Accurately complete all required documentation, including but not limited to, goal completion, actions toward outcomes, incident reports, timesheets, etc.
• Complete daily progress notes and communication log to assure appointments, goals, and interests are met and/or daily documentation.
• Assist in maintaining all necessary records, daily attendance, check sheets, production sheets, etc.
• Substitute in-house or in the community as demands occur.
• Assist new staff and/or current staff with orientation, mentoring, and training.
• Attend and actively participate in meetings and training as required. Maintain certification in all agency, state and federal training requirements.
• Demonstrate knowledge of and comply with all agency policies and procedures, as well as state and federal statutes and regulations related to specific program areas.
• Follow service definition guidelines for services being provided.
• Perform all other duties as assigned by the supervisor

Primary Functions
As MAP's Behavioral Health Peer Support Specialist, your primary focus will be to offer lived experience with behavioral health conditions and provide support focused on advocacy, education, mentoring, and motivation. Our Behavioral Health Peer Support Specialists are trained to advocate for their peers through the process of behavioral health recovery in ways that are specific to the needs of each individual.
Responsibilities
Provide routine, practical, real-world support in coping with life's day-to-day demands through modeling and coaching.
Relaying personal, lived experience of your own mental health recovery based on an established plan via telephone/telehealth with peers.
Oversee a personal caseload of peers, their families, and/or support system through consistent telephone/ telehealth sessions.
Maintain timely and accurate documentation of patient encounters and data.
Effectively manage call queues and pre-scheduled calls in a timely, engaging, or meaningful manner.
Participate in an on-call rotation with team members for MAP's 24/7 after-hours line.
Assist peers to identify additional resources to aid in their holistic recovery goals.
Assess any client issues, escalate as needed, and collaborate with leadership for solutions.
Maintain a professional and courteous image to MAP's patients and fellow employees.
Participate in a well-paced and succinct manner in all staffing and company meetings.

The Family Centered Treatment Specialist provides quality psychotherapeutic services to children, adolescents, and families utilizing the Family Centered Treatment model.
Provide Family Centered Treatment (FCT) Services as per the service definition and model fidelity requirements.
FCT Certification is required and must be completed within one year of employment in order to continue to provide FCT services and maintain employment with Health Connect America (HCA).
Complete FCT Checkoffs and other internal trainings within designated deadlines. Trainings are assigned and monitored through an e-learning system.
Deliver FCT services to children, adolescents, and their families, primarily in their living environments, with a family focus.
Provide 24/7 on-call coverage to a caseload of families and children. Service is regularly provided in the home and community and will require service provision at the convenience of the child and family requiring a flexible work schedule.
Complete all required Clinical Records and FCT documents with the child and family and ensure the documents are entered/uploaded timely in the Electronic Health Record, according to company standards.
Develop Service Plan in coordination with Treatment Team Process and maintain the plan per designated authorization processes in the clinician's area of service. Work collaboratively on behalf of the child and family with the identified Treatment Team members to ensure effective communication and coordinated service provision.
Conduct ongoing revisions of Service Plan and is responsible for implementation of Service Plan.
Provide psychoeducation as indicated in the Service Plan.
Conduct interventions with the family aligned with the FCT model to include facilitating the movement of the child and family through the four phases of treatment and completing all fidelity documents.
Assist the families and children to establish weekly goals and help them to identify the steps that need to be taken to reach the established goals.
Make referrals to services and resources on behalf of the families and children.
Develop and coordinate discharge plan based on treatment, indicating all relevant aftercare needs and plans. Access community resources that would be beneficial to client aftercare needs.
Coordinate and oversee the initial and ongoing assessment activities.
Convene the Treatment Team for Service Plan.
Consult with identified medical (e.g., primary care and psychiatric) and non-medical (e.g., Dept. of Child Services, school, Dept. of Juvenile Justice) providers; engage community and natural supports and include their input in the person-centered planning process.
Ensure linkage to the most clinically appropriate and effective services including arranging for psychological and psychiatric evaluations.
Provide and coordinate behavioral health services and other interventions for the youth or other family members with other behavioral health professionals and Treatment Team members.
Monitor and document the status of the consumer's progress.
Participate in Quality Assurance/Quality Improvement activities to include audits, corrections and designated improvement plans and projects.
Qualifications
Education & Experience:
Minimum of a Bachelor's Degree in human services or related field is required.
Two years' experience working with youth or families.
Current Family Centered Treatment Certification is preferred.

WORKING HOURS
CNHC's work week is Monday through Friday, 40 hours per week. Your supervisor will advise you of your work hours; however, business office hours are 8:00 am to 5:00 pm. CNHC's group homes operate with 24 hour a day awake shift staff, 7 days a week, including weekends and all holidays. The supervisor for each group home will coordinate a part-time schedule based on the required available shifts.
TRAVEL
Travel in and outside of Circuit 13 (Hillsborough) may be required.
TRAINING
All employees must complete fifteen (15) hours of on-line or classroom Training on an annual basis. If the employee is a Certified Child Welfare Professional, Training hours must total twenty (20) annually. In addition, group home employees must complete all trainings required by the Department of Children and Families and Children's Network of Hillsborough in relation to working with dependent populations in a residential group home setting.

How you'll contribute
A Clinical Rehabilitation Specialist who excels in this role:
Educate the community on acute rehabilitation to develop a census through face-to-face contacts.
Develop business based on the strategic goals of the rehabilitation program.
Face-to-face connections within the territory to build relationships with referral sources to increase census.
Identifies barriers to the admission process and creates solutions with the assistance of the program director.
Requires onsite and in-territory work through face-to-face contact with patients, families, and referral sources.
Completes in-person in-services and presentations to educate on acute rehabilitation programs and services.

What we're looking for
At a minimum, should hold a 2-year degree. A graduate holding a four-year degree from a college program with a bachelor's degree in a health related, business or marketing area of concentration, nursing preferred.
Previous clinical liaison/marketing/sales experience preferred
Applicable and current license to practice in accordance with the state licensure act
Other professional licensure considered based on experience and training as outlined in the state licensure act
Valid driver's license and own reliable transportation required
A track record of successful experience in a health care setting, strong interpersonal skills, and experience working with physicians, nursing management, discharge planners, case managers, and social workers
Clinical experience is preferred
Approximate time of external liaisons to be in the market territory: 80%

Job Description
SPECIFIC RESPONSIBILITIES:
• Establish and maintain effective working relationship with all MYCS consumers, striving to exceed their expectations for service, quality, and individuality. Individuals shall be recognized for their valued role within their community.
• Plan, participate in, and conduct customized individual or small group activities and instruction, which serve to maximize each individual's potential, as well as providing the tools necessary to make good choices and develop appropriate skills to foster independence as they grow and develop within their community. This includes paid work activities/training that require staff assistance.
• Learn, implement and document the Individual Support Plan (ISP) for each consumer.
• Complete incident/other reports and documentation in accordance with agency/program, county, and state regulations.
• Develop, implement and monitor goal plans as needed, according to agency/program policies and procedures. Goal plans shall be respectful of individual interests and aptitudes. Goals shall serve to minimize artificiality and restrictiveness, while maximizing holistic, integrated supports. Goals shall serve to connect the fabric of work, home, social and recreational needs and desires.
• Demonstrate proper judgment and decision-making skills when relating to consumers, their families and team members as specified by program/agency policies; consistently present a professional demeanor in regard to dress and behavior.
• Respond to individual's verbal and non-verbal communication, providing regular and consistent opportunities to express their gifts and capacities.
• Act to ensure safety of individuals, co-workers and self at all time by:
- maintaining a clean, hazard free environment
- responding immediately to crisis situations, such as accidents, illness, seizures or aggressive behaviors
- using verbal management and physical intervention techniques when appropriate
- anticipating crises effectively using knowledge of individual's likes/dislikes, stressors and communication style
• Provide a full range of assistance with tasks related to individuals' personal care: grooming, toileting, feeding, and transferring/lifting as needed. These services shall be provided while allowing the highest possible level of discretion and personal dignity.
• Provide the individuals served with a full range of opportunities to participate in everyday recreational/leisure activities from which to choose. Preferred activities are those which reflect the individual's interests and shall be age-appropriate and optimally integrated within the community.
• Attend required/agency program meetings, as well as communicate relevant information/new ideas in a professional manner.
• Provide transportation for consumers as required.
• Participate in approved annual trainings as specified by county/state regulations and MYCS policy.
• Must have mobility and flexibility (such as running, bending, lifting, climbing stairs, etc.) as to ensure safety and wellbeing of individuals served and of self.
• Establish and maintain productive work relationships with individuals served, co-workers, families, and all parties encountered while acting as a representative of the agency. This relationship shall promote a positive, team-oriented work environment.
REQUIRED MINIMUM QUALIFICATIONS:
• Education/Certifications: High School diploma or GED required.
• Experience: One (1) year experience working with individuals with disabilities.
• Clearances: Act 33 and Act 34.
• Driving: Must be 21 years of age or older with a valid PA driver's license and meet the guidelines to be accepted on the agency's vehicle insurance policy.
• Medical: Upon hire, provide a current physical and Mantoux test, and then ongoing as required by the Department of Public Welfare (DPW).

This position is a direct service role involving tasks of daily living, behavioral and emotional support, as well as community engagement. We help our clients maintain a safe and healthy living environment by assisting with housekeeping and organization. Summary
Home Based Residential Supports direct support staff are responsible for providing high quality direct care services to an adult with developmental disabilities and high medical needs. Each direct support staff member is responsible for ensuring the human rights of all Individuals served are met. Staff are responsible for treating all Individuals with dignity and respect.
Direct Support staff assigned to the individual are responsible for supporting the Individual in the completion of their daily routines. Direct Support staff are responsible for ensuring Individuals are provided with opportunities to learn and exercise increasing independence as much as practicable.
Essential Job Functions
* Responsible for the health, safety and wellbeing of the Individual served, seeking assistance and medical interventions as appropriate.
* Serves as a Mandated Reporter and is required to advise direct supervisor, DDS and DPPC immediately if abuse and/or neglect of an Individual is suspected.
* Handle crisis situations according to agency policies and procedures, seeking resources when necessary.
* Monitor home site for safety, cleanliness and upkeep and take necessary action to ensure same. Report safety concerns per established protocols.
* Establish rapport with Individual participating in the Home-Based Residential Supports program.
* Support the learning of the Individual by use of motivational techniques, effective interventions and various communication approaches including verbal, gesture, and written communication.
* Assist the Individual with any activities of daily living (ADLs) as applicable to each Individual's abilities.
* May be responsible for scheduling and accompanying individuals to medical appointments/treatments, ensuring accurate reporting to physician and follow through with respect to doctor's orders and recommendations.
* Responsible for producing accurate, legible and complete medical documentation for Individuals served, including but not limited to treatment charts and data sheets.
* Ensure Individuals are provided nutritionally sound food options along with the education to help them understand the impact of their food consumption.
* Carry out all interventions in an Individual's Behavioral Protocol Plan as written and trained. Redirect and/or physically support Individuals who may be aggressive using proactive techniques while maintaining self-control.
* Communicate effectively, professionally and respectfully with Individuals, co-workers, management, guardians, and others while on company time.
* Responsible for being familiar with and abiding by Agency, Department, and Program policies and procedures and for acting in a manner consistent with organizational values, mission, and goals.
