The Hospice Admissions Coordinator processes patient referrals, coordinates the admission of patients, and enters patient admission data after all requirements of hospice have been met. Position Specific Responsibilities Attends and participates in staff meetings, educational programs and community events, as requested. Communicates identified needs and potential solutions to supervisor. Receives and processes referrals from physicians, patients, families, friends, and community. Contacts physicians to secure an order for admission. If the Admissions Coordinator is not a Registered Nurse, s/he will have to ensure a nurse is present to take a verbal order or will have to ask for a “script” to be faxed. Contacts insurance companies for pre-certification authorization. Schedules appointments with patients and families for informed consents and the RN assessment visit. Explains hospice services and benefits to patients, family members, and significant others, if needed. Informs appropriate team members of time of appointments. Completes admission process according to agency/corporate policy. Ensures proper notification of admission to assigned interdisciplinary team (IDT) and ancillary providers. Enters admission data into a third-party system. Ensures patient chart, ensuring all documentation is signed and dated and turns chart over to Team Coordinator. Markets referral sources. Contacts long term care staff, hospital staff, and physicians to promote hospice services. Provides back-up support for Hospice Care Consultant. Documents marketing contacts, as directed. Performs other duties as assigned. Education and/or Experience High school diploma or GED required. Bachelor's degree preferred. Two (2) to three (3) years of related experience and/or training strongly preferred. An equivalent combination of education and experience will be considered. Experience communicating with physicians, nurses and other healthcare personnel highly preferred. Experience in insurance and Medicare/Medicaid billing and reimbursement preferred. Skills Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage. Language Skills: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties. Strong written and verbal communications. Other Skills and Abilities: Ability to understand, read, write, and speak English. Articulates and embraces hospice philosophy. Physical Demands and Work Environment: The demands of this role necessitate a team member to effectively perform essential functions. Oversees the Residency Program. Assists with and provides clinical supervision and evaluation of patient care related to echocardiography. Available to perform inpatient and/or outpatient cardiac ultrasound testing to include 2D echo / Doppler, stress exams, and transesophageal echocardiograms as well as other duties assigned by manager or director. Provides complex technical care with the use of ultrasound technology for adolescent, adult, and geriatric patients and provides all necessary documentation and preliminary exam findings. Must have excellent understanding of structural heart procedures as they relate to echocardiography. Must also have full understanding of the use of 3D cardiac ultrasound. KEY RESPONSIBILITIES: 1. Oversee Residency Program a. Assists with activities related to echocardiography laboratory accreditation for Piedmont Healthcare hospitals to ensure compliance to IAC standards. b. Training and Education a. Assess the educational needs and competencies of cardiac sonographer staff and interns and develop, schedule, coordinate, and conduct educational programming for new and existing staff to improve quality of care and reduce variation in workflow. b. Maintain an up to date knowledge of echocardiography trends and best practice and conduct, participate in, facilitate, and utilize research to foster evidence-based practice. c. Provide clinical supervision and evaluation of patient care related to echocardiography. d. Attend and participate in echocardiography leadership activities. e. Quality/Innovation on echo protocols, policies and procedures f. Maintain relationships with technical colleges and oversee student interns. 3. Clinical Support a. Perform patient evaluations including verifying patient identity, reviewing the medical record for orders and prior studies, and addressing patient questions and concerns. b. Perform echocardiograms including 2D / Doppler with and without contrast, dobutamine stress and exercise stress echo exams, and transesophogeal echocardiograms according to protocol. c. Complete appropriate documentation, post processing, and preliminary findings in the medical record and cardiovascular PACS system. d. Acquire and maintain training and competency in electronic health record system as required to perform duties. e. Maintain personal competency file if applicable.     Company: Mercy Health by Compassus The Patient Transition Coordinator supports seamless care transitions by coordinating referrals to hospice, home health, home infusion, and palliative services. This role works closely with Care Transition Coordinators and Home Health Care Transition Coordinators to ensure patients receive timely and appropriate services. The Coordinator promotes Compassus values of Compassion, Integrity, Excellence, Teamwork, and Innovation, upholds the Code of Ethical Conduct, and fosters positive working relationships across the organization and with external partners. Position Specific Responsibilities • Organizes, tracks, and builds a complete medical record for appropriate patient transition to the home health providers; and confirms start of care (SOC) for each patient referred. • Responsible for identifying all post-acute care needs and collaborating with the Care Transition Coordinator. • Collaborates with all Compassus/Ascension at Home intake teams. • Responsible for completing the transition of care, including submission of complete referral, and, securing outside agency placement if Compassus/Ascension at Home is unable to service the patient. • Organizes and prioritizes large volumes of information. • Displays exceptional customer service to our patients/families/referral sources. • Handles confidential and non-routine information. • Manages and creates general correspondences, memos, charts, tables, graphs, business plans, etc. • Utilizes computer software applications effectively in word processing, spreadsheets, database, and presentation software (MS Word, Excel, Access, PowerPoint). Work requires continual attention to detail in composing, typing, and proofing materials, establishing priorities, and meeting deadlines. • Performs other duties as assigned. Education and/or Experience Bachelor's degree in a health-related field preferred. An equivalent combination of education and experience will be considered. Minimum of one (1) year of experience in a related health care setting preferred. One (1) year experience in home health, hospice and/or palliative preferred. Prior experience with electronic medical records preferred. Knowledge of home health, hospice, and palliative eligibility requirements preferred. Skills Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage. Language Skills: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties. Strong written and verbal communications. Other Skills and Abilities: Ability to understand, read, write, and speak English. Articulates and embraces hospice philosophy. Physical Demands and Work Environment: The demands of this role necessitate a team member to effectively perform essential functions. Adaptations can be made to accommodate team members with disabilities. Regular standing, walking, and manual dexterity are fundamental, along with the ability to lift and move objects up to 25 pounds.
