POSITION SUMMARY: Provides leadership and oversight to the Community Psychiatry Nurse Practitioner Fellowship. Acts as the primary preceptor for the Fellows in the program. Develops and implements fellowship curriculum and training content. and collaborates with consultants and/or experts in the field for program growth and development. Maintains collaborative relationships with external agency partners in the community. Refines and implements methods of program evaluation. Performs assessment and treatment planning for patients in the area of Behavioral Health. Meets regularly with program staff to address clinical issues. JOB FUNCTION: Responsibilities include, but are not limited to: Participates in the recruitment and hiring of new internal CHN fellows. Acts as preceptor to the Psychiatric NP fellows: Will precept fellows at health center, guiding them with patient care and mentoring them. Consults with the Director of Psychiatry and the Director of Mental Health on a regular basis. May participate in monthly staff physician conferences chaired by the Vice President of Medical Affairs/Medical Director to discuss subjects of interests to staff physicians. Maintains chart documentation in compliance with Office of Mental Health and CHN standards. Travels between Mental Health treatment sites. Meets productivity standards as determined by the Director of Psychiatry and approved by the Chief Medical Officer. Facilitates related services for health center patients and CHN staff as appropriate, with respect to their confidentiality and privacy. Performs other related duties as assigned. WHAT WE LOOK FOR : Current NYS license as Psychiatric Nurse Practitioner. Three years of experience as Psychiatric Nurse Practitioner, preferably in a community based mental health clinic setting.

Community Healthcare Network is seeking for a Full-Time Maintenance Worker who will be responsible for providing support to the Facilities Department in overall maintenance of facilities, equipment and minor construction. JOB FUNCTION: Responsibilities include, but are not limited to: Under the direction of the Facility Management staff, coordinates work needed to keep Central Office, the clinics and satellite offices in compliance with regulations of funding sources, as well as, local, state, and federal code requirements, including OSHA regulations. Performs preventive building maintenance duties, caulks windows and doors, repairs and replaces broken windows. POSITION SUMMARY: The HIV Nutritionist Manger oversees the HIV Nutrition Program, working directly with a diverse HIV-positive patient population, providing nutrition education and counseling in both individual and group settings, and working as part of a clinical team to provide coordinated care (medical, educational, and social services). DUTIES AND RESPONSIBILITIES: Essential Functions: * Evaluates and monitors the nutritional status of patients living with HIV/AIDS. * Conducts HIV-related nutrition education and counseling sessions. * Organizes and develops nutritional care plans and individualized therapeutic diets as needed. * Completes necessary reports and forms and document patient services in the appropriate medical record. * Facilitates related services for health center clients as appropriate with respect to their confidentiality and privacy. * Organizes, reviews, and distributes HIV nutrition education materials and patient screening forms. * Serves as a resource and educator of HIV nutrition to the clinical and social service staff. * Attends conferences and participates in trainings/informal sessions as requested by the Director of Nutrition. * Works closely with HIV providers, treatment adherence, case management, and social service staff, to improve patient services. * Conferences with the Director of Nutrition as necessary to seek consultation and share information that is pertinent to patient care. * Participates in clinic and central office team meetings as designated. * Plans and facilitates educational presentations and workshops for the agency. * Compliance with Employee Health Services. * Participates in the development of HIV Nutrition program policies * Perform other related duties as assigned by the Director of Nutrition Certificate/License required: * RD/RDE preferred with New York State Certification * Bachelors Degree in Nutrition * 1 to 3 years nutritional experience in a clinical/community health setting * Experience or training in HIV Medical Nutrition Therapy * Bilingual in the language of the community being served a plus
