Advocates for patients to resolve crisis and encourages them to take an active part in their treatment plan Counsels patients in individual sessions to help them overcome dependencies and to adjust to life Provides individual and group education regarding bonding and parenting issues Meets periodically with all clients having positive drug screens for marijuana and offers referral for rehab services Facilitates and supports clients who are considering adoption; plans and makes appropriate adoption agency referrals. Collaborates with ENLC staff and consultants as needed Encourages clients to identify a goal for their pregnancy and plans for achieving those goals Monitors, evaluates and records client progress according to measurable goals described in care plan Develops or advises on HFPC social services policy and assists in community outreach and development Contacts Children's Services Bureau to report abuse and/or neglect cases Maintains accurate patient records in compliance with HIPAA Develops and facilitates Whole Woman Whole life program Strategic Management Assist in the development of annual outcome goals for all pertinent strategic initiative(s). Track and document steps taken to achieve annual outcome goals. Achieve annual outcome goals as outlined in the appropriate business plan. Job Requirements Experience, Education and Licensure, Bachelor's degree in social work (LSW) required within 30 days, counseling or related field required, Masters preferred. Hold or be working toward a valid social work license. Language and Other Skills Ability to use computer and associated software with little or no assistance; must be proficient in Microsoft Office Suite and/or Google Workplace. Work Environment and Physical Demands The noise level in the work environment is low - moderate. Position Description The Family Liaison will be assigned to a school location and is responsible for building cooperative working relationships between families and school personnel to empower families to partner with the school to become active participants in the education of their children.
