**How you make a difference**
The Operations Specialist is responsible for guiding and supporting the operational management team in clinical decision-making, policy development, and achieving company objectives across various sites. They provide expertise in correctional nursing and ensure compliance with accreditation standards such as ACA, NCCHC, and TJC. Additionally, the specialist may cover leadership roles at facilities for extended periods.
**Key Responsibilities**
+ Demonstrated technical and process knowledge and the ability to coach others in the leadership of operations procurement, flow and distribution including supplies, machinery, equipment or tools to be used or merchandise to be bought, stocked and sold.

* The medical director educates practicing physicians and others engaged in health care services regarding the hospice program and its potential benefits to patients.
In fulfilling the two primary functions explained above, the medical director performs the following duties:
* Quality of Clinical Care
* Assure appropriate evaluation and certification of terminal prognosis of patients.
* Assure the quality of initial plans of care.
* Assure the quality of comprehensive plans of care.
* Assure the accuracy of documentation.
* Review revocations.
* Review recertifications of terminal prognosis.
* Review the quality of pain and symptom management.
* Provide medical expertise on pain and symptom management to admission and patient care staff.

* Actively participate in formal QI functions and committees.

* Ensure that all contracted physicians (team physicians and consulting physicians) are properly credentialed via the VITAS Credentialing process.

* In Wisconsin the medical director also supervises the following functions of the consultant pharmacist:
* Ensure medications are utilized within accepted standards of practice.
* Ensure a system is developed and maintained that documents the disposal of controlled drugs.

* Educate community physicians on the principles of palliative medicine.
* Provide resource and consultative support to community physicians in palliative medicine.
* Attend and present at medical staff and other medical community conferences on palliative medicine.
* Serve as liaison between the hospice and community physicians.
* Make regular contacts with practicing physicians to introduce the hospice program, to educate physicians regarding individuals for whom hospice may be appropriate, and to answer clinical and other concerns of physicians with respect to hospice.
* Assist in introducing the VITAS program to long term care providers, managed care providers, hospitals, and others.
* Conduct educational seminars, in services, and presentations to physicians, nurses, and other health care audiences whose support for and understanding of the hospice program is integral to assuring that hospice services are made accessible to patients and families.

* Assist in the development of and actively participate in clinical training for all hospice patient care and admissions personnel.
* Actively participate in medical and nursing education programs on palliative medicine that may be provided by VITAS to medical and nursing colleges in the community.

* Be a member of and participate in professional organizations related to palliative medicine.
QUALIFICATIONS
* Knowledge of the principles and practice of primary medical care, with at least a working knowledge of oncology.
* Knowledge of palliative care with particular emphasis on control of symptoms associated with terminal illness.
* Ability to work collaboratively with patients' attending physicians to implement the hospice program and effect optimum medical care.
* Ability to work collaboratively with hospice employees and volunteers as part of an interdisciplinary team.
* Membership in good standing on the medical staff of an accredited hospital in the state in which the hospice is located.
* Medical malpractice and liability insurance in amounts and with carriers satisfactory to VITAS.
* Positive recommendation to the general manager or area vice president from corporate medical officer.
EDUCATION
* License to practice medicine in the state in which the program is operating.
* Board certification in a medical specialty which deals with care of the terminally ill, such as Internal Medicine, Oncology or Family Practice, with at least a working knowledge of medical oncology and demonstrated expertise in the principles of palliative medicine and symptom management.

Performs duties under the supervision of the Site Manager; Medical Assistant Tier 2 (MA 2) will closely work with the Registered Nurse and Provider/s in all clinical-related duties; prepares patients for medical office visits by charting in electronic health records (EHR) and preparing necessary paperwork as required; MA's adhere to the Medical Board of California scope of practice; responsible for following all clinical protocols/guidelines, objectives, philosophy and personnel policies of Community Health Systems, Inc. (CHSI).
DESCRIPTION OF KEY FUNCTIONS AND JOB RESPONSIBILITIES:
* Will smile; speak in a courteous and friendly manner with patients, visitors and staff members.
* Responsible for participating in quality improvement projects/activities and accountable for quality measures and PCMH.
* Shall report to the Site Manager for administrative related duties. Shall work closely with the Registered Nurse for clinically related duties.
* Communicates any form of safety concerns within the care facility to the clinical authorities.
* Create an incident report regarding the observed safety concerns within the care facility.

* Returns calls to patients and documents all communications in EHR; follows all guidelines of CHSI patient communication protocol.
* Adheres to medication reconciliation protocol and other important information into EHR prior to seeing the Provider.
* Participates in training as directed/required by the Director of Nursing/RN to provide better patient care such as learning evidence-based approaches to self-management support, proactively addressing the needs of patients/families in the community, cultural competency and effective communication.
* Assists in necessary training and development of medical assistant peers as directed.
* Maintains a safe, clean, and stocked environment at all times. Notify RN/Site Manager of any area or equipment that needs repair.
* Handles emergencies appropriately and seek qualified help when required per CHSI policy and protocols.
* Ensures patient readiness prior to entering the care facility by having them to undergo proper screening and educating them when necessary.

* Follow the required guidelines determined by each individual insurance plan/program.(i.e. PACT (Family planning program), (PE) presumed eligibility, sliding fee discount program, state health plans, Managed Care, etc.)
* Coordinates and schedules referral initial appointments and follow-up appointments as designated.
* Interact with patients', physicians and other staff both within the Clinic and at outside facilities providing accurate, timely and responsive information.
* Coordinate, processes, and routes all paperwork as required to meet physician and CHSI requirements.

As a hospice social worker, you will provide emotional and logistical support to patients and their loved ones during an important and vulnerable time. You will ensure VITAS patients reach the end of life with their affairs in order, honoring their wishes for finances, funeral arrangements, and more. WHERE YOU'LL WORK
Each day is different for our social workers. You will spend most of your time caring for hospice patients wherever they call home, which may be a private residence or an assisted living/skilled nursing facility.
About once a week, you'll come to the office to meet with the rest of your interdisciplinary team: the RNs, physicians, chaplains, and fellow nurses who work together to provide 360-degree support to the patients and families we serve.

Experience in providing counseling and support to patients and families.
