Collaborative Partnerships
Work in partnership with interdisciplinary palliative care team members including APRNs, physicians, social workers, and spiritual care providers.
Represent the program at community meetings and support holistic care planning.
Program Support
Educate clients on appropriate healthcare utilization, including reducing avoidable emergency room visits.
Travel frequently within the service area to engage with clients in person.

Job Description
The IHC has a solid understanding of human behavior; will be familiar with legal issues, ethical issues, insurance regulations and protocols for proper referral or application for various programs related to behavioral health. The incumbent must also be knowledgeable in medical care and emergency services and typical operations of ambulatory care centers. Must be comfortable providing coverage across all potential age populations and patients with co-morbid medical conditions. Commitment to an inter-professional environment and recognition of a bio-psychosocial conceptualization of patient health concerns is necessary. Must also possess skills that promote excellence in communication, flexibility, team-based care and decision making. The IHC conducts behavioral health services in a primary care clinic setting by providing diagnostic assessment, onsite crisis assessment and intervention, determining need for ongoing care according to level of clinical ability, medical and psychosocial complexity. The IHC is competent in multiple counseling modalities (ranging from very brief to long-term), making appropriate referrals, and providing educational services across the continuum of care. The IHC follows up with referrals to specialty care and ensures those patients are receiving appropriate behavioral health management. Serves as the primary team consultant to promote understanding of the relationship between health and psychological/behavioral factors and promote this understanding to patients, families, other team members and the community. Takes active leadership in universal health screening for behavioral and chemical health conditions that negatively impact overall health and wellness and works collaboratively with other team members to ensure quality delivery of care. Provides education and training to staff regarding management of behavioral emergency situations and current behavioral health information and how this interacts with overall health. Delivers empirically validated, short-term behavioral health care services (assessment, consultation, diagnosis, and treatment) to ambulatory primary care patients via both traditional face-to-face visits and innovative care delivery models such as telehealth when necessary. Works closely with other PCMH or PCC team members to address psychosocial and cultural elements of health and disease pertinent to the patient's ability to manage their acute and chronic conditions and appreciate other factors that influence health care (developmental, economic, familial, religious, etc.) and wellness. Assists the team in effectively managing populations of patients, tracking costs and quality associated with innovative integrated care models, and implementing the highest standards of behavioral health care. Systemically evaluates the quality and effectiveness of behavioral health care and actively participates in quality improvement activities. Documents in a responsible, accountable and ethical manner to promote safe, reliable patient centered care. You will develop and maintain offender supervision plans and monitor offender compliance. You will conduct drug testing, fingerprinting, DNA testing and assist with maintenance of SCRAM alcohol monitoring program. You will assist with and monitor collection of court costs, fines, fees, restitution and with transportation and body searches of clients. You will Interact with the judicial system and various agencies to assist with the rehabilitation of people on parole or probation.
Minimum Qualifications
Requires a high school diploma or GED and three years of work experience directly interacting with people in a correctional, law enforcement, education, business or behavioral health setting. College coursework in a behavioral science, education, business or field related to the duties of the position may substitute for the work experience requirement on a year-for-year basis.

Responsibilities/Duties
Conduct outreach, engage in public speaking, and ensure those seeking support are connected to services. Coordinate refugee medical screening for serious medical need cases, working in collaboration with case managers/community connectors and refugee health partners to complete medical screening within 30-90 days of arrival or eligibility.
Provide guidance to the case manager/community connector when appropriate to ensure timely provision and coordination of post-screening appointments and medical services, particularly for those with serious health conditions. Remove barriers to help clients establish a medical home whenever possible. Collaborate with the NDHHS-funded Refugee Support Service Programs to fulfill the scope of the health service provisions established by the State.
Facilitate timely communication among state, medical providers, and resettlement agency staff to support the provision of needed services for those with serious medical conditions. Oversee delivery of health-related community cultural orientation, design and deliver pre-and-post assessments, and provide written and visual information on health topics.
Remove barriers to help case managers/community connectors arrange transportation as needed to complete refugee medical screening and follow-up medical appointments.
Provide guidance on Health care coverage plans. Conduct outreach to potential new providers to ensure adequate support for clients. Maintain an up-to-date list of communityhealth resources.
Work collaboratively with refugee medical screening partners, refugee health partners, and the State Refugee Health Coordinator to troubleshoot other barriers that inhibit refugees' access to health services and systems.
