    Congreso is hiring for General Case Managers in the Community School Program. The General Case Manager collaborates with school personnel and families to support student success through holistic case management services. These services include referrals to social services, health navigation, socio-economic supports, and meeting other needs to support family stability. From within the student's school, the General Case Manager uses client-centered methods to engage and advocate for families, assesses needs, contributes to increasing confidence and motivation for student success, and provides education on school policies, resources, and connection to supportive services. This position is responsible for conducting home visits, strength- and needs-based assessments, developing a service plan, and implementing problem-solving interventions, including coordinating warm handoffs to community programs and/or services. This position reports to the Community School Program Manager. This is an ONSITE position, primarily conducted at the school site, and includes home visits within the community, and occasional court appearances with students and their families. Safeguards the confidentiality of client families. Conducts home visits for identified families. Promptly reports all conflicts among client families, and/or serious incidents involving client and families within 24 hours of the incident. Provides services and support outside of working hours to accommodate a family's emergency needs, when necessary. Adheres to documentation deadlines and keep detailed, clear case notes. Case Managers are to submit written testimony in the form of a Court Report for all hearings. Case Notes are to be entered in digital database systems within 5 days of contact event. All Case Managers will also maintain a neat and tidy paper file for each case. Ability to navigate the internet and digital data systems required. Prepares for and attends supervisory support meetings including one-on-one supervision, case reviews, team meetings, etc. Keeps quality case files that pass all internal and funder file audits. Completes assessments, home visits, and other required activities within the standard timeframe. Ensures program quality assurance tools are used and updated according to deadlines. They will perform heavy janitorial duties, provide upkeep of our facilities and grounds, assist in any large annual cleaning assignments, and at times, they may assist with picking up and delivering any received in-kind donations. Responsibilities :The Family Case Manager provides and coordinates trauma informed counseling services for a caseload of clients, either adults or children while serving as the direct service link for facility residents and/or their children to external resources necessary to establish safe and more permanent housing and support in accordance with the mission and vision of the Agency. Responsibilities include but are not limited to: Providing case management (Advocacy, Needs Assessment, Action Planning and Evaluation) according to established professional guidelines and norms for an assigned caseload of adults or children. Accompany residents and/or their children to outside appointments as necessary for support and advocacy. Develop, implement, coordinate and facilitate group meetings to address personal and emotional needs of children, residents and former residents. Provide ongoing individual support and informal counseling, making outside referrals when appropriate. Identify new resources, develop informational materials and networks with community resources on women's health, housing resources, financial literacy, domestic violence, parenting, children's health and others as needed for use by survivors of domestic violence. When working with children: register/transfer children to area schools visit schools as needed to address academic, attendance and/or behavioral concerns. When working with children, conduct intake interviews upon entry and psycho-social assessments within required timeline. Provide training and education on issues such as domestic violence, parenting and health care. Coordinate and conduct children's groups and activities. May also assist with summer camp and activities. Perform exit interviews with each assigned client; develop a follow-up plan and discharge summary. Participate in all required counseling, residential, team, staff and Agency meetings as assigned. Participate in networking and attending various meetings related to intimate partner violence. Collect and report data according to external requirements. All responsibilities are implemented according to our mission and values and according to Agency standards. Qualifications : A Bachelor's Degree in social services or related field with some experience in direct service and/or advocacy is required; experience in a residential setting is strongly preferred. Experience in a residential setting is strongly preferred Driver's License & Passing of MVR (motor vehicle record search) preferred Demonstrated ability to work effectively with a diverse population is required Experience working within an urban setting is preferred Demonstrated ability to work effectively as a team member is required Demonstrated proficiency in communication, both verbal and in writing, is required Proficient computer skills, including Microsoft Office software, Outlook, Word, Excel, required Prior experience using client database, is preferred, ETO experience a plus Some experience in working on domestic violence issues is strongly preferred Knowledge of and experience with Trauma Informed service delivery, preferred Bilingual or conversational in addition to English, preferred Prior Case Management experience preferred Demonstrated skill in crisis intervention, conflict resolution, group facilitation and counseling is strongly preferred Satisfactorily pass State & FBI criminal & Child Abuse clearances pre-hire; and regular renewals (all expenses paid by WAA) Must attend/complete upcoming Domestic Violence Advocate Paid Training (November 17 - December 3) Current Openings: Location: Philadelphia, PA Monday - Friday 9:00am - 5:00pm
