Home Base, a Red Sox Foundation and Massachusetts General Hospital program, is dedicated to healing the invisible wounds - including post-traumatic stress, traumatic brain injury, anxiety, depression, co-occurring substance use disorder, family relationship challenges and other issues associated with Military service - for Veterans of all eras, Service Members, Military Families and Families of the Fallen through world-class clinical care, wellness, education, and research.
The Home Base Veteran Outreach Coordinator is a point of contact for veterans and their families seeking care and/or education regarding Post Traumatic Stress (PTS), Traumatic Brain Injury (TBI) and other combat-related stress. When not assigned to the Intensive Clinical Program, the Veteran Outreach Coordinator role changes to educate and provide outreach to New England-based veterans and their families about Post Traumatic Stress (PTS), Traumatic Brain Injury (TBI), and encourages the community to take advantage of services available through the Home Base Program. The Coordinator is an employee of MGH and works alongside a team of world class medical professionals to help educate veterans, their families, social workers, employers, community service providers, veterans' groups, and others as to how to recognize symptoms of PTS/TBI and the ways in which they or their loved ones can seek help. The Veteran Outreach Coordinator guides veterans through the treatment evaluation process in the Home Base Clinic and works closely with the clinical staff in the Home Base Program around patient case management and ongoing monitoring of patient needs. The Coordinator provides active patient outreach, including phone, email, in-person meetings and text messaging.

The Outreach Coordinator collaborates with community partners, healthcare professionals, and social service agencies to promote mental health awareness and address the behavioral health needs of individuals.

Essential Functions
-Conduct proactive outreach within the community to identify individuals in need of behavioral health services.
-Establish relationships with community organizations, schools, primary care providers, and other relevant stakeholders to enhance outreach efforts.
-Conduct initial assessments to identify the behavioral health needs of individuals.
-Provide information and referrals to appropriate behavioral health services and resources.
-Respond to crisis situations and provide immediate support and intervention.
-Collaborate with crisis intervention teams, law enforcement, and emergency services as needed.
-Advocate for individuals with behavioral health needs to ensure they receive timely and appropriate care.
-Conduct community education and awareness programs on mental health topics.

The Texas Outreach Coordinator will facilitate base-building for offshore wind with a primary focus on outreach along the Texas Gulf Coast. The person who fills this role will help to develop and execute a comprehensive community outreach plan, collaborate with and support the Gulf of Mexico Campaign Manager in implementation of priority campaign activities, cultivate new and existing partnerships, and represent NWF's Offshore Wind Energy team in work with partner organizations and at public events.
Tasks will include supporting coalition-building, organizing educational events, and helping create campaign materials.
Principle Duties (major areas of responsibility):
* Support planning and implementing state and regional Offshore Wind Energy campaigns:
* Plan and implement grassroots organizing and community outreach efforts in target communities along the Texas coast individually and with partner organizations, supporting partner and activist engagement
* Track campaign activities by writing timely reports as needed
* Represent NWF in coalition settings and at public meetings and events
* Regional Campaign Manager support:
* Identify and develop opportunities to collaborate with coalition partners
* Facilitate information sharing in state coalition meetings
* Support the production of coalition resources
* Fulfill limited administrative tasks as needed, including list building, note-taking and maintaining and organizing digital records
Qualifications:
* Passion for addressing climate change in creative and pragmatic ways that allow wildlife and people of all backgrounds to survive and thrive
* At least 1 year of campaign and/or organizing experience, either in a professional or volunteer setting
* Excellent verbal and written communication skills in English

Qualifications: The preferred candidate will have two or more years of experience in community engagement in support of plans and public policies related to issues of growth and development, urban design, environmental sustainability, or public infrastructure. Strong writing, public speaking, and group facilitation skills are a must. A degree in communications, public relations, urban planning, public policy, public administration or related field is preferred. Foreign degrees and credentials will be considered.
Compensation: The minimum starting pay is $70,000. The offered pay rate will be subject to experience and qualifications. GNRC provides a fully paid pension through the Tennessee Consolidated Retirement System and offers a dollar for dollar match to a 401k up to 3% of gross salary. Key Responsibilities
Responsible for assisting Members in navigating the healthcare system by finding and following up by phone and in person with hard-to-reach Members, helping Members successfully participate in their medical and/or behavioral health care by overcoming barriers to care, and sharing information on barriers with IEHP and Providers to improve the Enhanced Care Management team.
Model the highest ethical behavior in relationships with co-workers, supervisors, Members, Providers, and colleagues in the community.
Responsible for engaging with Members, both in-person and on the phone, in a manner that utilizes evidence-based approaches (such as Motivational Interviewing) that promotes collaboration between the Member and his or her medical/behavioral team, as well as to increase the Member's sense of control over their whole health.

Responsible for promoting a collaborative and effective working environment within the Enhanced Care Management team by engaging in evidenced-based communication strategies (such as Motivational Interviewing) when discussing responsibility/sharing of tasks, effectively resolving conflicts as they arise, and collaborating on Member case discussions.
Collaborates on Member care issues with other Enhanced Care Management Team Members and consults with Nurse Care Manager and/or the Behavioral Health Care Manager before taking any action that is clinical in nature.
Model commitment to continuous quality improvement by engaging in quality improvement initiatives and projects, such as identifying and addressing HEDIS gaps, and by identifying, developing, and testing new practices for improving the outcomes of the Enhanced Care Management team.

About this Role
As a Community Health Worker, you will be Waymark's frontline presence in the community, connecting with patients to provide social support, advocacy and navigation. You will work to improve the health of patients from low- income communities, by connecting them with various providers and resources. Key Responsibilities
Attend a 2-3 week long paid training program.

Role Description

Zócalo Health is looking for a Community Health Worker to work directly with our patients to help them navigate their health and social needs. You will work with an assigned panel of members dealing with unmet social needs and numerous health conditions. You will educate patients on disease prevention and healthy behaviors, coordinate comprehensive care by scheduling appointments and facilitating follow-ups, and address social determinants of health by connecting patients with essential community resources. Your work enhances patient advocacy and satisfaction and reduces healthcare costs by preventing unnecessary hospital visits. will contribute in the following ways:
Play an active role in patient registration and enrollment, including organizing community engagement and outbound calls to patients.
Conduct outreach (virtually and in-person) to patients scheduled for appointments and complete initial intake.
Engage with a panel of assigned patients to provide care navigation, appointment logistics, prescription drug support, lab support, referral coordination, care plan adherence, and resource sharing.
Assess for social determinants of health (SDOH) needs and enroll patients in SDOH programs, including care planning development, referral to community resources, coaching, and graduation planning.

Document all patient and care team interactions across multiple systems and tools.
