Reporting to the Medical Director, this individual will be responsible for providing evidence-based Primary Care to our patients. This individual is an integral of the collaborative care team which includes nursing staff and other services such as Health Education, Nutrition, Social Work and Behavioral Health.
The ideal candidate will have a valid N.Y.S. License to deliver medical care and at least one (1) year of post-graduate/residency experience delivering care in a Primary Care setting.

Key Responsibilities:

Required Qualifications:
