Key Responsibilities Client Journey Management Serve as the clinical continuity partner from onboarding through renewal. Ensure all diagnostics (genetics, bloodwork, imaging, intake) are complete before the first physician consult. Be present on every physician-patient call to ensure communication needs are met, accurate notes are taken, and the health plan is updated in real time. After each consult, finalize notes and coordinate scheduling of future calls with the Longevity Strategist and with the Physician. Provide ongoing education, accountability, and coaching tailored to diagnostics. Maintain consistency and a personalized experience through proactive check-ins and responsiveness in partnership with the Lead Care Concierge. Diagnostic & Data Oversight Ensure all diagnostics (genetics, bloodwork, imaging, wearables, intake) are completed, integrated into the client's record, and translated into action. Manage all client data in the Catalyst tier in coordination with lead physicians. Support the physician in real-time clinical documentation during calls. Maintain and update the client's digital health report, roadmap, and active plan. Monthly Coaching & Lifestyle Optimization Review the client's health goals and clinical priorities. Conduct structured monthly coaching calls (and interim touchpoints as needed). Provide personalized support on sleep, stress, nutrition, movement, and daily routines. Track biometric and behavioral data (e.g., Oura, CGM, Whoop) and proactively guide adjustments. Review and troubleshoot supplement protocols and therapeutic response. Monitor client needs and flag urgent issues or escalations for physician's attention. Reporting, Progress Tracking & Renewal Support Help build, maintain, and present the Custom Annual Health Report. Track clinical KPIs (labs, HRV, glucose trends, symptom resolution) and create narrative summaries. Highlight progress wins for use in renewal framing and strategic updates. Comfortable with modern technology tools, including AI prompting. Ideal Candidate Profile Must-Have Experience & Skills 3+ years in health coaching, functional medicine, or precision health environments. Deep understanding of advanced diagnostics, data interpretation, and integrative therapies. Proven ability to translate science into action for high-performing clients. Familiarity with wearables, labs, peptides, and complex supplement stacks. Excellent communication skills, empathetic, confident, and able to hold clients accountable. Community Health Worker - ScreenNJ Grant Trenton, NJ Full Time Henry J Austin Health Center Entry Level Share Community Health Worker - ScreenNJ MAJOR FUNCTION The Community Health Worker is responsible for providing emotional, educational and overall support for clients who are experiencing multiple social determinants that are creating significant barriers to access and retention in care. This person is responsible to engage, inform, support and empower those who are eligible for any services provided by the Henry J Austin Health Center (HJAHC) and the community at large. ESSENTIAL FUNCTIONS Outreach and Health-Systems Navigation: * Engages in case finding and community outreach to bring out-of-care patients/clients who have never accessed care, or those that are lost to follow up into care services. * Establishes and maintains external contacts on a regular basis. Facilitates weekly/regular calls to check-in, for encouragement, appointment reminders, and to reschedule missed appointments. * Accompanies clients to medical, mental health, substance abuse treatment or social service appointments in order to improve attendance and for affiliation support. * Helps clients to schedule medical appointments and follows up with providers to ensure that clients attend appointments - coordinating and assisting with transportation. * Helps plan and coordinate Chronic Care Management in conjunction with medical providers. Health Education and Care Coordination: * Demonstrates health literacy of the field, assures treatment adherence education, and provides emotional support to clients. * Educates and assists clients in knowing what materials to bring or questions to ask in order to facilitate access to needed services, such as obtaining and identification (ID) card for housing application, health insurance, or application for other social support services. * Walks clients through initial appointments for medical care or other social services in order to ensure that clients have a clear understanding of initial steps that need to be taken to obtain appropriate services. * Promotes effective resource utilization with the multidisciplinary team. * Identify beneficial local resources and keeping track of patient's referrals and assisting with keeping appointments. * Promotes the program within HJAHC and other health care organizations that feed and draw from HJAHC. * Participates in staff training sessions and other meetings as required by the agency. Documentation, Compliance and Quality Assurance: * Assists in the collection of outcome data. * Maintains timely, accurate, legible and clear chart documentation according to agency requirements. * Maintains record keeping requirements and assists with chart reviews for Q/A purposes as requested. * Works with data resources and health information products to drive productivity and quality improvement. Safety: * Follows universal precautions according to Centers for Disease Control and Prevention guidelines. * Understands and appropriately acts upon assigned role in Emergency Code System * Understands and performs assigned role in agency's Continuity of Operations Plan (COOP). ADDITIONAL RESPONSIBILITIES: * Performs other duties and assumes other responsibilities as apparent and/or as assigned by [Reports to Position]. Duties, responsibilities and activities may change or new ones may be assigned at any time with or without notice. REQUIREMENTS: EDUCATION & EXPERIENCE: * High school diploma or equivalent required, college degree a plus. Associates degree or Bachelor's degree preferred. * One year of related experience working in programs with at risk populations and performing outreach is required. LICENSURE AND/OR CERTIFICATIONS: * Additional requirements such as certifications, industry-specific experience and the experience working with certain equipment KNOWLEDGE, SKILLS, ABILITIES AND OTHER (KSAO's) * Working knowledge of social determinants and impact on health outcomes is preferred * Knowledge of regional community resources/services is important. * Computer knowledge should include Microsoft Word and Excel. * Bilingual (English-Spanish/English-Creole) preferred. * Good communication, problem solving, team work and organizational skills are required in order to engage participants. * Ability to work with multicultural and diverse population is required. * Must be self-motivated, detail oriented, able to travel locally and work flexible hours. * Must have a passion about working to improve the health and quality of life of the population. * Demonstrates consistent effort to maintain sound working relationships with staff. * Demonstrates flexibility and cooperation in relation to workplace manpower and staffing needs by rotating on an emergent basis and providing assistance, as needed. * Consistently utilizes appropriate lines of authority, as needed.     Job Description Community Health Worker - Street Outreach Reports to: Program Supervisor Full time 35 hours per week Monday-Friday with occasional late days and weekends Salary Range: $45,000- 50,000 Program Summary: This program serves people who are primarily unhoused and living on the street, and engaged in behaviors that place them at high risk for contracting HIV/AIDS. These behaviors include unsafe sex practices and intravenous drug usage; The goal of the program is to provide them with basic needs, services, linkages and referrals, including HIV and HepC screening and testing, safer sex information, substance use services, medical resources, and other information. Position Summary: The Community Health Worker will work in a robust outreach program that offers education, testing, counseling, basic needs, resources, and referrals in communities where people are unhoused and that are at high risk for HIV, Hep. C and drug overdoses. These services are delivered with the use of our Shower Bus and Mobile Outreach Van. Responsibilities: Provides program services delivery to clients that are living on the streets and that are at risk for HIV, Hep. C, drug overdoses in compliance with funders and regulatory requirements; best practices; BCS policies and Core Values; Serve as a role model to all stakeholders and clients served. Provide education, counseling, testing, linkages, resources, and other information on HIV, Hep. C and substance abuse overdose prevention. Ability to distribute high-risk prevention supplies, including female & male condoms, safer hygiene kits, first aid, opioid overdose prevention, fentanyl testing strip kits, and provision of sterile syringes. Enter client information into BCS and funder mandated databases (AIRS System, Apricot, etc ) Drive and maintain the Shower Buse and Outreach Mobile Van Manage caseload of clients who are unhoused and high risk HIV Negative unknown with multiple psychiatric and health needs. Design and implement service plans for clients; provide follow-up referrals, needs assessments, advocacy, emergency interventions Maintenance of case records, discharge and transfer planning Escort clients to various appointments when needed Facilitate groups and workshops, including training peer advocates and volunteers Participate in Team and departmental meetings Completing required number of annual training hours. Follow Funder's and our organization's protocols for safety and confidentiality guidelines. Prepared to respond appropriately to emergency situations that may be encountered in the field in accordance with training received including overdoses Preparation, packing, and inventory of kits and program supplies.
