WORK SCHEDULE DEMANDS:
The position is full-time, 37.5 hours per week.
Monday - Friday 8:30am - 5:00pm.
Evenings may be required.
Requirements
JOB REQUIREMENTS:

Medical Office Experience

This will include long- term monitoring of clients, providing energy conservation tips, financial education, and workforce development as needed. We are seeking a Dentist specializing in pediatric mobile dentistry. A part-time position responsible for providing clinical dental services and preventive dental care in a pediatric mobile setting. The Staff Dentist supervises the dental assistant and dental hygienist on the pediatric mobile unit.
Work Schedule Demands:
Part-time positions, hours to be determined by our Dental Directors.

Requirements
Required Qualifications:
Graduation from an accredited dental school

KEY RESPONSIBILITIES:

The Nurse Care Manager will have the opportunity to work on a multidisciplinary healthcare team in a community health clinic care setting. The Nurse Care Manager will be part of a program charged with working within the patient care medical home and administering health care in a new and innovative way. The Nurse Care Manager is responsible for providing comprehensive screenings, assessment, care coordination services, disease education and self-management support to patients with chronic health conditions, such as, coronary artery disease and depression.

Issue WIC I.D. card and V.O.C. documents to WIC participants.
Describe the application process and benefits of the WIC Program.
Acquaint participants with WIC Allowed Foods.
Inform participants of their rights and responsibilities.
Inform participants of how to use WIC checks.
Provide translation of nutrition counseling for nutritionist when needed (if bilingual).

Act as liaison to hospital, long-term care and specialists.

Train staff on motivational interviewing
Interact and coordinate with insurance companies' and other external agencies' Case and Disease Management staff, when applicable in caring for the patients within the Patient Centered Medical Home.
