The Countywide Truancy Reduction Program supports youth throughout Kenosha County aged 10+ who are at risk for truancy and Youth Justice Court involvement. As the Truancy Reduction Worker, you will work closely with youth and their families to create individualized strategies and support services by providing concrete supports to youth and families. KEY RESPONSIBILITIES OF THE TRUANCY REDUCTION WORKER: Analyze client histories, capabilities, and needs to determine services required to meet treatment needs of client. Create, implement, and review individual client treatment plans. Monitor client attendance by conducting in-school checks on a daily basis. Organize and facilitate structured group, individual and/or family education/treatment and/or recreational activities three to four times a week. On a weekly basis, update parents/guardians on client progress and provide problem-solving interactions. Exercise judgment to intervene as an advocate for clients in emergency and crisis situations. Safely transport clients as needed. LOCATION: Kenosha, WI. Local travel will be required throughout Kenosha County to meet with clients within the school, home, or other community spaces. Essential Functions: 1. Maintains a caseload of approximately 8 children and families. 2. Contact enrolled children and families within 48 hours of enrollment, and set up initial visit to occur within 5 business days of enrollment to introduce self and supports available. 3. Schedule and attend weekly visits with assigned children and families, facilitate monthly Child and Family Team Meetings to discuss treatment planning, and maintain regular phone contact with assigned children and families and service providers as needed. 4. Assemble a Child and Family Team within two (2) weeks of enrollment by interviewing the family, identifying family members/natural supports/service providers and other significant persons. 5. Within the first 30 days, coordinates the meeting of the Child and Family Team and develops the initial individualized Plan of Care based on the identified strengths and needs, including a comprehensive 24-hour Crisis Plan. 6. Monitor the provision and quality of services provided to the family through the Child and Family Team and is the liaison when new services/resources need to be sought or developed. 7. Advocate for Children and Families across a variety of settings, including home, educational, court, and community settings, as needed. 8. Attend all scheduled court hearings as a representative of Wraparound Milwaukee to support children and families throughout the court process and explain supports available/in place. 9. Seek community resources with the assistance of the Team and modify the SAR whenever services or resources need to be added and/or deleted. 10. Provide or arrange for transportation for youth/young adults to appointments, crisis/respite services, including assisting a child and family with identifying natural supports and sustainable transportation plans, etc., if needed. 11. Provide or secure support and crisis/emergency services for youths enrolled and/or their families. This may be done through face-to-face contact, phone contact/ availability or an on-call system. 12. Completes all necessary paperwork in a strength-based manner per Wraparound Milwaukee/Agency requirements,(i.e., court letters, Plans of Care, change of placement forms, SARs, Referrals, Progress Notes, evaluation instruments, consent forms, etc.). 13. Maintain accurate information online, ensuring that family demographic information and Child and Family Team Member contact information is up to date. 14. Collaborate with other necessary individuals the youth and family may have contact with. 15. Keep Wraparound Milwaukee informed of all emergencies or critical incidents (refer to Wraparound Policy #14 - Critical Incident Reporting). Other Duties and Responsibilities: 1. Complete Comprehensive Crisis Plan within 1 week of meeting youth and families. Schedule and attend program-required visits, with assigned children and families, facilitate Child and Family Team Meetings (frequency depending on needs of youth: monthly as needed or at least within every 90 days) to discuss treatment planning, and maintain a minimum of weekly phone contact with assigned children and families and service providers, as needed. Complete wellness checks, send certified letters, and reach out to youth and families by phone call, text, and/or e-mail when youth and family lose contact with their Child and Family Team. Assemble a Child and Family Team within two (2) weeks of enrollment by interviewing the family, identifying family members/natural supports/service providers and other significant persons. Within the first 30 days, completes Youth CCS Assessment Summary, coordinates the meeting of the Child and Family Team, and develops the initial individualized Plan of Care based on the identified strengths and needs, including a comprehensive 24-hour Crisis Plan. PSG/CIP is a community-based social services organization seeking talented professionals like you to join our team. JOB SUMMARY: The Total Inter-Agency Model for Education (TIME) program was developed in 1985 as a self-contained, complete, alternative school to meet the needs of adjudicated, emotionally disturbed, and other high-risk youth who were unable to function in a conventional educational setting. Our philosophy, derived from Native American tradition, focuses on "The Circle of Courage" as the cornerstone of creating a positive educational climate for today's discouraged youth. As the Youth Worker, you will provide direct support to high-risk youth and their families to determine and implement the most appropriate educational options to meet each student's unique needs.
