Currently, we have part time positions in our community living support program with varying hours. We have short shifts as little as 2-3 hours or longer shifts based on the needs of the client. We make every effort to work around your schedule. Must be able to work some weekend shifts. Must be able to communicate effectively and professionally You must have reliable transportation and valid driver's license You must pass employment, criminal background, and Motor Vehicle checks as a condition of contract employment. Attend University where client/resident served is enrolled. Must be able to lift 50 pounds Essential Functions: Meal preparation, laundry, light household chores. Activities of daily living (such as bathing, grooming, dressing, etc.). Keep person served safe and continue to monitor health and safety. Complete documentation requirements (i.e. Progress notes, data logs, timesheets, etc.) which reflect goals and objectives worked on with person served. Encourage each person served, where appropriate, to engage in recreational, entertainment or educational activities, such as games, reading, movies, outdoor activities, arts and crafts, etc. Participation and assistance at medical appointments. Transport and attend classes with person served as needed. Knowledge of Culture of Gentleness/Gentle Teaching Others duties as assigned.     Medical Coordinator Community Living Centers, Inc. is seeking a detail-oriented, conscientious, and dedicated individual to serve as a Medical Coordinator in one of our AFC- Group Homes. The AFC-Medical Coordinator is responsible for coordinating and monitoring all resident medications, keeping accurate medical records, making all medical appointments, and transporting residents to appointments. This position requires a responsible, caring person, and patient person who loves working with people. Community Living Centers, Inc. provides support to adults that have developmental disabilities in a residential AFC environment. Our goal is to provide safe and comfortable homes that are welcomed in their communities. FULL JOB DESCRIPTION GENERAL STATEMENT OF RESPONSIBILITIES: Responsible for coordinating all medical and follow-up appointments and completing all required documentation. Provide direct hands-on care and assistance in daily living to individuals living within assigned Residential Group Home. Understand and execute the person-centered plan (PCP) and individual plan of service (IPOS). GENERAL FUNCTIONS: This position description is intended to be a general statement about this job and not to be considered as a detailed assignment. SERVICE FUNCTIONS: Maintain close contact with each resident regarding medical needs; is thoroughly knowledgeable of all medical conditions, medications, and maintains contact with all physicians, dentists, psychiatrists, etc. Attends all medical appointments with residents and completes al required paperwork. If there are two (2) appointments at the same time, the medical coordinator will ask for assistance from the Group Home Manager who will help coordinate the appointments. Checks in all medication for the group home to ensure that all medications have been delivered for all residents. Checks all medication sheets against resident's physician prescriptions to ensure that all medication sheets match the prescription. Be on call 24 hours a day for emergencies and is expected to work evenings and weekend hours as needed or scheduled by Residential Group Home Manager. Attend Person Centered Planning meetings and provides information regarding the resident's health status. Communicate with staff and outside agencies regarding the resident's health status. Provide hands-on care and assistance to individuals we serve including but not limited to mobility/lifting/ transferring, bathing, personal hygiene and grooming, laundry, meal preparation and mealtime assistance. Assist individuals we serve with activities in daily living including but not limited to household tasks, communication skills, and activities in the community. Assist in development of individuals' social and recreational interests and skills through appropriate modeling and supportive activities. Pass medications in accordance with DOCEBO training and physician's orders. Cooperate with other Group Home staff to assist in meeting the needs of individuals in the Group Home. Transport individuals and drive agency vehicle as needed, however if agency vehicle is not available it is expected that transportation of the resident to an appointment will occur in staff's personal car. Assure basic cleanliness and care of agency vehicle. Responding to emergency situations and maintaining timely communication with the Group Home Manager, Directors and co-workers. Currently, we have full time positions for Days (6a-2p) and Afternoons (2p-10p) Must be able to work flexible shifts including weekends. Must be able to communicate effectively and professionally You must have reliable transportation and valid driver's license You must pass criminal background, and Motor Vehicle checks as a condition of contract employment. Have a high school diploma or GED Must be able to lift 50 pounds Locations Include: Farmington, Farmington Hills, Royal Oak, and Southfield DUTIES Essential Functions: * Meal preparation, laundry, light household chores. * Activities of daily living (such as bathing, grooming, dressing, etc.). * Keep Consumers safe and monitor health and safety. * Take residents on outings * Participation and assistance at medical appointments. * Transportation Ability * Knowledge of Culture of Gentleness/Gentle Teaching * Other duties as assigned. QUALIFICATIONS Qualifications include: * Must be able to communicate effectively and professionally * You must have reliable transportation and valid driver's license * You must pass employment, criminal background, and Motor Vehicle checks as a condition of contract employment. * Have a high school diploma or GED * Must be able to lift 50 pounds Required training: * State Mandated Trainings i.e. VCE/DCW, DOCEBO-MORC * Bloodborne Pathogens * CPR/First Aid * Recipient Rights * CHAMPS MUST HAVE A CURRENT AND VALID DRIVERS LICENSE
