Participates in team meetings and assists as needed in any activities related to the clinic or the Brown University HealthCommunityHealth Institute. Serves as liaison between the professional staff and the community; including developing relationships with various stakeholders in the community. May transport ambulatory patients between their homes and clinics, hospitals or other social agencies, and meet with patients at home, in the hospital, or in community settings.Attends ongoing training for communityhealthworkers. Practices clear, effective, consistent communication with clients and colleagues. Demonstrate privacy and confidentiality, meeting all related training and practice requirements of the Brown University Health system.Works nights and weekends as required.Other duties as assigned. MINIMUM QUALIFICATIONS: QUALIFICATIONS - EXPERIENCE: Two (2) years of verifiable experience providing information, education, intervention and/or referral services to culturally diverse populations. Recent experience (last 5 years) working as part of a multi-disciplinary team in a health care setting preferred.CommunityHealthWorker certification in Rhode Island preferred. Experience with motivational interviewing, advising/counseling clients, and/or participating in health promotion and health education activities. Excellent verbal and written communication skills. Excellent organization, thorough record-keeping, follow through, and ability to juggle multiple priorities in fast-paced environment with multiple collaborators. Demonstrated excellent attendance and reliability. Prior experience using an electronic health record preferred.Bilingual English/Spanish preferred.

Participate with Trauma Systems Therapy- CommunityHealth Team in conducting a full needs assessment utilizing RI DOH standardized tools.

Provide coordinated care and monitoring through contact with individuals, families, medical entities, community providers and referral sources.
Build and maintain positive, working relationships with medical entities, community organizations and service providers.
Teach self-advocacy skills.
Complete all RI DOH and Family Service paperwork including evaluations and written documentation of all communications with families, physicians, referral sources, community partners, etc.
Complete all documentation in accordance with agency standards and within established guidelines and timeframes.

Qualifications:
CommunityHealthWorker Certification required; Bachelor degree preferred.

Review and educate self and consumers on various benefits and resources that will work to assist the individual in gaining a greater quality of life.
Assist the consumer in completion and submission of enrollment or benefit applications. Refer consumers to other services and public or private agencies for additional supports as needed.
Utilizing motivational interviewing skills and culturally sensitive methods to collaborate with patients to explore preferred post-discharge supports and identify social determinants of health and/or areas of need within their community environment. Review care options including natural supports, home care services, medical equipment, adult day health programs, senior centers and assisted living communities.
Maintain current on information about statewide housing services ranging from shelters, subsidized apartments and housing programs.
Be familiar with Homeless service providers, resources for individuals who are homeless and be familiar with common places clients, who are homeless, are regularly found.
Attend and complete CommunityHealthWorker trainings as required by the grant.
Maintain timely, accurate records, documentation, and reports as required.

The Opportunity
The CommunityHealthWorker (CHW) is responsible providing support to patients and families as part of Rhode Island Primary Care interdisciplinary care team within a primary care setting. The CommunityHealthWorker plays a critical role in assessing patient needs, offering community resources and referrals, providing navigation, support, care coordination, and ongoing case management to meet those needs. POSITION SUMMARY:
Coordinate the programmatic, educational, administrative, and operational functions essential for the efficient functioning of the Community First Responder Program (CFRP). Assist the Program Director and the Program Manager. Coordinate the activities of the Community First Responder Program, including scheduling and executing outreach events and delivering overdose response trainings. Provide coordination of financial matters pertaining to administration of CFRP budgets. Serve as coordinator support for all CFRP activities and naloxone distribution. Provide data compilation, entry into reporting platforms, and submission of progress reports.
Duties and Responsibilities
KEY DUTIES AND RESPONSIBILITIES:
Assist with program outreach and overdose education with naloxone distribution (OEND). Serve as support for Program Manager and undergraduate students with arranging, scheduling, staffing, and executing outreach and OEND events in the community.
Coordinate and assist with delivery of naloxone to community partners. Ensure accurate and comprehensive naloxone inventory records are maintained.
Enter all data collected from educational seminars and outreach efforts into the Rhode Island Department of Health REDCap platform and the University of Rhode Island ArcGIS platform.
Serve as a liaison to the Rhode Island Department of Health for CFRP to ensure outreach events and OEND trainings are offered for key populations including, but not limited to, those required by RI law.
Assist the Program Manager with the preparation and submission of required on-line reporting to the funding agency including, but not limited to, inventory reports and post-event participant data.
Coordinate grant activities outlined in the grant scope of work to ensure milestones are being completed in a timely fashion. Communicate and coordinate with all CFRP staff and affiliates, within the College as well as external partners, to ensure specific aims are being met.
Manage the marketing, design and distribution of announcements, educational materials, social media and publications related to the Community First Responder Program.
Assist in the preparation of specialized clinical data analysis, financial reports, and daily assigned projects as needed by the Program Director and Program Manager.
Provide coordination of financial matters pertaining to all administration of CFRP budgets and reconciling them with their approved goals and objectives. Monitor assigned accounts and maintain accurate financial records utilizing the PeopleSoft system. Prepare and maintain Excel spreadsheets for financial reporting and analysis.
Function as a liaison to the College's Business Office team and liaise with other University offices, when appropriate, including the Health Services and the URI Office of Sponsored Research.

Essential Responsibilities:
Participate in weekly street outreach hours.
Maintain a caseload of individuals identified through street outreach until they are successfully linked to other supportive services.
Perform comprehensive client assessments to collect functional, environmental, psycho-social, financial, employment, housing, educational, and health information as appropriate to develop and create a detailed case plan.
Administer VI-SPDAT and SPDAT assessments as required.
Obtain all necessary releases and consent forms from clients.
Establish and maintain confidential case files for residents and complete progress notes for each client and document any other clinical correspondence as required.
Accurately collect data for HMIS, including documentation of chronic homelessness; accurately collect and enter data into the Coordinated Entry System.
Accurately document time and effort and required funders.
Acquire and maintain knowledge of outside sources such as medical, psychological, educational, social services, legal, housing, and community resources and how to apply and use them effectively.
Engage potential clients; provide and coordinate referrals, emergency services, and crisis intervention, as needed.
Recognize emergency and crisis situations, and take action and/or seek intervention when needed.
Accurately document time and effort as required by funders/BHDDH.
Actively identify potential housing opportunities, both within Housing First and with other housing providers in the community.
Carry and enliven a positive Thrive image consistent with the Thrive's Guiding Principles and professional code of ethics.
Qualifications
Education, Experience and Competencies:
Bachelor degree in social work or human services, or equivalent of related field of education and experience. 5 years' experience in the fields of mental health, substance abuse, and/or working with homeless adults or families
Certifications, Licenses Requirements:
Supported Employment Professional Certification may replace one (1) year of experience in education or experience.
Valid driver's license and automobile insurance which meets Rhode Island minimum standards required, or the ability to obtain necessary transportation in order to perform the responsibilities/tasks of the job.
