POSITION SUMMARY:
The SocialWorker/MSW I is responsible for the social assessment of all adolescent patients; for intervention on behalf of any other patient who requests or is referred for social services. Responsibility also includes the mutual development and implementation of a plan designed to strengthen the life skills of that individual.
RESPONSIBILITIES/ DUTIES:
Essential Functions:
Assess client's social and emotional needs and jointly, with the client, develop a plan to meet those needs.
Provide concrete services to the client and family, or assist them in securing such services.
Record pertinent data in the client's medical record concerning services given.
Provide short-term goal-oriented social work intervention; a minimum of six cases will be carried by each socialworker for short-term counseling: statistics are to be submitted monthly.
Identifies outside resources for referrals and establish a working relationship (network or contractual); to monitor the effectiveness of contracted services and make necessary adjustments.
Conduct, after appropriate training, HIV universal counseling, including pre-and post-test counseling, on-going support and case management where needed.
Participate equally in the prenatal, family planning and primary care programs, including initial psychosocial on all patients, ensure appropriate financial screening. For adolescents, conduct an in-depth assessment to include adjustment to pregnancy, familial and social supports, parenting skills, etc.
Provide health education services; including individual and group sessions.
Participate in staff development opportunities.
Facilitate related services for health center patients and CHN staff as appropriate, with respect to their confidentiality and privacy.
Serve on committees or task forces, as assigned.
Co-facilitate group work services for special needs population.
Other duties as assigned.

Receives training to allow the provision of care and services support resources to trauma patients.
Responds to consults as required by policy

Identifying psycho‐social, environmental and financial problems that affect discharge planning.

Summary of position:
This position reports to the Deputy Director of Social Work Reentry within the Division of Correctional Health Services and is responsible for working with the Deputy Director on the management, supervision and innovative development of staff (socialworkers, case managers, administrative) who work to address the social service needs of patients on the mental health service upon re-entry into the community. Under the leadership of the Director, Life Enrichment, the SocialWorker is responsible for providing case management services and advocacy for residents while working with the interdisciplinary team to ensure quality care by following the policies and protocols of the Episcopal Church Home (ECH). Complies with the Code of Ethics of the National Association of Social Work.
Ensures the completion of the MDS 3.0 for social work per facility policy.
Ensures residents are provided with the least restrictive environment and medication regime to ensure a quality, resident centered experience at ECH.
Provides supportive contact to residents and family members as they acclimate to life at ECH; provides them with the opportunity to express feelings associated with long term care life.
Follows established departmental policies, procedures and standards related to including admissions assessments, supplemental admission documentation, routine progress notes, social work assessments, quarterly summaries, discharge notes, post discharge care plans, Mini Mental State Examination (MMSE), advanced directives (MOLST) and care plans within the established time protocols.

Coordinates appropriate discharge plans with community agencies to ensure safe discharge.

Involvement in the Ethics Committee as appropriate.
Coordinates room changes in accordance with the resident's Bill of Rights and departmental procedures.

Follows guidelines of HIPAA to ensure confidentiality of each resident.
