The Community Inclusion Specialist (CIS) provides assistance to adults with developmental disabilities and/or autism. The CIS creates and facilitates outings designed to promote independence when accessing the community. Other objectives are to increase community involvement, develop new skills, build social ties, and connect participants with relevant opportunities to help meet their larger goals . Participants may spend time in the community engaging in anything from cultural activities, social/recreational activities, life skills-based excursions, job exploration, volunteer opportunities, more. The CIS may accompany up to 5 people at a time in the community, using public transportation or in their own personal vehicle.Responsibilities/Duties
Essential Duties and Functions

Supervise and oversee participants in the community on a daily basis to achieve their personal goalstake part in activities and/or events.

Meet the basic needs of our participants while in the community. This may include assistance with other Activities of Daily Living (ADLs) as required by the participant.

Implement ISP's (Individual Support Plans) and behavior plans, as needed.

Maintain sufficient knowledge of participants' ISPs and personal needs required to work with them in a meaningful and productive way.

maintain tracking of receipts and expenditures for reconciliation.

Complete all required paperwork, including but not limited tocase notes on a regular and timely basis.

Drive personal vehicle or use public transportation to assist individuals in accessing the community.

Attend to basic first aid, when needed.

Required Experience

One to three years' experience with people with developmental disabilities.Required Education

High School Diploma/GED, Bachelor's degree preferred Required Qualifications

Basic Computer skills using Microsoft Products.

The Community Integration Specialist assists and trains participants in making informed decisions and choices which allow them to exercise self advocacy and control over their lives. Under the direct supervision of the Day Habilitation Supervisor, an employee in this position is responsible for assisting and training in the areas of communication, life skills, social behaviors, personal skills, health care, and promoting self reliance to the extent possible.

This position is Monday - Friday, 8:00 am - 3:30 pm (35 hours), Full Time and Benefit Eligible. On-call employment is also available. ARE PART-TIME HOURS

Illustrative Examples of Work:

Attends meetings with participant and service coordinator.

Maintains program setting in a neat, organized manner; adheres to safety standards.

Brings to the attention of supervisor any changes in the health condition of the participant or any other pertinent information.

Immediately contacts medical department in case of injury, seizure or changes in participant's health status.

Submits written reports (Case Action Summaries) on significant events which have occurred to the participant in program, the community or residential setting.

As needed, assists participants in/out of van.

Assists participants, as needed, with toileting and personal hygiene.

Assists participants, as needed, in transferring/lifting.

Schedules activities based on participants needs and/choice.

Completes written assignments as required.

Prepares all required paperwork in a comprehensive and timely manner.
