Provides general or specialized nursing care to patients in ambulatory setting or clinical practice. Provides nursing services to patients and families in accordance with the standards of practice of a professional nurse, Connecticut Children's policies and procedures and State and federal regulations. Education and/or Experience:
* ASN required upon hire. BSN preferred upon hire and required within 3 years of hire.

Collaborates and works with Cardiology Clinic joint programs. Maintains the INR clinic outcomes and database, trend analysis of data collected and shares with stakeholders. Reviews the pacemaker, heart failure and adult congenital patient policies and/or protocols and collaborates with clinical leadership to determine appropriateness of adopting/adapting guidelines or protocols. POSITION SUMMARY
We are seeking a dedicated

Registered Nurse (RN) to join our team at All Pointe HomeCare, LLC, a subsidiary of Connecticut Children's. In this role, you will provide essential nursing care to pediatric patients, contributing to their health and well-being while working in their home.
QUALIFICATIONS
Current RN license in the state of Connecticut.
Experience in home care or pediatric settings preferred.
Strong knowledge of medical terminology and diagnostic evaluation processes.
Excellent communication skills to interact with children and their families.
RESPONSIBILITIES
Coordinate total patient care by conducting comprehensive health and psychosocial evaluation, monitoring the client's condition, promoting sound preventative practices, coordinating services and teaching/training activities.

Accurately interprets and implements treatment regimen as prescribed by the Physician;
Assists the Physician during treatments and/or diagnostic procedures;
Keeps the Patient Care Manager/designee and/or physician, abreast of changes in patient's condition and/or treatments, as appropriate;
Uses clinical judgment in delegating assignments to the Patient Care Associate, Support Care Associate and Clerical Service Associate in providing patient care, and ensures that assignments are completed in a timely fashion.
8. Performs grade I-IV Decubitus Care
Performs preventive skin care measures
Applies simple dressings, maintaining principles of aseptic technique;
Applies warm & cold compresses, consistent with facility procedure;
Performs irrigations, consistent with Facility procedure;
Performs other procedures related to skin care, as necessary.
9. Administers medications correctly and safely
Correctly identifies medication: action; dosage; side effects and implications for the patient;
Meets standard on medication administration examination;
Demonstrates preparation of local solutions;
Demonstrates preparation of I.V. drips;
Administers and documents medication correctly;
Administers IVP medication as per department policy and procedure;
Educates patients and significant others related to drug and food interactions.
10. Participates in patient and family education
Provides the patient with an explanation of his/her condition as indicated;
Communicates assessment data in an orderly fashion by recording, updating and verbalizing pertinent information to care team members and to appropriate agencies;
Recognizes and utilizes health teaching opportunities and resources /materials available for this teaching;
Provides for early discharge planning and appropriate referrals for post-hospital care;
Evaluates the effectiveness of teaching by feedback from patient/family and documents same.
11. Performs related duties, as required.
Qualifications:
Graduate from an accredited School of Nursing. Associate's Degree in Nursing, required. Bachelor's Degree in Nursing, preferred.
Minimum 1-2 years of professional nursing experience in the related specialty.

DIRECT PATIENT CARE - Provides direct patient care (as defined by the unit) that addresses the population-specific needs of the patients on the unit with the exception of sitting for patients who require 1:1 observation. Activities may include any of the following: assist patient with feedings and personal care (e.g. hygiene, ambulating and activities of daily living, etc.); obtain vital signs, temperature, weight, height, head circumference or other patient data as required; obtain and test specimens (e.g. blood glucose, urine, etc.); transport patients; assist with patient admissions and discharges. Demonstrates an awareness of safety needs and a basic knowledge of medical terminology, infection control, risk management and environment of care policies in all interactions with patients/families. Responds with tact and discretion to the needs of patients and families, and reports needs beyond the PCA skill level to the RN.
DOCUMENTATION - Accurately documents patient information on appropriate forms. Demonstrates basic data entry and data retrieval competency in electronic patient records and/or patient supply systems utilized on unit.
UNIT SUPPORT ACTIVITIES - Maintains appropriate inventories of unit supplies in patient rooms/bedside stands, and in treatment, utility, medication and linen rooms. Cleans, maintains and inventories unit equipment. May perform quality control checks on equipment or supplies as directed by the RN. Utilizes all equipment appropriately and notifies appropriate person of equipment or facility breakdown or damage. Assist with miscellaneous errands, and transportation of medication (including controlled substances) blood, equipment and supplies, as directed by the RN. Assists with routine clerical tasks. Demonstrates knowledge of the population-specific differences and needs of patients in appropriate, specific populations from neonate through adolescence and applies them to practice.
Demonstrates cultural sensitivity in all interactions with patients/families and co-workers.
Demonstrates support for the mission, values and goals of the organization through behaviors that are consistent with the Connecticut Children's Standards.
POSITION-SPECIFIC ROLE RESPONSIBILITIES
The Flynn Fellowship Student Nurse Intern participates in a comprehensive scope of direct observational and learning experiences, including exposure to:
Oncology Nursing (possible roles/typical responsibilities, applied practice, possiblecareer opportunities, etc.)
Medical ; Surgical Oncology (including treatment plans, key policies/procedures, patienteducation, etc.)
Clinical Research
Oncology Office/Functional Organization (organization, functions, roles/responsibilities)
Palliative Care Department (organization, resources, services)
Infusion Services
Radiation Therapy
Patient Support Programs
In addition, as part of this learning/development experience and in collaboration with the Connecticut Children's Flynn Fellowship Educator, the Student Nurse Intern completes an "evidence-based practice" research project in some aspect of Pediatric Oncology Nursing. The Nurse Intern presents his/her project at the Flynn Fellowship Graduation Ceremony at the completion of the Fellowship Program.
Program Schedule: The Connecticut Children's Flynn Fellowship runs for 8 consecutive weeks during the summer months. The Student Intern works 40 hours per week, and the daily schedule may vary during the week to allow for diverse clinical learning experiences.
