The Palliative Care Coordinator is responsible for the patient chart, including establishing the chart upon the initial referral and the ongoing maintenance of documentation until the patient is discharged and the chart is closed in accordance with Medicare/Medicaid rules and regulations. S/he facilitates the flow of patient care from initial consultation to patient discharge, including patient scheduling and phone triage. Additionally, s/he is responsible for the submission and tracking of patient billing and maintenance of patient metrics.

Position Specific Responsibilities
* Supports agency and corporate policies, goals, and objectives.
* Attends and participates in staff meetings, educational programs and community events, as requested.
* Communicates identified needs and potential solutions to supervisor.
* Establishes patient chart.
* Ensures all referral documentation is signed and dated.
* Submits requests for medical records and hospital inpatient charts.
* Files documents accurately in corresponding patient charts, and thins charts as necessary.
* Schedules new and follow-up Community Palliative Care visits.
* Confirms appointments with Community Palliative Care patients the day before the scheduled visit.
* Answers incoming telephone calls, takes accurate messages, and transfers calls appropriately.
* Rotates deceased/discharged patient charts from active chart status, and establishes deceased/discharged patient files.
* Performs billing tasks as outlined in the Palliative Care Guideline: Preparation for Submission of Palliative Care Bills.
* Coordinates communications with the members of the Palliative Care service.
* Communicates effectively with members of Palliative Care service, patients, families, and the community.
* Transfers phone system to secondary system as appropriate.
* Faxes documentation of Palliative Care encounters to attending physician/Primary Care Physician.
* Performs other duties as assigned.
Education and/or Experience
* High school diploma or GED required.
* Two (2) to three (3) years of related experience highly preferred.
* Experience as a Licensed Practical/Vocational Nurse preferred.

+ On-going assessment of the branch will be provided on a regular basis and weekly reports to the VP of Clinical Quality will be provided to analyze outcomes and possible changes in plan.
+ Aids with claim denials and in the resolution of claims.
+ Collaboration with the Director of Clinical Operations and Vice President of Clinical Operations would be maintained to ensure coordination is maintained and delineate roles and responsibilities related to support and education actions.
+ Ensures compliance with local, state and federal regulations and accreditation standards. Provides support, direction and feedback to staff during the clinical compliance process. May delegate responsibilities to ensure follow-up to resolution of each action item identified.
+ Proactively assesses office performance and implements changes to prevent noncompliance. Communicates with up line manager, Chief Clinical Officer, Chief Compliance Officer, legal department, Utilization Management and other corporate resources as appropriate to address/resolve issues.
+ May work as support or back up to a Director of Clinical Operations at times when increased survey response is needed. This role would be a resource to help manage and contribute when appropriate.
+ Direct and monitor resolution of identified deficiencies in conditional surveys.
+ Support offices with quality measures in all areas which may include but not limited to claim denials, quality metrics, quality reviews and quality care.

Hospice Aide Requirements
Certified Nursing Assistant highly preferred.
In states where licensure is not required (AL, AZ, CT, GA, IA, IL, KS, LA, MA, ME, MI, MS, MT, NM, OH, OK, PA, SC, TN, TX, WV), applicants must meet one of the following:
They are already a licensed/certified aide; or
They have completed a state-approved nurse aide training program AND have been continuously employed in that capacity for the previous 24 months
Current CPR certification required.

The Volunteer is responsible for modeling the Compassus values of Compassion, Integrity, Excellence, Teamwork, and Innovation and for promoting the Compassus philosophy, using the 6 Pillars of Success as the foundation. S/he is responsible for upholding the Code of Ethical Conduct and for promoting positive working relationships within the company, among all departments, and all external stakeholders. The Volunteer is a hospice-trained individual from a professional or non-professional background who provides non-medical support services to patients and families, to the professional hospice staff, and the organization. S/he is essential to the successful operation of the hospice program. S/he may choose the area in which they prefer to function and the hours they are willing to donate. All duties and responsibilities of volunteers require professionalism, sound judgment, and effective communication skills.
Position Specific Responsibilities
Administrative Volunteers: Provides assistance in the hospice office or with hospice activities.
Bereavement Volunteers: Provides support to bereavement clients under the direction of the Bereavement Coordinator and the Volunteer Coordinator.
Patient Care Volunteers: Provides visits to support patients and families as outlined in the Plan of Care, including companionship, caregiver relief, running errands, and other duties as assigned. Provides Angel/11th-hour support to patients and/or families as outlined in the Plan of Care, sitting bedside with actively dying patients.
All Volunteers: Submits documentation of all contacts with patients/families, or of any volunteer activity, in a timely manner to the Volunteer Coordinator. Participates in the in-service programs to keep informed about current information pertinent to the volunteer.
Education and/or Experience
Professional, Certified and/or Complementary Therapies Volunteers: Must provide a current license, certification, or proof of training and experience. Minimum of one (1) year of experience required. Ability to maintain infection control requirements. Must be at least 18 years of age.
Skills
Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage.
Language Skills: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties. Strong written and verbal communications.
Other Skills and Abilities: Ability to understand, read, write, and speak English. Articulates and embraces integrated healthcare at home philosophy. Ability to support dying patients and their families utilizing skills learned in volunteer orientation. Maintain confidentiality of patient/family interactions and information. Ability to follow through on patient care assignments in a reliable manner.
Physical Demands and Work Environment: The demands of this role necessitate a team member to effectively perform essential functions. Adaptations can be made to accommodate team members with disabilities. Regular standing, walking, and manual dexterity are fundamental, along with the ability to lift and move objects up to 50 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus. In a healthcare setting, exposure to bodily fluids, infectious diseases, and conditions typical to the field is expected. Routine use of standard medical equipment and tools associated with clinical care is essential.
