    ABOUT THE POSITION: The Housing Case Manager provides trauma-informed services to individual program participants, many of whom have multiple barriers (i.e., mental health, substance use disorder, chronic health conditions, frequent contact with law enforcement, etc.), so that secure, stable, and permanent housing may be achieved and/or maintained. This service includes assessment, documentation, information and referral, goal setting, direct advocacy, cooperative case planning, and coordination of services. The Case Manager assists and empowers participants to make necessary connections with the community and providers to stabilize and improve the quality of their lives. This is a unionized position, represented by Local 8 of the Office and Professional Employees International Union (OPEIU). ABOUT THE PROGRAM: Compass Day Services includes the Client Services Office (CSO), the Hygiene Center, and the Day Services Center. The Day Center offers comfort and stability to those in need. People can seek refuge, meals, and basic services, as well as connect with housing navigation support, case management, mental health counseling, and nursing services. The Hygiene Center offers individuals experiencing homelessness a place to use the restroom, take a hot shower, and do a load of laundry. The CSO provides free mail services for individuals who are housing insecure. Other duties, responsibilities, and activities may change or be assigned at any time with notice. Engagement & Assessment Provide trauma-informed case management utilizing motivational interviewing and strength-based techniques to assigned participants. Engage professionally and compassionately with participants in all settings and environments. Write clear, objective, and accurate case notes of significant interactions in participant files, including stability plans, measurement scales, leases, and all other required forms and paperwork including logs, records, reports, and statistics, as requested. Perform documentation (following DAP protocol) and input data in the Clarity Homeless Management Information System (HMIS) and other organizational data and case management systems. Identify barriers and steps necessary to overcome obstacles to successful program participation. Hoarding and housekeeping support and interventions Assess participant histories and resource/support needs (e.g., food, health, transportation, CPS APS engagement etc.) and opportunities (e.g., family, friends). Screen individuals for program eligibility (this may include collaboration with referring agencies). Complete a needs assessment, services acknowledgment, and orientation to the program. Assess eligibility for permanent housing resources, such as deposit/move-in financial assistance, rapid rehousing, affordable housing, and permanent supportive housing. Develop a plan with Program Manager to utilize flexible program-based participant relocation and assistance funds in a way that emphasizes equitability not equality. Identify need for interim or bridge housing, link as requested and available. Establish goals and objectives, monitor achievements through regular meetings and case consultations with referral partners, as needed or required contractually. Provide information, referrals, and direct advocacy as needed, utilizing both internal and external resources. (landlords, rental assistance, Veteran programs, supportive guidance on housing selection) Link participants with services intended to address significant barriers to housing stability, such as mental health services, chemical dependency services, primary health services, legal services, Veteran support services, employment and/or educational supports for adults and youth (where applicable). Build and maintain positive relationships with a network of community providers and services to address and advocate for participant needs. In supporting housing stability and retention, work with property management, supervisor and participants in successful tenancy practices and resolutions. Work with program management on situation assessment and follow-up. Work with supervisor to devise and implement corrective action plans for participants failing to meet goals. May be required to assist management in running monthly, quarterly, and/or annual reports (e.g., HMIS). Daily activities are varied, and the day may be interrupted by emergencies or other daily events. Mediate conflicts provide de-escalation and crisis intervention. Model appropriate problem solving. Refer to appropriate services as needed. Provide milieu management as well as support to the program and operations team in daily operations of the facility. In coordination with program management, develop and facilitate life skills trainings and other relevant workshops. Develop and implement community programs supporting housing stability, wellness, and promote meaningful daily activities for participants, which could include groups, classes, workshops, and special events with the support of the Program Manager. Dependent on your work site, certain responsibilities will be prioritized by the Program Manager to meet the needs and funding requirements. Attend community meetings such as CEA, Veteran meetings, or other community services to represent Compass Housing Alliance in helping people obtain housing. Experience and education that demonstrates competency with issues of housing instability, mental health, substance use, sexual assault and/or domestic violence, preferred. Awareness and/or training around belonging, compassion, and meeting people where they are, preferred Experience working in an environment where language may be a barrier, preferred. Fluent reading, writing, and understanding skills in English, required. Current WA State Food Handlers Permit, preferred (If not current, will be sponsored within 1-month of hire). Certain positions will require driving duties. Driving related qualifications include Valid WA driver's license, proof of car insurance, and a driving record that meets Compass standards. About the Opioid Recovery & Care Access (ORCA) program DESC's ORCA program provides compassionate, low-barrier, and evidence-based care to individuals who are recovering from opioid overdose and those seeking treatment for opioid use disorder (OUD). The ORCA Center, our brick-and-mortar location, provides post-overdose subacute stabilization services and provides walk-in access to medications for opioid use disorder (MOUD). The ORCA Patient Outreach Division (POD) delivers field-based MOUD care - meeting people where they literally are in the community. ORCA Center The ORCA Center is open 24 hours per day, 7 days per week, including holidays. It is staffed by a multidisciplinary team, including providers, nurses, medical assistants, peers, and milieu specialists. The ORCA Center offers four overlapping types of services: Care to individuals in stable condition who have experienced an opioid overdose, brought to the ORCA Center by first responders or transferred from local emergency departments. Rapid initiation of medications for opioid use disorder (MOUD), including buprenorphine (sublingual and long-acting injectable) and methadone (under the “72-hour rule”, or CFR Title 21 §1306.07 (b)), for people with OUD, regardless of whether an opioid overdose has recently occurred. Harm reduction counseling and OUD-related physical and behavioral health services. Follow-up care for individuals who have started MOUD to ensure support in MOUD continuation. ORCA POD ORCA POD is ORCA's field-based care team, bringing MOUD induction, stabilization, and maintenance services as well as education and other supports directly to patients in the community. ORCA POD operates during business hours (Monday through Friday, 9 a.m. to 5 p.m.). The POD Provider may also support the DESC ORCA Center, which operates 24/7 offering rapid MOUD access and post-overdose care. This is a clinical, non-supervisory position. MAJOR DUTIES AND RESPONSIBILITIES: Field-Based Clinical Care and Community Engagement: Provide evidence-based MOUD treatment (buprenorphine, including LAI formulations; methadone under the 72-hour rule) in a variety of community settings. Provide telemedicine care to patients and remote consultation for nurses in the field. Deliver low-barrier clinical assessments, MOUD initiations, acute follow-up, and ongoing management for individuals engaged in outreach. Conduct harm reduction counseling, including safer-use strategies and overdose prevention planning. Manage common conditions such as skin and soft tissue infections and connect individuals to primary care or mental health services as needed. JOB DEFINITION: On-Call Administrative Support staff fill staffing gaps on the administrative team when full-time staff are taking PTO, need to be away from their work site for meetings and trainings, during job transitions, when there is a need for additional help for special projects, or for other staff capacity needs. Shift locations vary across DESC's downtown Seattle offices at 515 3rd Ave and 216 James St. Shifts are sometimes split between locations. On-Call Admin Support prioritize open administrative shifts but may also pick up open shifts at DESC's housing projects, after approval and training. This position requires a strong interest in providing administrative support for a non-profit agency that specializes in services for those experiencing homelessness, mental health crisis, or currently engage in substance use. The successful candidate will be willing to build skills which specialize in engaging and, at times, deescalating clients to provide smooth communication between the client and their care team. This position is in addition to a 7-person full-time Admin Support team (including supervisor) which reports to the Senior Program Manager of Drop-in, Employment Services, Administrative Support Services and Client Engagement Services (DEAC). On-Call Administrative Support staff must pick up a least 4 shifts per month. MAJOR DUTIES AND RESPONSIBILITIES: Front Desk & Lobby - Client Interactions: Arrive promptly and complete pre-opening tasks, including populating schedules, preparing print outs, gathering intake information, and preparing the office for visitors. Provide Front desk coverage from 9 am - 5pm. Maintain a polite, convivial, and welcoming presence in DESC lobby spaces. Greet clients and visitors who enter the office, learn why they are visiting, and inform appropriate staff of their arrival. Aid and provide information to people experiencing homelessness and others who are experiencing crises and looking for help. Orient visitors to available services, DESC resources, and resources in the community as appropriate. Assist in maintaining a comfortable and clean lobby area, office area, and conference rooms, while being tolerant of unusual and bizarre client behavior. Intervene when necessary to discourage loitering, fighting, use or sale of drugs and alcohol, and other uncivil or illegal behaviors. Assist with providing front desk coverage to relieve teammates for breaks (Each staff working a full shift receives two 15-minute breaks and one 1-hour lunch break). Engage with people experiencing a variety of behavioral health conditions whose behaviors may be bizarre, unpleasant, or uncooperative. Engage in de-escalation as needed in accordance with agency values and training. Phones/Mail/Office Supplies: Answer incoming calls promptly with proper etiquette; route calls to appropriate persons; transfers calls to voice mail or take messages as needed. Provide basic information on agency and services as appropriate over the phone. Process mail for clients and staff: Accept and document mail from USPS, UPS, FedEx, and other independent vendors, sort/distribute mail, and enter received client mail into tracking database. JOB DESCRIPTION: The On-Call Support Staff works under the direction of the Clinical Supervisor or Project Manager and acts as a member of a multi-disciplinary team. Act as the PACT team's subject matter expert for substance use disorders. Provide consultation to other team members regarding SUD issues, treatment, and education, including Harm Reduction best practices. Provide staff training on substance use issues, as appropriate. Provide individual and group substance use disorder (SUD) treatment in office and in community settings using a stage-based treatment model that is non-confrontational, considers interactions of mental illness and substance use, and has client-determined goals. With the clients participation, develop support and treatment plans; facilitate linkages to collaborative resources when appropriate. Plan, organize and facilitate treatment and support groups for clients, as appropriate. Monitor and coordinate replacement of PACT harm reduction supplies. Advocate for client access to community resources and services, ensuring that client needs are met and rights maintained; consult and collaborate with community providers to ensure continuity of care. Develop and maintain strong collaborative relationships with DESC staff, and other service and resource organizations to ensure full continuity of care for clients. Participate in psychiatric consultation, supervision, program team meetings and in-service trainings; participate in clinical reviews and case conferences. Comply with the agency's clinical accountability policies and procedures; maintain current and complete clinical records; participate in quality assurance reviews when assigned. Comply with data gathering/submission requirements, and with applicable program research and evaluation procedures. Participate in verbal de-escalation in emergent situations and be willing and able to assist other staff as needed to maintain a safe and secure environment. Participate in the planning, organizing and facilitating of unit mitigations for clients on your caseload. Unit mitigations are coordinated efforts to support clients with maintaining healthy living conditions. This can include but is not limited to attending care conferences related to unit mitigation, outreaching and supporting clients in their residential units with tools and skills to maintain their units, coordinating with housing staff to ensure proper work orders are filed in a timely manner, participating in cleaning out clients units, and documenting barriers to unit mitigation. Collaborate with the greater DESC SUD team by attending the weekly SUD team meeting and consulting with the SUD Clinical Supervisor and SUD Senior Program Manager at least monthly. This is a multidisciplinary role, serving clients with dual diagnoses. Other duties as assigned. Specialty Area: Facilitate PACTs SUD program as described above. Complete and update monthly substance use assessments, according to each clients stage of change for all PACT enrolled clients. Prepare assessments for inpatient treatment programs. It offers four overlapping types of services, delivered by a team of prescribers, nurses, medical assistants, peer specialists and milieu specialists, with operational leadership support: * Care to individuals in stable condition who have experienced an opioid overdose, brought to the ORCA Center by first responders or transferred from local emergency departments. * Rapid initiation of medications for opioid use disorder (MOUD), including buprenorphine (sublingual and long-acting injectable) and methadone (under the "72-hour rule", or Code of Federal regulations Title 21 §1306.07 (b)), for people with opioid use disorder (OUD), regardless of whether an opioid overdose has recently occurred. * Harm reduction counseling and OUD-related physical and behavioral health services, such as hepatitis C testing and treatment and HIV pre-exposure prophylaxis. * Follow-up care, at the ORCA Center or at times field-based, with individuals who have started MOUD to ensure support in MOUD continuation. This is a clinical role without administrative or supervisory duties. This team is designed to work with individuals who have experienced long durations or multiple episodes in psychiatric hospitalizations and/or residential mental health treatment. Individuals served experience chronic or multiple episodes of severe mental illness, co-occurring substance use disorders and possibly homelessness. These individuals have either been unable or unwilling to participate and succeed in traditional treatment settings. Therefore, it is critical that the members of this team work together in a collaborative manner that eliminates the traditional treatment borders and barriers. MAJOR DUTIES AND RESPONSIBILITIES: Coordinate client care with a multi-disciplinary team of social workers/case managers, chemical dependency professionals, psychiatric nurses, peer counselors, employment specialists and other mental health staff within the PACT team. Perform full psychiatric evaluations and provide treatment and recommendations as appropriate, at DESC sites and in the community, as needed by program and individual need.
