POSITION SUMMARY:
The HIV Nutritionist Manger oversees the HIV Nutrition Program, working directly with a diverse HIV-positive patient population, providing nutrition education and counseling in both individual and group settings, and working as part of a clinical team to provide coordinated care (medical, educational, and social services).
DUTIES AND RESPONSIBILITIES:
Essential Functions:
* Evaluates and monitors the nutritional status of patients living with HIV/AIDS.

* Organizes and develops nutritional care plans and individualized therapeutic diets as needed.
* Completes necessary reports and forms and document patient services in the appropriate medical record.

* Organizes, reviews, and distributes HIV nutrition education materials and patient screening forms.
* Serves as a resource and educator of HIV nutrition to the clinical and social service staff.
* Attends conferences and participates in trainings/informal sessions as requested by the Director of Nutrition.
* Works closely with HIV providers, treatment adherence, case management, and social service staff, to improve patient services.
* Conferences with the Director of Nutrition as necessary to seek consultation and share information that is pertinent to patient care.
* Participates in clinic and central office team meetings as designated.
* Plans and facilitates educational presentations and workshops for the agency.
* Compliance with Employee Health Services.
* Participates in the development of HIV Nutrition program policies
* Perform other related duties as assigned by the Director of Nutrition
Certificate/License required:
* RD/RDE preferred with New York State Certification
