The Case Manager is responsible for providing case management to participants. Provide advocacy, support, and referral information and assist individuals or families to overcome barriers to become self-sufficient.
Essential Duties & Responsibilities:
Complete intake and needs assessments with each participant, which may involve a broad range of areas including housing, finance & budgeting, employment, education, food & nutrition, school & daycare, legal issues, health, parenting, and daily living skills.
Interview and evaluate clients to formulate Individualized Service Plans based on Program requirements. Work closely with clients to identify and build upon strengths and develop strategies to address barriers and concerns identified through the assessment process.
Help clients build self-sufficiency plans. Assist clients to transition to self-advocacy roles.
Advocate on behalf of clients and attend administrative hearings if necessary.
May perform home visits and/or unit inspections.
Establish, evaluate and monitor goals for clients linked to measurable outcomes. Work collaboratively with interdisciplinary team of collateral providers and partners to ensure coordination of services.
Maintain the structure and policies of the program through orientation of participants by reviewing program rules, policies, and case management visits.
Maintain accurate case records by documenting all services rendered including up to date case notes, telephone contact logs and referrals. Enter required data into the agency databases with reporting as required.
Provide emergency response management to address any crises or safety issues that may arise.
Accompany and/or transport participants to appointments as necessary. Assist families with arranging appointments.
On-call duties as assigned.

Qualifications:
High School Diploma or equivalent required. Bachelor's Degree in related field preferred.
Minimum of one-year assessment, advocacy and case management work in a Human Service field required.
Knowledge and experience working with low income, culturally diverse, and at-risk families and individuals.
Demonstrated knowledge of community resources available to low-income individuals and families.
Excellent customer service, organizational, oral and written communication skills.
Ability to manage multiple priorities and work in a fast-paced environment.
Ability to work and travel between Bedford, MA and Lowell, MA locations.
Must have reliable transportation.
Proficient computer skills required in basic computer software and data collection systems.
Excellent crisis management skills.
Ability to maintain accurate records and confidentiality of information.
Excellent attention to detail.
Valid driver's license.
Bilingual English/Spanish strongly preferred.

Primary Responsibilities:
Assist in the overall payment process of the RAFT program.
Serving as a primary point of contact for payment related inquiries from internal staff, community partners, property owners, and external agencies.
Provide timely, accurate information about program guidelines, eligibility, benefits, and required documentation.
Monitor and track program data, ensuring that all applications, payments, reallocations, terminations, and funding requests are processed accurately and within EOHLC timelines.
Attend all RAFT related meetings and provide ongoing training, technical assistance, and program updates to staff, ensuring consistency and accuracy.

Problem-solve landlord issues, missed payments, and unit-related challenges to reduce risk of housing loss and ensure timely supports for families.
Create and maintain both digital and hard-copy client files, following SMOC's document retention standards and EOHLC compliance requirements.
Conduct the annual review of client funding allocation to ensure full compliance with the RAFT Administrative Plan, adjusting internal practices when needed.
Assist in running reports, voiding payments, and processing check runs in PHA-pro (aka HAPPY), ensuring accuracy in all financial transactions.
Work closely with Finance to troubleshoot payment issues, verify funding availability, and ensure documentation aligns with program billing standards.
Conduct fraud reviews and ensure compliance with EOHLC rules and regulations when it comes to fraud, waste, and abuse workflows
Engage all clients by understanding and addressing their needs whether within or outside the scope of work.
Attend & participate in team meetings as requested and communicate effectively with clients and staff in other areas.
Maintain confidentiality of client, employee and agency information in accordance with federal and state laws and funder requirements.
Ensure compliance with program/department, agency and/or funder requirements, as well as SMOC policies & procedures.

Knowledge and Skill Requirements:
Associate's Degree or a minimum of three years' experience in Human Services or related field
At least one-year experience with check run processes and/or payment processes for a non-profit
Sensitivity to low-income families of diverse backgrounds
Ability to work independently

Solid knowledge of Microsoft Office, Tracker, ETO. IRS portal, Salesforce and other databases
Organizational Relationship:
Directly reports to Housing Consumer Education Center Director
Physical Requirement: Must be able to lift 25lbs. Must be able to carry and lift items over prolonged periods of time.
Working Conditions: As part of the responsibilities of this position, the Program Coordinator will have direct or incidental contact with clients served by SMOC in various programs funded or administered through the Executive Office of Health and Human Services. A successful background check is required.

Hiring Process : Potential Counselors must submit an application and resume. Your application materials will be reviewed, and we will determine whether we feel that you would be a strong candidate for Camp Melstone. If so, we will contact you for a interview. If you are a candidate for hire, we will run a Criminal Background check (CORI and SORI) and check four references. Once your background is clear and your references are excellent we will offer you a position to work at camp.
The Camp runs from 7:45 am to 4:15 pm, Monday through Friday. Applicants must be available during these days/times as schedule varies week to week. Staff hours are dependent upon camper enrollment numbers but can be between 20 and 45 hours a week. There are mandatory trainings prior to the start of camp that must be completed in order to work. If unable to attend these trainings staff are ineligible to work. Trainings will be held within the month of June. First Aid and AED training will be offered numerous times!
Day Camp Counselor General Function: The counselor will help supervise and lead a group of youth in daily activities. The counselor must have a positive outlook on summer activities and be willing to help children grow to their full potential. Staff will provide a safe and nurturing environment for youth and will be actively involved and have a positive attitude about all Camp Melstone formalities and activities.
Entry Requirements
· Prior childcare experience preferred.
· At least 15 years of age.
· Ability and experience in supervising and leading children.
· Good role model, high integrity, and adaptable.

QUALIFICATIONS:
1. Minimum age of 15.
2. Certifications: CPR, AED and First Aid certification are preferred. Certifications are offered once hired.
3. YMCA Swim Lessons certification preferred. On-site training provided.
Physical Demands:
1. Ability to demonstrate strokes.
2. Must be able to remain alert.
3. Adequate ability to hear noises and distinguish distress signals.
4. Ability to continuously see all areas of the pool with clear vision.
5. Ability to perform strenuous physical tasks necessary for a water rescue.
6. Ability to communicate verbally, including projecting voice across distance in normal and loud situations.
7. Any other tasks assigned by the Aquatics Director.
Shift Requirements Shifts include weekend, weekday, morning, afternoon and evening.
