TLC Nursing Associates, Inc. is seeking a highly attentive and skilled Registered Nurse - Tele to care for patients requiring continuous cardiac monitoring and intermediate-level medical attention. The Tele RN plays a critical role in detecting subtle changes in patients' conditions and coordinating care in a telemetry unit.
Job Responsibilities
Provide direct nursing care to patients requiring telemetry monitoring for cardiac and medical conditions.
Continuously assess cardiac rhythms and alert physicians to significant changes or arrhythmias.
Administer medications, monitor IVs, and manage treatments per provider orders.
Interpret telemetry strips and collaborate with telemetry techs and the care team for appropriate interventions.
Educate patients and families on conditions, medications, and discharge instructions.
Document all nursing assessments, interventions, and responses accurately in the EHR.
Promote patient safety and follow evidence-based clinical protocols.
Qualifications
Active RN license in good standing.
Associate's or Bachelor's degree in Nursing (BSN preferred).
Minimum 1 year of telemetry or cardiac nursing experience preferred.
Current BLS and ACLS certifications required.
Proficient in reading EKGs and recognizing common cardiac arrhythmias.
Strong communication and clinical decision-making skills.

The Clinical Nurse is an engaged and credentialed member of the Professional Nursing Organization and is responsible for autonomous practice directed by the professional tenets of practitioner, leader/decision maker, scientist, and transferor. The Clinical Nurse is responsible for utilizing the nursing process to provide evidence-based care and to continuously monitor and evaluate practice to ensure safe passage of patients that is in the best interest of populations served. The Clinical Emergency/Forensic nurse will be an expert Emergency Department nurse with an adjunct specialization in forensic nursing. Emergency Nursing is a specialized area of practice that is both independent and collaborative, requiring the continual acquisition and application of a specialized body of knowledge and skills. This demands a broad scope of practice to promptly deliver emergency, urgent, and non-urgent care to patients of all ages and from all cultural backgrounds. Emergency nursing care is episodic, primary, and typically acute, but may be chronic in nature requiring knowledge and skills to care for patients of all ages, acuities, and physical or psychological conditions. Forensic nursing is the practice of nursing when health and legal systems intersect. Forensic nurses provide specialized care for patients who are experiencing acute and long-term health consequences associated with victimization or violence, and/or have unmet evidentiary needs relative to having been victimized or accused of victimization. Forensic nurses are a critical resource for anti-violence efforts. The forensic nurse provides consultation and testimony for civil and criminal proceedings relative to nursing practice, care given, and opinions rendered regarding findings. Forensic nursing care is not separate and distinct from other forms of medical care, but rather specialized and integrated into the overall care needs of individual patients. The forensic nurse provides specialized medical forensic evaluations to patients experiencing violence, abuse and/or acts of crime. Patient populations served will include vulnerable adult and pediatric patients in the field of sexual assault, domestic violence, abuse, neglect, mistreatment, and death investigations. The Emergency/Forensic nurse position will be a collaborative role delivering patient care primarily in the emergency department in conjunction functioning as a forensic nurse providing medical evaluations in the unit or affiliate system hospitals as needed.
This is a Part Time, 2 12hr shifts/week position with a 3p-3a schedule. Rotation is across all seven days.
On Call requires a minimum 48 hours/month for call which includes one weekend/month
Responsibilities
Provide forensic nursing care in a trauma informed manner. Responsible for collaborating with the medical team and ensuring a Medical Screening Exam has or will be completed. Provides medical care simultaneously, while providing the associated forensic needs of the patient specific to each patient population. Provides medical forensic evaluation options of care specific to the patient population seen to include gathering the history of the violence and performing a comprehensive physical assessment. If evidence collection is completed as part of the medical forensic evaluation, the nurse examiner will adhere and apply forensic nursing care in conformity with the State of New Hampshire Acute Care of the Sexual Assault Patient standards. Collaborate and facilitate advocacy services to appropriate patient populations. Perform and document the comprehensive medical forensic evaluation; including documentation of detailed injuries with support of photography/videography with consent of the patient and if required parent/guardian. Maintains chain of custody for each piece of evidence collection when completed. Coordinates care of agencies involved and coordinates referrals appropriate to each patient medical and/or mental health follow up care. Will facilitate mandatory reporting as required by law to reporting agencies when needed. Provides testimony as a fact witness when subpoenaed and coordinates with attorneys during the judicial process. Maintains open communication with Risk Management during judicial process proceedings. Collaborate with team members to continue to grow the program internally and externally creating partnerships with community partners. This position will be open/eligible to additional on call hours requiring a one-hour response time. Emergency Nurse Responsibilities Utilizes the nursing process to assess, diagnose, identify outcomes, plan, implement and evaluate an individualized plan of care. Utilizes critical thinking and the nursing process to anticipate and recognize changes in patient status, taking action to modify the plan of care or to elevate to the care team as necessary. Practices in accordance with the ANA Code of Ethics to advocate for patients, uphold their autonomy in decision-making, ensure informed consent and assist patients in families in expressing self-determination. Job Summary and Qualifications
The Float RN is primarily responsible for adhering to the hospital's Standards of Patient Care to deliver safe and quality patient care. Documents all pertinent information on the patient's medical record in an accurate, detailed manner adhering to procedures for charting.
Assumes Charge Nurse role upon demonstrated knowledge of responsibilities when assigned.
Participates actively in department meetings.
Supports Department's customer service efforts. Provides service excellence to patients and their families, medical staff, peers, support services and other hospital personnel. 9. Makes appropriate and timely notification to providers on patient condition changes and carries out provider orders.

Job Summary and Qualifications
As a Registered Nurse (RN),you will provide direct nursing careand administrative support to our 23-bed intensive care step-down unit. In this role, you willwork closely with the clinical team to create and implement a care plan that meets each patients needs.Our team is amazing, and we consider ourselves a family who works hard to support each other. Your role is to provide direct patient care dedicated to stroke and trauma, including titrating and vasoactive IV-drips.
You will monitor, record and communicate patient conditions as appropriate utilizing computerized documentation systems.
On our team, you will manage patients with high risk surgeries as well as ortho and trauma related surgeries.
You will work in a team-oriented, fast-paced, collaborative environment
You will instruct and educate patients and family members.
Coordinate patient's discharge planning needs with members of the healthcare team.
REQUIREMENTS
Bachelors Degree in Nursing (BSN) preferred.
Registered Nurse (RN) License in NH or valid Compact State License
BLS and ACLS(American Heart Association and/or American Red Cross)
Successful completion of Basic Arrhythmia (within 3 months from hire)
PCCN Certification (preferred)
TNCC Certification (preferred)
TCAR Certification (preferred)

What you will do in this role:
Evaluate patients prior to surgery
Provide patient and family education surrounding the procedure and peri-operative process
Assume responsibility for patient preparation, including intravenous therapy, preoperative medications, and surgical site verification
Participate in surgical site verification, including briefing, time out, debriefing, and universal protocols, according to hospital policy
Recognize and respond to patient emergencies
Deliver high-quality, patient-centered care while managing evidence-based practices and continuous improvement initiatives
Be an integral member of a dynamic interdisciplinary team consisting of the provider(s), surgeons, surgical technologists and more
Consistently apply the nursing process, critical thinking skills, scientific judgment, technical skills and leadership to independently deliver safe nursing care to patients
