This individual provides psychiatric assessment and services to severely mentally ill adults at Community Support Services, Inc. The Psychiatrist functions as liaison between CSS, the medical community and the State facilities: assists other clinical staff in developing individualized service plans for individuals served by the agency. What You'll Do: * Provide psychiatric evaluations and medication evaluations. * Administer somatic therapies, including prescriptions and periodic review of medications, including drug interactions and medication side effects * Monitor the physical health of agency clients using appropriate labs and evaluation of movement disorders. * Perform assessments of mentally ill adults through reviewing clinical records and attaining collateral information. * Lead interdisciplinary treatment team(s). * Provide clinical consultation to professional and paraprofessional staff. * Assist in reviewing the effectiveness of adult programs. * May provide liaison with medical community, CSS and State facilities. * Maintain safety and security of clients, while ensuring the use of least restrictive methods. * Determine the need for, and request referrals to obtain services of other medical specialists. * Provide coordination of treatment for the physical health of clients for services being provided by CSS or in the community. * Examine and certify patients requiring involuntary psychiatric evaluation pursuant to the provisions of the Mental Health Code. Typical Workday and Duties: * CSS business hours are 8:30 am to 5:00 pm on weekdays. There is no expectation for evening or weekend work. * 8:30 - 9:00 am - Administrative Time * 9:00 am - 12:00 pm - Scheduled Patients * 12:00 - 1:00 pm - Lunch/Administrative Time* * 1:00 - 4:00 pm - Scheduled Patients * 4:00 - 5:00 pm - Administrative Time * *Psychiatrists attend medical staff meetings from 12:00 - 1:00 pm twice a month. * There are currently no offsite assignments, including hospitals. * There is no initial supervision of residents or APNS. These responsibilities are assigned as needed and only after adequate experience with the agency is gained (1 and 2 years respectively). * During your initial two weeks of employment, you will spend time shadowing other providers, completing orientation responsibilities, and learning the basic functioning of the clinic/agency. * When you begin seeing patients on your own, you will initially have an hour scheduled for each visit, allowing you extra time to get to know your patients, review charts, and complete your notes. * After your initial meeting, existing patient follow-up visits are normally scheduled at 30-minute intervals, and psychiatric evaluations are scheduled for one hour. The Opportunity: We are seeking to hire a Housing Retention Specialist to join our Homeless Outreach Team. This individual is responsible doing aggressive case management of persons who are newly housed, mentally ill, and recently engaged with a mental health system. The Housing Retention Specialist will engage individuals who are newly housed in community services and assist with maintaining housing, entitlements, and community support. What You'll Do: * Continuously and independently provide case management to individuals exiting homelessness by assessing client needs, evaluating treatment options, coordinating care, and gauging progress in housing maintenance and stability. * Assess persons who are homeless to determine appropriateness for service, and then either develop rapport to help persons to be served in their environment or connect them with other resources through referrals. * Work closely with other social services agencies that provide services to those who are homeless or newly housed and at risk for homelessness, and accept referrals from these agencies. The Opportunity: Under the general supervision of a Clinical Supervisor the Community Rehabilitation Specialist (CRS) provides ongoing recovery focused community rehabilitation services to individuals with severe and persistent mental illnesses primarily residing at one of the site based Permanent Supportive Housing projects (PSH). The CRS is a service broker, coordinator, monitor and planner of services, offering practical help, building community support, teaching recovery-based skills and providing active outreach, advocacy and crisis assistance based on the strengths, needs, abilities and preferences of the individuals. What You'll Do: * Monitor and coordinate delivery of necessary evaluations (e.g., psychiatric, medical, vocational), surveys and assessments in order to identify each person's needs. * Coordinate and participate in the development of an Interdisciplinary Service Plan (ISP) that shall include services and assistance necessary to achieve specific objectives, as well as an explanation of the availability, intensity and duration required of each service. Write up the "ISP" and distribute to necessary participants for approval and sign-off. * Assist individuals to identify and minimize the effects of the psychiatric symptoms, enabling individuals to self-manage their illness. * Assist individuals to develop/increase interpersonal and community coping skills individually or in groups. * Review and monitor services and activities and note progress. * In cooperation with the individual, reassess strengths, needs, abilities and expressed preferences as required by the "ISP" standards specified in Rule 5122-27-04 of the Administrative Code, and/or at the request of the individual. * Available to the individual to provide, coordinate and/or assist in crisis intervention and stabilization services during regularly scheduled working hours and available for "after hours" emergencies in accordance with established agency on-call procedures. * Assist the individual in achieving the objectives in the ISP and maximizing independence and productivity through support and training in the use of personal and community resources. * Assist the individual in increasing social support networks of relatives, friends, and/or volunteers that ameliorate life stresses resulting from the individual's disability. * Accompany individuals to appointments and activities and assist in daily living skills training as needed. * Provide rehabilitation activities in community settings as appropriate to the needs of the individuals. * Assist the PSH project in ensuring residents are compliant with occupancy rules established by the property managers and Akron Metropolitan Housing Authority. * Provide input to Clinical Supervisor on planning residential activities that will support the goals of residents living independently. * Participate in Resident Council meetings if requested by Clinical Supervisor * Maintain relevant documentation and provide statistical reports in accordance with agency documentation standards. * Attend regularly scheduled staff and supervisory meetings. * Responsible for being knowledgeable about rights of individuals and procedures governing them. * Observe all rules of confidentiality as it relates to clinical information both internally and in dealing with outside individual(s) and/or agencies. * Participate in ongoing agency and staff training development programs and become familiar with all relevant agency policies and procedures. * Assume other duties and responsibilities as assigned by supervisor, which may include serving on various agency committees. The Opportunity: We are seeking to hire a Residential Aide to join our Residential Treatment Teams. This individual will provide assistance to residents in various residential settings in the community and will provide training and support to the extent determined by each individual's service plan. This role is different from many Residential Aide positions as these are hands-off facilities - there is no expectation for bathing, dressing, grooming, etc. of residents. This individual is responsible for preparing financial statements and analyses that help support the achievement of the agency's financial objectives and maintaining the integrity of the CSS computerized accounting system. What You'll Do: * Assist in maintaining the integrity of general ledger. * Contribute to the completion of timely monthly and annual financial reports and statistics detailing financial results. * Prepare journal entries to the general ledger. * Evaluate subsidiary journals for obvious errors and omissions and enter corrected data into the general ledger. The Opportunity: We are seeking to hire an Accountant II - Purchasing to join our Finance Team. This individual performs a variety of complex accounting and administrative tasks to include accrual accounting, expense monitoring, recording daily transactions, monthly reconciliations, and supply/inventory tracking. The Opportunity: We are seeking to hire a Case Manager - ACT Team. This individual provides ongoing community psychiatric support treatment services for a select group of clients who have a serious and/or persistent mental illness co-occurring with substance abuse disorder. This Case Manager will provide training, assistance, and support to help this client receive wrap-around rehabilitation services in the community. Our ACT Teams work together, meeting frequently, to facilitate an integrated, multidisciplinary team approach to service regarding progress, general problems, and concerns pertaining to clients served and their community adjustment. What You'll Do: * Collaborate with the client and the Service Coordinator to develop treatment plans that shall include services and assistance necessary to achieve recovery goals, including an explanation of the availability, intensity, and duration of each service. * Implement programming according to the developed treatment plan, participating in the ongoing assessment and therapeutic engagement of ACT team clients. * Make observations and document progress in accordance with established agency procedures. * Assist clients in developing daily living skills, including teaching and assistance with medically necessary services such as laundry, cooking, cleaning, communication and social skills, recreation, grocery shopping, personal hygiene, and self-care skills. * Monitor medications in accordance with agency procedures to ensure that proper dosages are taken as prescribed. * Assist clients in the proper budgeting and use of their funds, acting as an advocate for clients to secure needed services and financial entitlements. * Perform on-call duty and weekend coverage on a rotating basis. * Maintain relevant documentation. * This position is full-time (40 hours a week) working normal business hours.
