Under general supervision of the Chief Pharmacist, the Clinical Pharmacist - Advanced provides direct pharmaceutical care in the outpatient pharmacy program. This position is considered as the major source of pharmaceutical clinical information and is expected to manage selected patients with chronic disease states, as well as providing pharmaceutical care for acute patients. Maintains confidentiality of all privileged information at all times.
Essential Duties & Responsibilities:
Takes the appropriate health history, follows and adjusts the developed treatment plan and monitors the patient's outcome to the prescribed treatment plan.
Takes the appropriate history and performs limited physical assessment related to the specialized pharmacy clinics, such as insulin titration or anticoagulation management.
Discriminates between normal and abnormal findings to recognize early stages of serious physical, emotional or mental problems.
Documents all clinical activities, updates lists and medication allergy information in the medical record.
Orders and interprets laboratory tests.
Provides pharmaceutical services to focus on therapeutic drug monitoring. Analyzes medication regimens to ensure the most effective, least toxic, and most economical treatment plan, and communicates recommendations to ordering or primary care provider.
Fills and dispenses accurate prescription and over the counter drugs on orders of physician, optometrist, physician assistant, family nurse practitioner, and dentist.
Counsels patients on the use, storage, cautious and side effects of all medications dispensed as well as documentation of cognitive services provided.
Initiates prescriptions for approved formulary agents, medical supplies as documented in clinic guidelines.
Renews prescriptions up to 30 days if they have expired or were previously canceled in error; may change doses on existing medications.
Provides guidance, training, and mentoring for Clinical Pharmacists.
Upholds all principles of confidentiality and patient care to the fullest extent.
Performs other duties as assigned.

Minimum Qualifications:
A Doctor of Pharmacy degree (PharmD) is required. Must be a graduate of an accredited school of Pharmacy and Licensed by a State Board of Pharmacy.
Completed an ambulatory or hospital residency and board certification, or three years of clinical pharmacy practice experience and board certification, or five year of clinical pharmacy practice experience.
National Certification in one chronic disease state is
preferred

A BLS certification is required.
An ACLS certification is required within a year of hire.
Bilingual skills in English and Navajo language

About this role
As a Clinical Pharmacist Manager in Illinois, you will be key to our efforts to provide better care and support to vulnerable populations. We are looking for a Clinical Pharmacist Manager with experience working with vulnerable populations; collaborating with a multidisciplinary team including Community Health Workers, Care Coordinators, and Therapists; supporting patients through direct patient outreach, medication adherence and chronic disease education, and assistance in obtaining medications and durable medical equipment after initial rejection; and using analytics dashboards to proactively engage with patients who may be at high risk for adverse drug events and/or avertable acute care events. The Clinical Pharmacist Manager will supervise and support pharmacists and pharmacy technicians working to address patient barriers to medication access and identify patients who can benefit from Waymark's assistance.
Key Responsibilities & Duties
Provide telephonic and/or video and/or SMS-based outreach, engagement, and education of vulnerable patients in the Medicaid program in Illinois
Work with a multidisciplinary team to provide care management services that ‘wrap around' primary care provider practices; team members include Community Health Workers, Care Coordinators, and Behavioral Health Therapists
Support patients and providers around transitions of care
Provide clinical services that support day-to-day operations and safe practices of the program
Participate in QI activities to assure quality, cost-effectiveness, and ROI
Utilize analytics dashboards that help you proactively outreach patients who receive medication and/or equipment rejections, admissions/readmissions, or who may benefit from medication adherence counseling
Lead and mentor pharmacists and/or pharmacy technicians
Minimum qualifications
PharmD with advanced practice license in good standing in Illinois. Remote, located in Illinois. Chicago or central Illinois preferred
Completion of accredited PGY1 residency training in related discipline
5+ years of experience in pharmacy practice including delivery of interventions/direct patient care in an ambulatory setting among vulnerable populations
Experience in multi-disciplinary care management
Positive referrals from prior colleagues
Preferred qualifications
Completion of PGY2 residency training in related discipline (ambulatory care, geriatrics, pharmacotherapy, internal medicine)
Strong analytic skills and ability to navigate dashboards and registry data for proactive outreach campaigns including for HEDIS gaps in care
History of experience with medication management through collaborative practice agreements with primary care providers
BPS Board Certification Preferred (prefer BCPS and/or BCACP)
Experience serving vulnerable populations with a skill set including motivational interviewing, harm reduction, and behavioral health
Leadership experience supporting other pharmacists, pharmacy technicians and/or other care team members
Salary Range
US Employees in Chicago - $98,000 - $141,000
US employees outside of the above listed US City - $95,000 - $136,000

To Qualify for This Position, You'll Need the Following:
Required Education: Bachelors in a job-related field

Required Skills and Abilities: Strong clinical pharmacy knowledge. Excellent communication (verbal and written), and interpersonal skills. SUMMARY
As a Pharmacy Patient Advocate, you will be a crucial link between patients, medical providers, and our pharmacy. You will make a direct impact by eliminating barriers to medication access, ensuring patients receive their specialty medications in a timely and affordable manner. What You'll Do (Key Responsibilities include but are not limited to):
Serve as a Patient Liaison: Act as the central point of contact between patients, physician offices, manufacturer programs, and insurance payers to resolve medication access issues.
Navigate Benefits & Authorizations: Conduct comprehensive benefit investigations, manage prior authorizations, and coordinate benefits to minimize patient out-of-pocket costs.
Manage Patient Communications: Handle inbound calls for prescription inquiries and outbound calls for patient welcome, refill scheduling, and provider follow-ups.
Ensure Seamless Coordination: Coordinate timely and accurate medication deliveries with patients and providers, ensuring adherence to treatment plans.
Maintain Accurate Records: Collect and maintain confidential patient information, including allergies, contact details, and payment methods, while upholding all HIPAA regulations.
Support Clinical Integrity: Direct all clinical inquiries and patient concerns to a licensed pharmacist to ensure patient safety and proper care.
Utilize Technology: Leverage pharmacy operating systems and other software tools to manage patient cases, track communications, and ensure efficient service delivery.
What You'll Bring (Qualifications & Skills)
High school diploma or GED.
Active Florida Pharmacy Technician Registration required.
A minimum of two (2) years of experience in a customer service setting OR one (1) year of experience in specialty pharmacy.
Prior authorization experience required.
Strong knowledge of medical and pharmacy terminology.
Benefit investigation experience preferred.
Excellent communication skills.
Basic computation skills.
Bilingual - English and Spanish preferred.
Microsoft Office or Google Suite proficiency.
RMA or CMA certified preferred, but not required.
Work Environment & Physical Demands
This role is primarily sedentary and requires the ability to sit for extended periods (minimum of 7 hours). The position required strong multitasking skills, attentions to detail, and the ability to lift and/o move up to 20 lbs.
