This position reports to the WIC Program Manager. SPECIFIC DUTIES: * Promotes the mission, vision, and values of the organization. * Develop and implement education materials for teaching. * Assist the WIC Program Manager in the development and implementation of the annual Nutrition Services Plan (NSP). Ensure all sites have the needed lesson plans and nutrition education materials needed to conduct classes and individual education. * Provide staff training on nutrition education lesson plans & methods of education. * Develop nutrition education classes and materials for WIC participants. * Develop and deliver educational in-services for staff enrichment. * Observe staff in individual and group sessions for accurate and appropriate delivery of effective education. * Evaluation of participant feedback as needed to make appropriate changes to nutrition education being given. * Provides nutrition education to WIC clients in individual and group settings. * Provides individual evidence-based counseling techniques and develops care plans for high-risk WIC participants and develops nutrition plans to support goals, and documents it in WIC's documentation system by the end of the day. * Develops nutrition plans to support goals and documents them in a complete, concise and appropriate manner. * Coordination of National Nutrition Month activities for all of Community Medical Centers * Makes appropriate referrals and follow-up appointments. * Communicate with local vendors regarding WIC-specific shopping or customer service issues as needed. * Promote breastfeeding as the norm to all pregnant and postpartum WIC participants, CMC patients, and their families. * Demonstrate use and issue breast pumps to breastfeeding participants. * Provides breastfeeding support for WIC participants. The overall goal is to reduce the risk of hospital remission and to avoid unnecessary emergency department visits by navigating the patient through the healthcare system. The PSN reports to the Patient Navigation Center Supervisor or designee. SPECIFIC DUTIES: * Promote human milk feeding as the norm to all pregnant and postpartum patients/participants and their families or support persons * Assesses and counsels patients/participants, and families, with complex lactation concerns * Communicate with providers regarding patient condition and document individualized care plan in the electronic health record (EHR) or WIC WISE system * Assist in maintaining and tracking lactation supplies inventory * Assist in developing lactation education materials * Follow-up with patients/participants with subsequent appointments or by telephone/video * Demonstrate use and/or issue manual or electronic breast pumps * Participates in professional meetings, trainings, and continuing education * Participates in quality improvement measures for perinatal and/or lactation outcomes * Provides participant/patient centered counseling using motivational interviewing techniques * Acts on all referrals and warm hand-offs from other health care or WIC team members regarding complex lactation situations beyond their scope of practice * Works within the scope of practice as defined by the International Board of Lactation Consultant Examiners (IBCLE) and follows Standards of Practice as defined by their professional association, International Lactation Consultant Association, and adheres to policies, procedures, and guidelines of care * Schedule patients for appointments needed other services outside of the IBCLC scope of practice including but not limited to social worker, dietitian, nurse, or medical provider * Supports baby friendly health center initiatives including competency-based employee trainings on infant feeding appropriate to the departments of practice * Facilitates classes education to patients/participants and their families or support persons * Travel to different sites as needed * Perform other duties as assigned

SPECIFIC DUTIES: * Develop a clinical risk and infection control strategy in conjunction with leadership including objectives, identification of key risks, and controls that are aligned with policies, procedures and operating principles to ensure a culture of safety, timely capture of events, and initiation of risk analysis for appropriate events. * Develops and coordinates in collaboration with the Associate Director of Nursing, Chief Nursing Officer, Director of Quality Improvement, and General Counsel and other senior leadership clinical risk management processes to * Provides oversight of the incident reporting system and processes * Review and evaluate related policies and procedures and recommend revisions as needed. Monitor and take subsequent actions to ensure learning, compliance, and documentation * Analyze for leadership all statistical reports and advisories that identify clinical risk management, infection control, and/or patient safety patterns and trends. * Keep up to date with new and revised state and federal regulations and statutes related to hospitals and patient care. * Serves as staff resource in remodeling/set-up activities to assure infection control measures are taken when developing and/or changing work environments * Functions as Safety Officer in absence of Director of Environmental Safety * Actively participate in a variety of committees as assigned. * Performs other duties as assigned.
