Below is a list of your responsibilities as a Behavioral Health RN with YesCare: Assist in ensuring 24-hour coverage nursing services of Mental Health Unit and appropriate provision of clinical treatment. Coordinates Mental Health Unit sick call to ensure nurse triage and appropriate referral for physician follow-up. Clinical duties include: Psychiatric and physician orders are implemented in timely, clinically effective manner. Medications are administered and monitored in accordance with the policies and procedures of Mental Health Unit. Inmates admitted to the Mental Health Unit receive Initial Nursing Assessment upon admission to the unit or as soon as inmate's condition permits. Changes in inmate's functioning and critical incidents are documented in the Medical Record. Nursing Discharge Summary is completed when inmate transferred from Mental Health Unit. Assists in coordination of psychiatric services to ensure psychiatrist is informed of psychotropic medication compliance, changes in inmate functioning and results of laboratory testing in a timely manner. Provide group counseling and activities for inmates to ensure Level II inmates are provided two (2) hours of out-of-cell time daily. Ensure Medication Administration Records (MARS) are maintained and updated. Abides by the security regulations and confidentiality of inmate information requirements of the facility. Monitors medical supplies to facilitate appropriate inventory and ordering. Interrelates and works effectively with Health Services staff, prison staff and CMS administrative staff. Complies with employee standards of CMS and applicable employee standards of the facility. The Palliative Care Nurse Practitioner facilitates palliative care for Gentiva Palliative Care patients, ensuring a comprehensive, individualized treatment plan, while demonstrating excellent clinical skills and ethical, relevant judgment. Provides diagnosis, treatment, consultation, and follow-up under the direction of the Palliative Medical Director (if required under State law). As a Nurse Practitioner, you will: + Work in collaboration with the Palliative Care Medical Director and other palliative team members to deliver palliative care by providing diagnosis, treatment and follow-up for patients referred to palliative care. + Take responsibility for patient palliative care from the initial assessment through discharge. Assessment at a minimum should include: disease-specific changes, decline or change in function, nutrition and/or cognition, inadequately controlled pain or other distressing symptoms, falls, infections or similar events, the impact of the disease burden on the patient's condition, caregiver and/or family's quality of life. + Establish the palliative plan of care, individualized for each patient, in consultation with the patient, family and other members of the Palliative Care Team. + Facilitate obtaining care for patient when a need is identified in accordance with the Plan of Care. + Participates in evening/weekend call as required, conducting on-call services in a clinically competent and responsive manner. + Prescribe medications including controlled substances to the extent delegated and licensed, in accordance with state law. You will play a vital part in building strong professional relationships, coordinating admissions, and supporting community awareness of hospice services. Essential Responsibilities + Receive and respond to referrals from hospitals, home health, physicians, case managers, skilled nursing facilities, and other healthcare professionals + Assess patient eligibility for hospice services by reviewing medical records, conducting clinical evaluations, and communicating with physicians and families + Educate patients and families on what hospice care includes, how it works, and why it matters-providing clarity, reassurance, and compassion + Complete necessary documentation and coordinate the intake and admission process, ensuring all legal, clinical, and payer requirements are met + Serve as a liaison between referral sources and our hospice care team-ensuring seamless handoffs, clear communication, and rapid response to needs + Make daily visits to hospitals and facilities, maintaining visibility and accessibility with discharge planners, case managers, and physicians + Build and maintain strong relationships with referral partners and identify new outreach opportunities to increase awareness of hospice services About You Education + Licensed Nurse with three to five years of hospice experience required. + BSN with five or more years of hospice experience preferred. Experience + Minimum of three years of hospice experience required. + Prior experience as a discharge planner, case manager, or educator preferred. Licenses and Certifications + Current RN license to practice nursing in the state of employment. + CPR certification. + Valid driver's license and automobile insurance. Skills and Knowledge + Strong knowledge of hospice care and the unique needs of patients and families. + Familiarity with community resources and referral networks. + Excellent patient assessment and documentation skills. + Ability to supervise staff, coordinate services, and manage programs effectively. + Strong communication and interpersonal skills with sensitivity to diverse backgrounds.
