JOB FUNCTION:

Utilize SBIRT to provide brief interventions and referrals for patients with at-risk substance use.
Coordinate and schedule weekly case conferences for each medical provider.

POSITION SUMMARY:

POSITION SUMMARY:
Provides administrative support to Director of Psychiatry. To act as a psychiatric consultant on clinical issues to all areas of CHN including primary care and health homes. Eighty percent (80%) of the provider's time is spent conducting psychiatric evaluations and management of patients as well as providing consultations and education to clinical staff. JOB FUNCTION:

POSITION SUMMARY:
The Care Coordination Training Coordinator is an integral part of the Health Homes team. POSITION SUMMARY:
The Mother and Child Health Coordinator will implement a wide range of clinical and community health education activities for prenatal patients. As part of a multi-disciplinary team, the Mother and Child Health Coordinator is responsible for coordinating clinical outreach and education relating to prenatal health, specifically, breastfeeding, postpartum care, and reproductive health to improve health outcomes for pregnant people, new parents, and their children.
DUTIES AND RESPONSIBILITIES:
* Support the Population Health Department in maintaining quality standards and services that are in accordance with the recommended clinical guidelines for prenatal and postpartum services.
* Provide prenatal, postpartum, and infant care education to prenatal patients and people of childbearing age in the clinic setting, by telehealth, and in group settings.
* Conduct and document prenatal assessments, including but not limited to lactation assessments, safe sleep assessments, and depression screening.
* Support patients with obtaining durable medical equipment, as prescribed by the provider.
* Provide visits for postpartum patients up to 12 months postpartum to assess needs such as infant's initial milestones and SIDS/safe sleep risk.
* Identify high-risk prenatal patients at CHN health centers and follow-up to ensure patients made their medical appointments and reschedule as needed.
* Coordinate prenatal and postpartum appointments for routine/follow-up care, delivery, and connect patients to specialty services, particularly nutritionists and behavioral health services and external referrals.
* Conduct home visits (including but not limited to post-partum) as required pursuant to the terms

* Meet programmatic deliverables for the number of patients seen each week, conducting screenings, addressing concrete needs and education reviewed with the patients.

* Travel between health centers and balances schedule to provide education at multiple sites.

POSITION SUMMARY:
