Position Summary: This position works with clients to support them in achieving their highest possible level of independent functioning and stability while facilitating recovery in a strengths-based model. In this role you will be responsible for providing direct clinical therapy services to clients presenting with a wide range of mental health concerns and diagnoses. Therapists will conduct client assessments, participate in care planning, provide individual therapy, assist with the facilitation of services, engage in group supervision, and support continuity of care upon discharge from the program. Services may be provided in the client's home, through telehealth or at Community Living Alliance's office. Primary Duties: Provide ongoing counseling and support to assigned clients. Create and manage a treatment and assessment plan. Provide back up to team-assigned clients. Provide MHP consultation with team for CCS clients and/or consultation and education to CLA team members and providers working with client. Provide necessary support for individuals working toward recovery using a supportive, strengths-based, culturally humble approach to treatment. Maintain documentation within CLA and county/regulatory information systems including but not limited to case notes, goal development, and client service/goal related activities. Participate/collaborate in client meetings and service coordination referrals. Utilize effective therapeutic strategies to engage client, enhance motivation, and encourage meaningful, collaborative and strengths-based goal planning and progress. Provide conflict resolution/mediation, lifestyle coaching and support, and advocacy with individuals. Assist the client with identifying and establishing natural and professional supports in the community. Education/Experience and Requirements: Required Master's Degree in Mental Health, Human Services, or Related Field 2 years of related experience providing community based mental health services and knowledge of individuals with physical and developmental disabilities and other chronic conditions Wisconsin Licensed Clinical Social Work (LCSW), Licensed Professional Counselor (LPC), or Licensed Marriage and Family Therapist (LMFT), Advanced Practice Social Work (APSW, Substance Abused Counselor (SAC) or Relevant/Equivalent License Must own or have access to reliable transportation to commute to the CLA office, client homes, vendor locations, community partner offices, and other relevant locations to complete essential functions of the position. Must possess a valid driver's license, current WI auto insurance.
