Position Summary:
The Health Insurance and Services Navigator I ensures HealthPoint patients and community members have access to vital resources such as health insurance, basic food, and transportation by assessing patients needs to connect them with necessary resources.
Compensation is dependent on skills and experience.
Your contribution to the team includes:
* Responsible for assisting clients in recertification and applications for Medicaid, Apple Health, Qualified Health Program, Family Panning, Basic Food, Medicare Savings and other state or federal assistance programs which includes identifying and screening clients for eligibility and providing education on relevant programs
* Maintain and develop working relationships with external organizations such as WHPF, HCA, PH, DSHS and others, to actively advocate and troubleshoot for clients
* Adhere to exchange and navigator license requirements including recertifications, updates and annual in-service days to ensure HealthPoint's ability to continue to provide navigator services to our patients and community
* Responsible for providing consistent documentation and patient follow to complete applications processes and ensuring correct assignment of insurance which may include helping patients switch insurance or insurance assignment by completing switch forms or contacting insurance providers directly on behalf of the patient.
* Maintain confidentiality of patient information in line with exchange and HIPAA guidelines by properly documenting patient interactions in approved programs, and meeting with clients in private spaces where patient information can be openly shared
* Required to review, build and work reports that may require outreach to specific patient populations to ensure HealthPoint patients are receiving eligible education on available services which includes uninsured, unengaged, and aging out reports
* Responsible for remaining current and participating in Annual Enrollment and Open Enrollment periods which includes actively sharing information with sites on performance, updates and changes along with being a point of contact for questions
* Represent HealthPoint at community events as determined by leadership to provide insurance, Basic Food, HealthPoint and other resources to clients.
* Attend internal and external meetings required of the role, which may include presenting material or sharing of information
* Assist in the development and implementation of outreach activities and materials to enhance and improve customer service and client relationships through internal and external relationships.
* Implement strategies to collect data on service and access issues as directed through mechanisms such as surveys, questionnaires, and time logs. Identify ways to improve current systems and report the information to appropriate individuals.
* Contribute to a positive work environment centered around teamwork, collaboration, accountability. Demonstrate respectful, professional, and appropriate behavior.

Must have's you'll need to be successful:
* Technical College or Associates Degree and 1-2 years of experience with Medicaid medical insurance preferred or a combination of education and experience.
* WHPF Certified Navigator within one (1) month of being hired.

Summary of Job Description:

ESSENTIAL DUTIES AND RESPONSIBILITIES:
Provide Legal Systems Navigation for clients and how the juvenile and criminal justice systems operate, including court procedures, probation, diversion programs, and youth rights.
Provide Cognitive Behavioral Therapy (CBT) to support youth with behavioral and emotional challenges.
Provide the Daily Living Activities-20 tool, which helps assess functioning and track progress in youth ages six and up, regardless of diagnosis.
Provide outpatient Mental Health and Substance Use Disorder evaluations, assessments, care plans, and therapy to children and adolescents as needed in a variety of professional therapeutic modalities.
Provide outreach and referral assistance to clients unable to access services in the community.
Provide Crisis Intervention and De-escalation to manage high-stress, potentially violent situations using non-violent communication and de-escalation techniques.
Conduct Motivational Interviewing (MI)
Provide Trauma-Informed Care to ensure sensitive and effective engagement with youth impacted by violence and systemic trauma.
Must have signed and agreed to the National PREA (Prison Rape Elimination Act) Standards, ensuring compliance with safety and ethical requirements.
Track client outcomes using evidence-based tools and case management systems.
Achieve at least a 75% increase in participants reporting an improved sense of safety and/or security.

Maintain the caseload of active clients assigned to meet productivity requirements.

QUALIFICATIONS:
Master's required in social work or related social services field
MHP Required.
Current Substance Abuse Disorder Trainee (SUDT) License.
Two (2) years of experience in providing mental health counseling and case management services.
Active State of Washington Counselor License as required by the Department of Health.
Proficient in speaking, reading, and writing both English and Spanish.
Skilled in understanding Hispanic/Latino cultural issues and sensitivity in relating to cross-cultural, social, economic, and social determinants of health.
Experience working with multiple systems serving children.
Experience in assessments, case management, advocacy, and referrals working with hard-to-serve, low-income, youths of color who are at risk of or have already dropped out of school.

Class: Non-Exempt Salary Range: 7/8 $28.23 - $31.12/hr DOQ Status: Part-Time, 30 hours per week The South Puget Intertribal Planning Agency's (SPIPA) Health and Wellness Department is funded through a five-year cooperative agreement between SPIPA and the Centers for Disease Control and Prevention (CDC) to facilitate the Comprehensive Cancer Control Program (CCCP) and the Native Women's Wellness Program (NWWP). The Community Outreach Liaison reports directly to the Health and Wellness Programs Manager and works closely and collaboratively with the CCCP and NWWP Coordinators. The Community Outreach Liaison will provide outreach and event planning services to support the programs goal in improving cancer prevention, early detection, and survivorship resources within the 5 Consortium Tribes. The position is located at the Intertribal Professional Center (IPC) on the Squaxin Island Reservation near Shelton, WA.
Responsibilities:
Work as a liaison between the Tribal community members and SPIPA staff, programs, and the five Tribal Health Clinics and communities.
Provide technical assistance and support to the staff at each of the five Tribal Health Clinics on increasing cancer screening rates and identifying women eligible to enroll in the NWWP.
Provide technical assistance and support to the staff at each of the five Tribal Health Clinics on cancer survivorship needs, resources, and support services.
Develop and recommend activities and strategies at each Tribal site to support and drive initiatives to increase cancer screening rates and reduce health disparities.
In coordination with the SPIPA Health and Wellness team, seek out and engage in Native American activities to improve overall health and wellness of the communities.
Act as a liaison to ensure culturally appropriate quality health and survivorship care for the SPIPA Tribal communities.
Provide one-on-one and group outreach to Tribal communities regarding cancer information, resources, and education.
Identify implementation challenges and roadblocks that require attention to better ensure program efficiency and efficacy.
Work closely with the program Coordinators to prepare and provide updated information monthly for SPIPA, the Tribal communities, and the Centers for Disease Control and Prevention to include education materials, flyers, data tracking, and dissemination documents.

The Community Outreach Liaison, as an employee of the South Puget Intertribal Planning Agency, is expected to collaborate, engage, and work closely with all SPIPA programs to fully support the agency's Vision, Mission, and Core Values in accordance with all agency directives.
Minimum Requirements:
Associate's Degree in relevant medical field, communications, or related field with three years of experience in patient navigation, health care/social service, public health outreach or similar responsibilities.

Job Summary
The CommunityHealthWorker - School Based facilitates access to ICHS school-based health centers and primary care clinics to connect youth and their families to quality affordable care. This role will support referrals and linkages to external and internal services for school-based patients. This role will also provide operational support for the school-based health center. Additionally, this person will identify and maintain relationships with school partners who serve populations with identified health disparities. Activities may include promoting access to preventive care through independent outreach and education with individuals or groups in the community. They will also provide support with insurance enrollment.
Education - Associate's degree. Experience in a health care setting may be substituted year for year for the education requirement.
Experience - One (1) year experience in a health care setting, communityhealth education and/or outreach position.
Other Requirement(s) - Fluency in English is required: additional fluency in another language is valued. Proficient computer skills.

Plan and oversee daily schedules.
Model and encourage good social skills, e.g. strong communication and conflict-resolution skills.
Monitor and report developmental or safety concerns.
Help clients take prescribed medication.
Assist clients with ambulation and mobility around the house or outside (doctor's appointments, walks etc.)
Help clients with physical therapy exercises
Plan and prepare meals with assistance from the clients (As they are able)
Do the client's shopping or accompany them when they shop
Perform housekeeping duties that clients can't complete on their own
Be a pleasant and supportive companion
Report any unusual incidents
Act quickly and responsibly in cases of emergency
Requirements:
High school diploma or equivalent.
CNA or HCA Certification with Nurse Delegation Preferred - Training Available for Qualified Candidates
Good communication skills to report on each client's individual needs/performance/development to superiors or parents.
Physical energy to provide quality support.
Driver's license Preferred
Clean criminal record/background check.
