POSITION SUMMARY: Provides administrative support to Director of Psychiatry. To act as a psychiatric consultant on clinical issues to all areas of CHN including primary care and health homes. Eighty percent (80%) of the provider's time is spent conducting psychiatric evaluations and management of patients as well as providing consultations and education to clinical staff. JOB FUNCTION: Responsibilities include, but are not limited to: 1. Obtains and/or reviews/evaluates an obstetric patient's medical and psychosocial history. 2. Performs complete family planning and/or obstetric comprehensive physical examination. 3. Performs screening procedures including Pap Smears, GC cultures, Chlamydia tests and Wet preps. 6. Facilitates related services for health center patients as appropriate with respect to their confidentiality and privacy. 7. Performs routine interim physical examinations on patients that are risk specific, and as outlined in the agency's Medical Policies and Procedures Manual. 8. Evaluates individual patients and prescribes appropriate medications, treatments, referrals as indicated in the Policies & Procedures Manual and/or is consistent with current standards of care. 9. POSITION SUMMARY: The Mother and Child Health Coordinator will implement a wide range of clinical and community health education activities for prenatal patients. As part of a multi-disciplinary team, the Mother and Child Health Coordinator is responsible for coordinating clinical outreach and education relating to prenatal health, specifically, breastfeeding, postpartum care, and reproductive health to improve health outcomes for pregnant people, new parents, and their children. DUTIES AND RESPONSIBILITIES: Support the Population Health Department in maintaining quality standards and services that are in accordance with the recommended clinical guidelines for prenatal and postpartum services. Provide prenatal, postpartum, and infant care education to prenatal patients and people of childbearing age in the clinic setting, by telehealth, and in group settings. Conduct and document prenatal assessments, including but not limited to lactation assessments, safe sleep assessments, and depression screening. Support patients with obtaining durable medical equipment, as prescribed by the provider. Provide visits for postpartum patients up to 12 months postpartum to assess needs such as infant's initial milestones and SIDS/safe sleep risk. Identify high-risk prenatal patients at CHN health centers and follow-up to ensure patients made their medical appointments and reschedule as needed. Coordinate prenatal and postpartum appointments for routine/follow-up care, delivery, and connect patients to specialty services, particularly nutritionists and behavioral health services and external referrals. Conduct home visits ( including but not limited to post-partum ) as required pursuant to the terms of the applicable grant[s]. Meet programmatic deliverables for the number of patients seen each week, conducting screenings, addressing concrete needs and education reviewed with the patients. Track and monitor the collection process for weights of infants delivered by CHN prenatal patients, and coordinate follow-up efforts with department management. Assist with data collection and analysis to measure prenatal care and birth outcomes through structured quality improvement projects. Participate and record minutes for internal committee meetings related to prenatal care, to identify and address areas for improvement in clinical services. Identify patient materials for prenatal, postpartum, infant care, and breastfeeding education that need to be updated and/or developed as needed, seeing the process to completion. Complete any necessary reports and forms and document patient services in the appropriate medical record. Travel between health centers and balances schedule to provide education at multiple sites. Plan and participate in special CHN events, including but not limited to, World Breastfeeding Week. Identify community resources for health education resources and/or services. Participate in all clinic staff meetings and training/informational sessions as appropriate. Facilitate related services for health center patients and CHN staff as appropriate, with respect to their confidentiality and privacy. Attend and participate in ongoing training and professional development opportunities to stay up-to-date on the latest research and best practices relevant to pre-natal care. Healthy Start Queens grant funded Prenatal Program Coordinators, must utilize the UniteUs and Go Beyond platforms to register and refer patients for home visiting, maternal child health and social drivers of health services. EDUCATION AND EXPERIENCE required: Bachelor's degree in Health Education, health-related field, or satisfactory equivalent combination of education and experience. Certification as a Lactation Counselor and/or Breastfeeding Counselor or obtain one of the above-mentioned certifications within one (1) year of hire.

As a team leader, the HHCM is ultimately responsible for the overall provision and coordination of services to assigned patients. JOB FUNCTION: Responsibilities include, but are not limited to: Provides direct service to a caseload of approximately 15-20 patients. Conducts and documents initial comprehensive biopsychosocial assessments of patients' needs including medical, mental health, substance use and social determinants of health in accordance with Health Home Plus guidelines. Assessments, reassessments, and plan of care updates must be conducted face to face in patients' place of residence. Provides crisis intervention and health education services as needed. DUTIES AND RESPONSIBILITIES: * Support the Population Health Department in maintaining quality standards and services that are in accordance with the recommended clinical guidelines for prenatal and postpartum services. * Provide prenatal, postpartum, and infant care education to prenatal patients and people of childbearing age in the clinic setting, by telehealth, and in group settings. * Conduct and document prenatal assessments, including but not limited to lactation assessments, safe sleep assessments, and depression screening. * Support patients with obtaining durable medical equipment, as prescribed by the provider. * Provide visits for postpartum patients up to 12 months postpartum to assess needs such as infant's initial milestones and SIDS/safe sleep risk. * Identify high-risk prenatal patients at CHN health centers and follow-up to ensure patients made their medical appointments and reschedule as needed. * Coordinate prenatal and postpartum appointments for routine/follow-up care, delivery, and connect patients to specialty services, particularly nutritionists and behavioral health services and external referrals. * Conduct home visits (including but not limited to post-partum) as required pursuant to the terms of the applicable grant[s]. * Meet programmatic deliverables for the number of patients seen each week, conducting screenings, addressing concrete needs and education reviewed with the patients. * Track and monitor the collection process for weights of infants delivered by CHN prenatal patients, and coordinate follow-up efforts with department management. * Assist with data collection and analysis to measure prenatal care and birth outcomes through structured quality improvement projects. * Participate and record minutes for internal committee meetings related to prenatal care, to identify and address areas for improvement in clinical services. * Identify patient materials for prenatal, postpartum, infant care, and breastfeeding education that need to be updated and/or developed as needed, seeing the process to completion. * Complete any necessary reports and forms and document patient services in the appropriate medical record. * Travel between health centers and balances schedule to provide education at multiple sites. * Plan and participate in special CHN events, including but not limited to, World Breastfeeding Week. * Identify community resources for health education resources and/or services. * Participate in all clinic staff meetings and training/informational sessions as appropriate. * Facilitate related services for health center patients and CHN staff as appropriate, with respect to their confidentiality and privacy. * Attend and participate in ongoing training and professional development opportunities to stay up-to-date on the latest research and best practices relevant to pre-natal care. * Healthy Start Queens grant funded Prenatal Program Coordinators, must utilize the UniteUs and Go Beyond platforms to register and refer patients for home visiting, maternal child health and social drivers of health services.
