The remainder of the day is spent visiting patients in our day center medical clinic, attending interdisciplinary meetings, intake & assessment care-planning meetings, and office time for documentation. Required Education & Experience: Graduate of an accredited nursing school with at least one year of experience working with a frail elderly population, preferably in long-term care, home health care or community health setting. Experience teaching patients and other healthcare workers preferred. Required Certifications and Licensure: Licensed Registered Nurse in Pennsylvania. Valid Driver's License. Required Skills: Comprehensive knowledge of current nursing practice and theory. Knowledge of the medical, social, and emotional needs of a frail, elderly population. Effective written and oral communication skills. Strong organizational and planning skills; ability to manage multiple priorities. Must be able to work independently and to utilize critical decision making skills. Working knowledge of utilization review, quality assurance and managed health care concepts.     Schedule : Monday-Friday 7:30 am-4:00 pm Summary of Job Duties : The Intake Specialist (IS) provides benefits information, assessment and assistance to individuals and their families who are interested in enrolling in Community LIFE. The IS explains Community LIFE's benefits and services, and answers questions or concerns, and completes an assessment of medical, psycho-social and environmental conditions to further identify the needs and goals of potential participants and their family caregivers. The IS assists potential participants with gathering information needed for clinical and financial eligibility, works closely with Community LIFE's Medicaid Eligibility Coordinator, and monitors the intake process using a customer relations management system. The IS introduces participants to the Interdisciplinary Care Team, providing background information, and coordinating an in person orientation which occurs prior to enrollment. The IS must have excellent interviewing, record keeping, writing, and verbal presentation skills. Additionally, he or she must be able to represent the organization professionally to referral sources and the potential enrollee's family members and caregivers. Required Education: Associates degree in related field. Bachelor's degree preferred. Required Experience: Minimum of one year working with the frail or elder population. Two or more years working in community health, senior health, marketing, or enrollment for a public health, social service organization or health plan. Required Certifications/Licensing: Valid Pennsylvania Driver's License. Required Skills & Abilities: Ability to work effectively with culturally, economically and educationally diverse populations. Ability to communicate effectively and comfortably in public forums. Ability to prepare concise and clear reports on marketing and outreach. Superior assessment skills applying principles of logical thinking to determine the services that meet what the potential enrollee wants/needs. Active listening and interviewing skills; applying principles of asking the correct follow-up question to have the potential enrollee determine what they need/want. Empathy and compassion for the aging population and their needs. Ability to form positive interpersonal relations with a wide range of staff as well as external contacts. Goal-driven with a competitive edge. Well organized, and able to manage multiple projects simultaneously. Excellent time management. Able to thrive in a fast-paced, changing environment. Computer literacy including ability to use Microsoft Word, Excel, and Outlook. Proactive, forward thinking. Ability to work with minimal direction and still meet deadlines. Ability to promote the program in a positive, patient approach that builds trust in the Community LIFE brand and contributes to continued growth and expansion for the company.
