Essential Job Function
* Case Management/Care Coordination: Collaborate with interdisciplinary teams to develop, implement, and manage individualized care plans for patients, ensuring comprehensive, holistic support.
* Act as a liaison between patients, families, and healthcare providers to ensure continuity of care across settings.

Summary:
The Assistant NurseManager practices at the level of an expert practitioner of nursing, as defined by Benner, P. (1984). From Novice to Expert. Addison-Wesley Publishing Company. Proactively identify high risk cases that need to be escalated to the list that are not scheduled for discussion that week. Complete CCM follow-up after the meeting as assigned.
