PRIMARY FUNCTIONS:
The Interventionalist provides intensive, in-home crisis intervention services to youth at risk and their families.
SPECIFIC DUTIES:
· Carries a low caseload of up to 3 families for approximately 4-6 weeks for crisis services. Cases require multiple visits weekly, and crisis intervention, in the client's home environment.
· Conducts screening, assessment, and planning for youth/family in accordance with program standards.
· Collaborates with youth/family support in creating an individualized safety plan to ensure the safety of the physical environment.
· Collaborates with youth/ family to develop an evidenced- based, individualized treatment plan.
· Provides de-escalation and crisis support for youth and families.
· Collaborates with community agencies, coordinating services as needed to facilitate crisis management, family rehabilitation, reunification, and child permanency.
· Provides outreach, family support and education, and referral linkage to a variety of community resources.
· Maintains timely record keeping, and case documentation in the electronic health record in accordance with program standards.
· Provides reporting and statistical information as required.
· Participates in weekly individual or group supervision.
· Participates in staff meetings and agency wide events as scheduled.
· Attends and completes necessary training to ensure high quality, evidenced based service delivery.
· Provides on call services for Home Based Crisis Intervention program on a rotational basis.

QUALIFICATIONS:
(1 Licensure as a New York State Qualified Health Professional such as LMSW or LMHC, preferred. OR(2 Master's degree in related field with at least one year of relevant experience in community-based mental health or case management.OR

Possess excellent verbal and written communication skills.

Job Summary
Under the general supervision of Director of Sexual Assault Support & Advocacy Services, provides services for victims/survivors and their loved ones impacted by sexual and relationship violence and stalking as part of rape crisis team. In addition to providing services onsite, the Civilian Advocate provides services at area police departments and on campus. Works with Northwestern District Attorney's Sexual Assault and Domestic Violence Unit and Victim Witness Advocates to support victims involved in criminal justice processes and consults with legal system personnel (i.e., prosecutor, parole officer, advocates,) to gather or share information for safety planning and to discuss progress of the legal case in the criminal justice system.
This is a grant-funded position, dependent on funding availability.
Essential Functions
Provides crisis intervention; police, legal, medical and academic advocacy; safety planning and support to survivors of sexual and domestic violence and stalking in addition to their loved ones. Provides assistance to navigate police, legal and/or campus conduct processes and partners to referrals for wraparound services. Complies with all relevant regulations, policies, standards, and protocols including those regarding privileged communication, mandated reporting of abuse, data collection, record keeping, and security related to use of technology including cell phone and computers.
Builds and maintains effective working relationships with broad constituents including collaborating police departments, SAFEPLAN Advocates, Hampshire County Court, human service providers, and campus-based departments. May serve on relevant committees and teams. Provides outreach to community and campus-based programs to inform providers, educators, and survivors about CWC services.
Coordinates and provides trainings related to sexual and domestic violence to partners and CWC staff. Maintains current knowledge of relevant legal processes. Functionally supervises advocates regarding legal advocacy as needed.
Assists with coordinating and facilitating grant related meetings, including Sexual Assault Response Team (SART) and partner meetings; reviews, evaluates, and revises interagency protocols. Compiles data and prepares reports including tabulating statistics on sexual and domestic violence calls and reports. Participates in site visits and program development as needed.

Compensation:
Horizon is consistently evaluating our compensation and benefit packages to ensure we remain competitive. The range of pay for this position is $50,960 - $62,296. This Position is Contingent on Contract Award.
This position is in support of a proposed repatriation initiative. The purpose of the facilities is to provide comprehensive care for migrant individuals who are awaiting reunification or repatriation. This care will begin as soon as each occupant arrives at the facility and will last until their arranged departure. The Substance Use Counselor (Methamphetamine Recovery Specialist) is responsible for providing psychosocial assessments, collaborative treatment planning, engagement, referrals and evidenced-based clinical interventions as indicated for individuals dealing with a variety of substance use and co-occurring mental health concerns. While the Substance Use Counselor (Methamphetamine Recovery Specialist) may assist patients with any substance use disorder issue, the position primarily focuses on clinical issues relating to methamphetamine use and/or Chemsex activities. The counselor will create a sex-positive, judgement-free atmosphere, utilizing a harm reduction approach to services and supports to meet the needs of members of the LGBTQIA community and others. A particular focus of the work is engaging with and meeting the behavioral health treatment needs of men who have sex with men who are people of color. This position collaborates extensively with the Pride Center of WNY to create, deliver, and report outreach/engagement/treatment services for individuals who self-identify as LGBTQIA and using methamphetamine.
As part of the essential functions of this role, Substance Use Counselor (Methamphetamine Recovery Specialist) :
Conducts initial psychosocial assessments and provides treatment recommendations, including the collaborative development of a harm reduction and evidence-based treatment plan in partnership with the patient.
Effectively assesses and develops clinical recommendations to address the frequently co-occurring needs of individuals using methamphetamine and/or engaging in Chemsex activities, including recovery from chemical dependency, compulsive sexual behaviors, internalized homophobia/gender identity and other self-acceptance issues, and past traumatic experiences.
Provides individual, group, and crisis intervention counseling services to patients, at all stages of change, via both walk-in and pre-scheduled visit types.
Equips patients with the knowledge and resources to improve their general health through activities associated with reducing risk of drug overdose/using more safely, managing/decreasing substance use, extending periods free from substance use and/or other unwanted sexual behaviors, reducing risk of HIV, STI, HCV, and other communicable disease transmission, and/or developing strategies to avoid missed doses of prescribed medications.
Makes referrals to a wide range of medical, behavioral health, family, and other social support/service providers in the community, based on the unique needs and preferences of each patient.
Actively engages, to the extent each individual is comfortable, with patient identified family, friends, and other collaterals to support their treatment goals.
Assists with the development of culturally affirming services for and engages in on-going continued education related to treatment of individuals who use methamphetamine, including those who engage in Chemsex activities, are people of color and/or are part of the LGBTQIA community.
