Responsibilties:
Responsible for providing professional rehabilitation therapy to patients in their home through evaluation, development and implementation of individual goal-oriented therapy programs in accordance with organization policies and procedures and external organizations.

The Clinical Therapist II treats patients by utilizing a variety of modalities as a member of the multidisciplinary clinical treatment team. includes:
1. Provides a comprehensive assessment of a patient's psychosocial needs, develops treatment plans and treatment plan reviews based on findings and according to department policy.
2. Provides effective behavioral health treatment and education to patients, whether including assessment or therapy (individual and/or group), and shares and interprets information to staff, families, and/or other approved agencies.
3. Completes all documentation written reports in a timely fashion in accordance with department and hospital policy. as required by the Medical Center or Agency policy.
4. Provides services to patients in accordance with Licensure regulations defined by the respective licensure board, i.e., ASWB or NBCC.
5. Utilizes and obtains and interprets information in terms of patient needs, knowledge of growth and development, and an understanding of the evidence- based treatment options available range of treatment required for specific populations that age groups, when developing treatment plans.to develop a treatment plan specific to patient needs.
6. Provides comprehensive case management services/assessment as warranted.
7. Assess Social Determinants of Health (SDOH) and provide information, education and/or referrals as appropriate to meet identified patient needs.

Responsibilities
Screens and provide an in-depth evaluation of the needs of ventilator and respiratory clients.
Consult with the attending physician regarding treatment protocol. Make written recommendations for interdisciplinary care planning and treatment.
Inform residents or qualified representative about care and treatment plan and possible
outcomes of treatment, or lack of treatment if resident refuses participation in a respiratory rehabilitation program.
Identify problems and develop a complete plan of care according to facility/unit policy and
federal/state regulations. Initiate ventilator weaning protocol and/or respiratory rehabilitation in accordance with the physician's orders; maintain required contact with physician.
Maintain written records, evaluations, progress notes, and discharge summaries of residents
receiving respiratory treatment and monitoring according to state, federal, and facility policies.
Attend, as necessary, and provide input at the Resident Care Planning (RCP) meetings for interdisciplinary
team and the Resident/Family Care Plan Review Meeting; complete required documentation after the meetings.
Record resident's plan of care in the Interdisciplinary Care Plan as required or as needed.
Contribute to the completion of the Minimum Data Set (MDS) within the established time frame

Qualifications
Graduation from an accredited AMA approved school of Respiratory Therapy preferred

A spacious, up-to-date therapy gym accommodates residential subacute rehabilitation and outpatient physical, occupational and speech therapy.
Responsibilities
Speech-Language Pathologist:
Screens resident for communication problem upon request; conducts an in-depth evaluation and makes written recommendations for a therapy plan of care, if necessary and upon request.
Coordinates audiometric screening test for resident and makes referrals for further audiology evaluations, or treatments if necessary.
Initiates speech-language therapy plan of care according to physician orders; and maintains required contact with the physician during the therapy program of resident.
Observes rights of residents and treats residents with respect and kindness.
Maintains written evaluations, progress notes, and discharge summaries for all residents in a speech-language treatment program.
Confers with other staff regarding special needs of residents and integrates speech-language treatment program with goals and approaches of other disciplines.
Attends Resident Care Planning and Family/Resident Care Plan Review meetings for residents currently in a speech-language treatment program.
Maintains records that include statistical reports about resident, in-service education information, budget information and a material/equipment inventory.
Submits statistical reports to the Business Office staff for Medicare billing payments, and completes assessment forms upon request.
Attend staff meetings and required or preferred in-service training.
Participates in developing and conducting in-service training programs for facility personnel involved in resident care.
Reviews written speech-language policies and procedures annually and makes necessary revisions.
Maintains costs of program within the parameters of the annual budget.
Qualifications
Speech-Language Pathologist:
M.A. or M.S. Degree in Speech-Language Pathology from an approved school required.
New York State license to practice as a Speech-Language Pathologist required.
Clinical Competence Certification from the American Speech and Hearing Association preferred.
Ability to communicate well verbally and in writing required.
Current, valid CPR/BLS credential.

PRIMARY RESPONSIBILITIES AS A MEMBER OF OUR TEAM TO LEARN AND EXECUTE
PT Business Management
Build and maintain an active client base
Manage and maintain an accurate schedule, inclusive of client appointments, meetings, and open availability within our internal scheduling systems
Market and sell personal training programs to members and/or clients through phone calls, emails, and in-person interactions
Maintain knowledge of all club services, programs, and products
Service and Hospitality
Execute fitness assessments, guided workouts, and other complimentary services for members

Create and coach personalized programs for both in-person and virtual clients
Maintain an organized and safe fitness floor during shifts and sessions
Professional Development
Attend Continuing Education classes to elevate knowledge and qualify for promotions

Qualifications
Current Personal Training certification or willingness to obtain one through certification reimbursement program.
Eligibility to begin a higher Tier & pay rate based on previous experience and education (Degree in the field)
Current CPR/AED certification

Respiratory Therapist team members assist with ensuring the health and well-being of our residents by carrying out skilled respiratory therapy treatments.
At Elderwood at Williamsville, we are seeking kind collaborative and dedicated Respiratory Therapists (RTs) to join our clinical network.
Responsibilities
Screens and provide an in-depth evaluation of the needs of ventilator and respiratory clients.
Consult with the attending physician regarding treatment protocol. Make written recommendations for interdisciplinary care planning and treatment.
Inform residents or qualified representative about care and treatment plan and possible
outcomes of treatment, or lack of treatment if resident refuses participation in a respiratory rehabilitation program.
Identify problems and develop a complete plan of care according to facility/unit policy and
federal/state regulations. Initiate ventilator weaning protocol and/or respiratory rehabilitation in accordance with the physician's orders; maintain required contact with physician.
Maintain written records, evaluations, progress notes, and discharge summaries of residents
receiving respiratory treatment and monitoring according to state, federal, and facility policies.
Attend, as necessary, and provide input at the Resident Care Planning (RCP) meetings for interdisciplinary
team and the Resident/Family Care Plan Review Meeting; complete required documentation after the meetings.
Record resident's plan of care in the Interdisciplinary Care Plan as required or as needed.
Contribute to the completion of the Minimum Data Set (MDS) within the established time frame
and incorporate the results in the plan of care when appropriate.
Confer with personnel of other departments and provide required training to meet special needs
of residents.
Qualifications
Graduation from an accredited AMA approved school of Respiratory Therapy preferred
New York State Licensed Respiratory Therapist. Registered or Certified Respiratory Therapist by National Board of Respiratory Care.
Previous experience with ventilator dependent residents preferred.
Interest in respiratory therapy treatment program for chronic ventilator, ventilator weaning, and respiratory rehabilitation population.
Ability to verbally communicate well with residents, families, and staff members.
Ability to read and understand task assignments, ability to report verbally and provide written documentation as required.
Display of genuine concern for the elderly; a dependable, patient and compassionate attitude towards residents. Display a demeanor which is always respectful of residents' rights and wishes.
Ability to cooperate and maintain respectful treatment with others. Display a sense of responsibility, maturity, diplomacy, and ability to work well with residents, families, and other staff members.
