    Job Description Schedule: Part Time, 3 days per week (7:30am-4:00pm) Summary: Our Social Workers maintain a caseload of Community LIFE participants (adults over age 55) and provide them and their families with psycho‑social assessments and interventions related to the maintenance of the participants in the community. As the primary contact, the Social Worker performs utilization case management for all acute inpatient admissions and transfers. Required Education: Master's Degree in Social Work (MSW) from an accredited school of social work. Required Experience: Must have a minimum of one year experience working with the frail or elderly population. Geriatric social work experience in a hospital, long term care or community setting. Required Skills: Good verbal and written communication skills. Ability to work with people from various socio‑cultural backgrounds in an effective manner. Ability to accept and utilize supervision. Ability to work independently and be self-directed. Good problem-solving skills. Must have a compassionate and empathetic attitude for the frail older adults.     Schedule: Part Time, 3 days per week (7:30am-4:00pm) Summary: Our Social Workers maintain a caseload of Community LIFE participants (adults over age 55) and provide them and their families with psycho‑social assessments and interventions related to the maintenance of the participants in the community. As the primary contact, the Social Worker performs utilization case management for all acute inpatient admissions and transfers. Required Education: Master's Degree in Social Work (MSW) from an accredited school of social work. Required Experience: Must have a minimum of one year experience working with the frail or elderly population. Geriatric social work experience in a hospital, long term care or community setting. Required Skills: Good verbal and written communication skills. Ability to work with people from various socio‑cultural backgrounds in an effective manner. Ability to accept and utilize supervision. Ability to work independently and be self-directed. Good problem-solving skills. Must have a compassionate and empathetic attitude for the frail older adults.
