the target population.
Utilizes the electronic healthcare record, excel spreadsheets, word processing and other software programs to format documents, spreadsheets, brochures, flyers, newsletters, training materials and other documents. Produces technical graphics, charts and tables, selecting the format that will best display the material. Enters data and produces various reports as required under the CommunityHealthWorker contract with MPCA and in accordance with requirements of the Primary Care Health Home (PCHH) program.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
Duties may include, but are not limited to:
* Develop appropriate camp curriculum based off your camp theme and have lesson plans completed & submitted by specific due date.
* Plan, organize, implement, and lead age-appropriate activities for children in your camp.
* Read and respond to all communications from leadership via email and on Microsoft Teams regarding camps, expectations, and camper concerns
* Read and understand information and expectations about health and background/IEPs for campers in your assigned camp PRIOR to the beginning of camp
* Submit supply requests & receipts to Program Specialist in a timely manner
* Work with an assigned assistant (as applicable) to provide a welcoming camp for all children, including greeting parents and creating a positive environment
* Make sure all arrival /departure procedures are followed (signing in/out with ID from parents)
* Communicate effectively with staff, assistants, campers, and parents, including sharing any issues that arise, and reporting all safety and/or health issues in a timely manner to the Program Specialist and Lead Support Staff
* Take responsibility for the care and safety of the participants, monitoring campers and ensuring campers follow camp rules and work to prevent accidents.
* Work to de-escalate situations in the classroom and utilize Support Staff when additional help is needed.
WORK SCHEDULE:
* Weekly between June 1 - August 7, 2026
* Instructors will be assigned based on availability and specialized field. Compensation is paid on a stipend basis for half or full day camps and is tiered based on education and experience. Minimum stipend for a full 5-day week is $375/half day camp or $750 for a full day camp.
* Camps run from:
* 9 a.m.-4 p.m. (Full day)
* 9 a.m.-12 noon (AM half day)
* 1 p.m.-4 p.m. (PM half day)
* Instructors should plan to be available with enough time to set-up and clean-up each day of camp, prior to campers arriving and after campers leave.
