Sheboygan Home Care Team- Weekends- approximately one per month.No Holiday requirement.M-F: 745 am-430 pm
Major Responsibilities:
Conducts a physical therapy evaluation of each patient and establishes a plan of care and treatment goals. Interprets physician referrals and completes musculoskeletal, neuromuscular, cardiopulmonary, and/or integument evaluations utilizing appropriate methods. Evaluates outcomes within an appropriate time frame.
Provides on-going assessment of patient response to treatments and teaches and applies interventions as appropriate. Updates and revises plan of care as indicated and provides physician with status report on a regular basis.
Communicates problems to physicians and management and clearly states plans, actions and goals so changes are reflected as they occur. Completes a plan of care as indicated and provides physician with status report. Maintains and updates clinical and patient records according to state and federal guidelines.
Manages assigned caseload including scheduling and informing site leadership of coverage needs and ability to provide coverage. Manages patient's medical condition including medications. Acts as a patient advocate to insure optimal outcomes. Manages assigned caseload for optimal functional and financial outcomes; reviews caseload for Low Utilization Payment Adjustments (LUPA), optimal number of visits based on progress, Prospective Payment Plan (PPS), and Outcome Based Quality Initiatives (OBQI) outcomes.
May manage a caseload of Physical Therapist Assistants (PTA) including evaluating their patients, insuring compliance with established Care Plan, and adhering to all State of Wisconsin regulations regarding PTA supervision.
Performs Home Health Aide joint visits as appropriate to maximize patient's functional abilities in the home. May assist with evaluating patients, turning patient over to PTA and informing PTA of Plan of Care (POC) per State of Wisconsin regulations. Teaches appropriate treatment procedures and supportive activities to patients and caregivers and develops plan for obtaining adaptive devices as needed.
Discusses caseload issues and changes in case census with management. Communicates problems to physicians and management, and clearly states plans, actions and goals so changes are reflected as they occur.
Participates in chart audits and continuous quality improvement activities. Facilitates discharge planning process and utilizes interagency/system and community resources to assure continuity of care after discharge.
Acts as a resource to employees, which includes training/orienting, mentoring, and providing guidance on more complex issues. Serves as an instructor for students, meeting all expectations of the Clinical Student Program.
Performs various patient positioning and transporting duties, which require lifting and pushing/pulling, while utilizing proper technique.
Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. Age-specific information is developed further in the departmental job standards.
Licensure, Registration, and/or Certification Required:
Physical Therapy License in the State of Illinois or State of Wisconsin, must be licensed in the State upon which you work.
Basic Life Support (BLS) for Healthcare Providers (obtained within 90 days unless department leader has determined is not required) by American Heart Association (AHA)
A valid drivers license issued by the Division of Motor Vehicles.
Education/Experience Required:
Doctorate degree in Physical Therapy from an accredited school or educational requirements commensurate with the date of graduation. Maintains continuing education requirements as mandated by applicable state professional standards.
Requires 1 year of clinical experience or demonstration of strong spectrum of clinical affiliation experience
Knowledge, Skills & Abilities Required:
Demonstrated knowledge of indications and contraindications of modalities and exercise.
Demonstrated skill in use of modalities, exercise equipment, and other devices that are available in work area.
Demonstrated proficiency in clinical skills with the ability to work independently and make sound judgments.

Physical Requirements and Working Conditions:
Must be able to obtain transportation to attend to the daily schedule of home visits and other business in a timely manner.
Must be capable of the manual dexterity required to use and operate a variety of medical equipment and supplies, writing implements and office equipment.
Must have the physical ability to move about in confined spaces, including bending, twisting, kneeling, squatting and occasionally reaching one or both arm overhead.
Must be able to work safely with people of various sizes and weight with diverse and complex medical conditions and physical limitations.
Must be able to:
Lift up to 50 lbs from floor to waist.
Lift up to 20 lbs from waist to overhead.
Lift and carry up to 40 lbs. at waist height reasonable distance.
Must be able to:
Push/pull with 30 lbs. of force.

Major Responsibilities:

Assess and support Club member's behavioral challenges by working one-on-one with them for a wide range of presenting concerns
Oversee daily operations of the Sensory Room
Facilitate Sensory Room sessions with Club members
Conduct small group programs for Club members as needed to support Social Emotional Learning
Work with Club members to understand and define individual goals based on their behavioral challenges to promote full program participation
Guide Club members to reduce or eliminate concerning behaviors at Club
Participate in team meetings with Branch staff to discuss support for Club members
Maintain organized and detailed case files on all Club members receiving behavioral support services
Demonstrate commitment to learning key trauma concepts and applying trauma informed practices
Demonstrate commitment to diversity, equity and inclusion
Develop positive relationships with Club members, families, and staff by being visible and accessible throughout the Branch Club programs

Qualifications
The Youth Advocate requires a majority combination of the following:
Education and Experience
A bachelor's degree in social work, sociology, psychology, human services, or related field preferred but not required.
Minimum of at least two years of experience working with at-risk children, youth, and families.
Demonstrated record of some or all of the following: child and adolescent development, behavior management techniques, social emotional learning & trauma informed care.
Skills and Requirements
Ability to prioritize and organize multiple tasks and responsibilities in a manner that fulfills daily responsibilities while continuing to make progress towards longer-term objectives.
Exceptional communication skills, with the ability to convey information in a timely, open, and clear manner.
Reliable transportation.
Knowledge of and ability to effectively use computers, software, and other technology tools.
Ability to establish and maintain effective working relationships with others.

Working Conditions
Work will occur both in a climate-controlled environment and outdoors with limited exposure to safety or health hazards. This role routinely uses standard office equipment such as computers, phones, photocopiers, and filing cabinets as well as non-hazardous program supplies and equipment such as basketballs, paint, markers, billiard balls, musical instruments, etc. This role also requires the use of vehicles to transport youth and staff.
Physical Demands include:
Must be able to move independently indoors and outdoors.
Must be able to operate a computer and other office productivity equipment, such as a phone and photocopier.
Must be able to communicate efficiently and effectively.
Must be able to quickly detect safety concerns.
Must be able to work effectively in outdoor weather conditions.
Must be able to work effectively in an environment where the noise level may be loud at times.

Position Overview:
The Behavioral Support Assistant will provide security and supervision of clients, monitor their whereabouts and behavior, maintain paperwork (daily logs, progress notes, event reports), conduct facility checks, and assign client activities (chores, tasks, urinalysis/breathalyzer testing). The Behavioral Support Assistant will supervise diverse clients in duties such as meal preparation and household chores to help provide the necessary skills to maintain responsibility, take initiative, and accountability.
What You'll Do:
· Provide a safe environment for clients.

Qualifications
Education & Work Experience:
High School Diploma or Equivalent.
One year experience and/or education relating to corrections, community based residential facilities (CBRF), criminal justice, human services, psychology, sociology, security, social service, mental health, or law enforcement preferred.
Knowledge, Skills & Abilities:
Excellent interpersonal skills and demonstrated ability to work well with individuals of diverse backgrounds.
Ability to interact with staff, clients, and agents in an effective manner while maintaining professional boundaries.
Ability to maintain the utmost confidentiality of all agency information.
Strong decision making abilities and able to work independently.
Knowledge of nutrition and sanitation.

We are seeking Crisis Counselors to join our team. This full-time position is responsible for providing 24-hour counseling services for consumers in need of emergency mental health services. Services include assessment of consumer functioning and implementation of appropriate short term treatment plan developed in tandem with consumer and designed to meet consumer short term physical and mental health needs. The Crisis Counselor works cooperatively with other community professionals to maintain consumer safety through assessments for general mental health, suicide, homicide, and ability to care for self.
Key Responsibilities
Serve as a liaison between the consumer and other service organizations in providing and receiving referrals.
Provide 24-hour crisis intervention services.
Assess consumer mental health status and coordinate emergency services.
To assess for and implement Chapter 55 Protective Placements and subsequent placement at appropriate facilities when necessary after normal business hours for county Adult Protective Services.
Qualifications
Education:
Required: Bachelor's degree or equivalent work experience
Extensive training is provided to new hires so new graduates are encouraged to apply.
Skills and Competencies:
A working knowledge of human growth and development, mental health, adolescent psychology, marital and family structure, and the impact of the environment on individual behavior.
Time management skills and the ability to meet work demands on a moderately independent basis.
Ability to establish and maintain relationships within and outside the agency in a professional manner.
Ability to evaluate one's own performance, capabilities, and limitations with reasonable accuracy.
