Summary:

Responsibilities:
Conducts initial comprehensive patient and family psychosocial assessment, and triages care accordingly.
Communicates with staff regarding assessment, treatment/interventions and interdisciplinary care plan through documentation and meetings such as patient rounds.
Demonstrates the knowledge and skills necessary to provide competent care for the physical, psychological, social, educational, and safety needs of the patients served regardless of age.
Provides case management for assigned patients that involves consultation and collaboration with hospital and community professionals and programs in order to provide the most appropriate and comprehensive care.
Consistent use of evidence-based trauma and risk assessment tools and application of crisis intervention skills when indicated, and determine formally the need for a referral to the Emergency Department or more suitable community agency.

Use of CBT and DBT (Dialectical Based Therapy) intervention through individual, group, and family therapy, to support client in achieving therapeutic goals.
PIRC Add On:
Works in a fast-paced environment conducting risk assessments and using crisis intervention skills as needed.

What You'll Do:
Assesses initial client treatment needs and formulates appropriate diagnosis. Provides treatment to patients with more complex or multiple diagnoses.
Performs intake session with clients and completes Integrated Biopsychosocial Assessment.
Formulates a mentalhealth diagnosis using the current version of the Diagnostic and Statistical Manual of Mental Disorders V (DSM V) standards.
Develops treatment plan incorporating standard practice guidelines for each diagnosis.
Provides therapy, supportive therapeutic counseling, and education to assigned individuals.
Applies therapeutic treatment models such as behavior modification, cognitive behavioral therapy, dialectical behavioral therapy, and others as appropriate.
Measures change and adjusts treatment as needed, using appropriate screening tools.
Completes encounters and associated documentation according to YVFWC requirements and coding and billing rules.
Enters all documentation into the electronic health record (EHR).

Who We're Looking For:
A licensed therapist who has direct experience treating children or adolescents
A team-focused individual who can engage with all levels of an interdisciplinary team
A therapist who can handle complex, multi-service treatment plans that may require expertise in evidence-based practices
Bilingual (English/Spanish) is strongly preferred but not required
Qualifications:
Education: Master's Degree in Social Work, Psychology, Counseling, or related field.
Required Experience:Two years' post-graduate experience in the direct treatment of persons with mental illness or emotional disturbance. One year's full-time experience in the treatment of children is preferred.
Licenses/Certificates/Registration:One of the following licenses is required for this position:

An LICSW,(Licensed Independent Clinical Social Worker), LPCC (Licensed Professional Clinical Counselor) or LMFT (Licensed Marriage and Family Therapist) will work with a group of non-profit primary care clinics serving North and Northeast Minneapolis and the northern suburbs. They provide quality health care services that are affordable and accessible. This individual will primarily work with kids ages 6-12 along with family members involved in a small team environment (4 therapy providers and 1 therapy telehealth provider). This individual will provide diagnostic assessment, treatment planning and outpatient therapy in both Spanish and English to patients, and work together with the Medical Directer and health care team including MDs, NPs, RNs, PAs, CNMs, MAs and LADC.

Job Requirements:
Provide individual and family therapy to children ages 6-12 using evidence-based therapeutic approaches.
Develop and implement individualized treatment plans aligned with each child's emotional, behavioral, and developmental needs.
Monitor client progress and adjust treatment interventions as needed,
Provide telehealth support as needed.
Participate in team meetings, case consultations, supervision, and trainings as required.
Adhere to ethical standards, confidentiality requirements, and professional best practices.
Maintain accurate, timely clinical documentation in compliance with agency, state, and federal requirements.

Qualifications:
LISCW, LPCC or LMFT in the state of Minnesota.
Master's degree or higher in psychology, counseling, social work, or marriage and family therapy.
Minimum of two years experience in direct patient care, experience in a primary care setting is preferred.
Must be bilingual (fluent) in English and Spanish.
Experience providing assessment and therapy to children ages 6 to 12 and using age-appropriate and developmentally sensitive approaches.
Great communication skills and attention to detail, comfortable with computers and documentation.
Knowledge of assessments, planning, impementation and care coordination in mentalhealth.
Skills in diagnosing patient conditions, interpreting findings from clinical evaluation/diagnostic tests, and risk assessment/intervention.
Demonstrated competence in independent clinical practice, work well individually and with a small team.
Reliable transportation to commute between clinical locations as needed.
