General Description: Hospice of Montezuma believes that Primary Nursing fulfills the goal of continuity of care for the unique needs of those who are terminally ill or seeking palliative care by establishing and maintaining the plan of care. Level of Responsibility: Works under supervision of the Hospice Patient Care Coordinator. Directly supervises CNAs assigned to patient/caregiver. Monitors contracted healthcare workers and provides feedback to Patient Care Coordinator.
Duties and Responsibilities: The Primary Nurse provides those services requiring specialized nursing skill: assesses patient condition and needs; develops and coordinates a comprehensive and on-going plan of care for each patient/caregiver; communicates changes in patient/caregiver condition and care with primary physician and Hospice team members; counsels the patient and caregiver in meeting nursing and related needs; teaches patient/caregiver and Hospice staff; and maintains current skills through in-service.
Admits patients and collaborates with the Hospice team to develop an ongoing plan of care for patient/ caregiver.
Implements, evaluates, and updates ongoing plan of care.
Documents all client contacts and care plan changes in patient's permanent record within 48 working hours.
Is available to patient/caregiver on a regular basis, takes rotation for 24 hour on call, and participates in continuous care.
Schedules nursing and CNA visits and collaborates with the Hospice team to ensure appropriate service for clients.
Submits all required documentation in a timely fashion, ensuring that Patient Care Coordinator or Quality Assurance Coordinator receive materials requiring review 5 days before the certification deadline.
Submits 90 and 60 day summaries to primary physicians with a copy to the Patient Care Coordinator.
Provides skilled nursing visits and appropriate paperwork when the level of care for the Hospice patient changes.
Teaches the patient/caregiver regarding specific aspects of the patient's care and course of disease in order to facilitate their participation in care.
Functions as an advocate for the patient/caregiver, ensuring the patient's right to cope with his/her disease process and death in the manner he/she chooses, within the scope of the law.
Teaches other team members as needed.
Participates in all Interdisciplinary Team Meetings.

Function:
To provide administrative and operational support to the Service Center and Thrift Store, including but not limited to supporting seasonal assistance/projects, seasonal fundraising efforts, such as our Christmas Kettle fundraising campaign, coordinating local volunteers, maintaining physical and electronic databases, providing social services to clients, and assisting the Thrift Store Manager with certain daily operations. This position will split time between the center's office and our thrift store.
Duties and Responsibilities
* Case Management/Work
* Ascertain needs of clients. Determine if emergency need fits TSA guidelines and prepare complete case information sheets
o Obtain photo ID, and other required documentation
o Identify appropriate resources and make appropriate referrals
o Update application forms, information forms, and other client forms
o Data input of case documentation using WellSky or other TSA databases
o Assist with center programs and seasonal assistance efforts
o Must have good working knowledge of TSA Family Services and all social services in the local area
* Administrative
o Manage walk-in clients and answer phone calls
o Helps manage volunteers, including coordinating volunteers for fundraising and other special events
o Assist in grant writing and fundraising
o Work with vendors to obtain invoices, W9's and other relevant paperwork
o Submit request for payments to AP Workflow, prepare all paperwork/vouchers, including all backup documentation required by auditors and submit to DHQ Finance Department for payment.
o Submit accurate weekly and monthly reports and statistics to DHQ in a timely manner using NSS application
o Ensure office supplies and hospitality items (e.g., coffee, tea, water, restroom items) are replenished and well-stocked
o Oversee the general appearance and cleanliness of shared office spaces
o Identify and report facility issues such as needed repairs, safety hazards, or supply shortages to the Service Center Director
o Attend meetings and trainings as requested
* Thrift Store
o Oversees the following functions:
* Oversee their center's homeless clothing program and all documentation needed for said program
* Assist the Thrift Store Manager in keeping the store clean, well-organized and clutter-free
* Recruit, train, oversee, and schedule dependable volunteers
* Maintain responsibility for store keys
* Keep store stocked with paper and cleaning products
o Assists the Thrift Store Manager with the following functions:
* Cash management which includes counting cash drawers, preparing deposits, ringing in sales,
counting back change, recording daily and monthly numbers
* Transporting donations and purchases in and out of the store

Job Description: Licensed Practical Nurse
General Description: Hospice of Montezuma believes that Licensed Practical Nurses are responsible, as a member of the interdisciplinary team, for providing care and comfort to patients according to written physician's or nursing plan of care. They work with the primary nurse and other team members to ensure safe and appropriate patient care.
Level of Responsibility: Works under the supervision of the Hospice Primary RN's and Patient Care Coordinator.
Duties and Responsibilities: The Licensed Practical Nurse provides those services requiring specialized nursing skill: respond to patient condition and needs; assists in developing and coordinates a comprehensive and ongoing plan of care for each patient/caregiver; communicates changes in patient/caregiver condition and care with primary physician and Hospice team members; counsels the patient and caregiver in meeting nursing and related needs; teaches patient/caregiver and Hospice staff; and maintains current skills through in-service.
Collaborates with the Hospice team to develop an ongoing plan of care for patient/ caregiver.
Implements, evaluates, and updates ongoing plan of care.
Documents all client contacts and care plan changes in patient's permanent record within 16 working hours.
Is available to patient/caregiver on a regular basis, takes rotation for 24 hour on call with support of triage RN, and participates in continuous care, when needed.
Schedules nursing and CNA visits and collaborates with the Hospice team to ensure appropriate service for clients.
Submits all required documentation in a timely fashion, ensuring that Primary RN or Quality Assurance Coordinator receive materials requiring review 5 days before the certification deadline.
Provides skilled nursing visits and appropriate paperwork when the level of care for the Hospice patient changes.
Teaches the patient/caregiver regarding specific aspects of the patient's care and course of disease in order to facilitate their participation in care.
Functions as an advocate for the patient/caregiver, ensuring the patient's right to cope with his/her disease process and death in the manner he/she chooses, within the scope of the law.
Teaches other team members as needed.
Participates in all Interdisciplinary Team Meetings at which clients assigned to the LPN have care plans updated, as needed.
Completes all documentation within 2 working days, and always prior to submission of timesheets.
Supports Hospice in the community through participation in fund-raising events, speaking engagements, etc.
Attends 20 hours of continuing education per year.
