POSITION SUMMARY: Provides services in the areas of outreach, assessment, vocational exploration, case management, counseling, development of customers' individual plans, job search assistance and follow-up services; assists in preparation of administrative assignment.
ESSENTIAL RESPONSIBILITIES include the following:
1. Initiates and maintains contact with youth, High School staff, employers, job services and community agencies who express interest in the In-School Youth program opportunities.
2. Provides supportive services to at-risk youth and interprets and/or makes appropriate referrals.
3. Conducts initial assessment interviews and interprets testing results as part of initial assessment.
4. Places youth in subsidized and unsubsidized employment.
5. Provides job search assistance to participants.
6. Assesses skill levels and service needs of participants, including administering and interpreting objective assessment, test results, completing formal objective assessment, to include training and employment needs of the youth, and determining supportive services needs of customers.
7. Prepares and completes Individual Service Plans.
8. Provides counseling to at-risk youth, such as job counseling and career counseling.
9. Provides case management services to participants.

Position Summary:
The Case Management Assistant (CMA) provides administrative and care coordination support to the Population Health Management (PHM) team, including Nurse Case Managers and Social Services staff. The CMA assists by conducting outreach, gathering and organizing information, coordinating appointments and referrals, documenting activities, and helping to address routine member needs. This role is member-focused, ensuring individuals receive timely communication, follow-up, and access to services and resources. The CMA supports member engagement activities such as wellness calls, enrollment outreach, and care gap closure, helping to improve the overall member experience and promote better health outcomes. The CMA requires strong interpersonal and organizational skills to communicate effectively with members, assist with referrals, and help coordinate services among providers, community resources, and internal care teams. The CMA also supports the Interdisciplinary Care Team (ICT) activities by gathering information, preparing documentation, and relaying member needs to the case management team.
Requirements
Job Duties and Responsibilities:
• Work collaboratively with Nurse Case Managers and Social Services staff on care coordination, member follow-up, communication with agencies.
• Support PHM outreach activities by conducting calls for program enrollment, Health Risk Assessment (HRA) completion and other relevant questionnaires, wellness checks, and quality initiatives to help close care gaps.
• Contact members at regular intervals per their acuity level and care plan needs.
• Document all outreach, coordination activities, and member interactions in the case management system (CM System) accurately and in a timely manner, in alignment with program protocols.
• Support members experiencing transitions of care by assisting case managers with coordination of post-discharge needs, follow-up appointments, and transportation.
• Gather clinical information and assist with coordinating services such as home health care, prescriptions, durable medical equipment (DME), and social service referrals.
• Prepare and send member correspondence, as directed by case managers or in alignment with case management protocols.
• Report and escalate member concerns, variances, or changes in condition to the appropriate care team members to ensure timely intervention and coordinated follow-up.
• Assist members with appointment scheduling, transportation, referral coordination, and other care coordination needs.
• Gather clinical information from outside sources such as PCPs, specialists, hospitals, SNFs, and electronic health records, and upload them into the case management system.
• Verify member eligibility, demographics, and benefits; confirm provider assignments to ensure authorizations are linked appropriately.
• Support closed-loop referrals to community supports, housing, and social service agencies, with follow-up to confirm services were delivered.
• Participate in ICT meetings by gathering and presenting information and communicating member needs and preferences to the care team.
• May be assigned to manage a caseload of low-acuity PHM members, under the supervision of Nurse Case Managers or Social Services staff.
• Serve as an associate and resource to members, providers, staff, and external customers regarding policies, benefits, and care coordination.
• Provide administrative support, including answering phones, assisting with correspondence, filing, scanning, and maintaining department data systems.
• Receive and route incoming faxes, medical records, and other documentation to the appropriate case management staff.
• Track and log outreach attempts and results in the case management system to support reporting and follow-up.
• Conduct outreach to members to verify needs are being met and services delivered.
• May be assigned to conduct in-person meetings with members during clinic visits.
• Assist with training and orientation of new staff, as requested.
• Assist case management team with oral interpretation, as applicable.
• Attend mandatory staff and departmental meetings.
• Perform other duties as assigned.
Qualifications:
• High School diploma or GED required.
• Minimum of 3 years of experience in a healthcare or community health setting.
• Knowledge of case management processes and regulatory requirements for Medi-Cal, Medicare, and commercial programs preferred.
• Experience in a managed care environment preferred (IPA, HMO, or health plan).
• Medical Assistant or Community Health Worker certification preferred.

Responsibilities

The Case Management Assistant (CMA)- Transitional Care Services provides support to the Population Health Management (PHM) team with a focus on members undergoing transitions of care. The CMA assists in enhanced care coordination activities for members transitioning between settings such as emergency departments, inpatient admissions, and post-acute facilities. This role supports members across the PHM continuum, from low risk to highly complex, by coordinating services, scheduling timely follow-up appointments, gathering clinical information, and conducting outreach to confirm that needs are being met. The CMA plays a proactive role in contacting members during transitions, including prior to discharge, to help coordinate the transition process and post-discharge follow-up. The goal is to reduce readmissions and avoidable ER visits by supporting effective transitional care services. Job Duties and Responsibilities:
• Work collaboratively with the PHM team, including nurse case managers, social workers, and TOC clinic staff, to support members in need of transitional care services.
• Contact members during care transitions, especially prior to discharge, to help coordinate the transition process and post-discharge follow-up, with the goal of reducing readmissions and avoidable ER visits.
• Gather clinical information and assist with coordinating post-discharge services, including home health care, prescriptions, durable medical equipment (DME), and transportation, ensuring referrals are received and confirming members understand follow-up needs.
• Schedule primary care provider (PCP) post-discharge visits and/or Transitions of Care (TOC) Clinic appointments in a timely manner.
• Proactively coordinate with referral sources and internal partners to support seamless member transitions between care settings. • Support PHM members of all risk levels, from low to complex, who are experiencing transitions of care (e.g., ED, inpatient, post-acute, and other settings).
• Contact members at regular intervals per care plan needs and acuity level, or as directed by the member's Primary Case Manager.
• Initiate and/or complete applicable assessments, such as Health Risk Assessments (HRA), initial assessments, and/or Transitional Care Services (TCS) assessments, and consistently document activities and encounters in the case management system.
• Support case managers with follow-up, communication with agencies, and preparation/distribution of documents or reports.
• Report and escalate member concerns, variances, or changes in condition to the appropriate care team members, including Nurse Case Managers and Social Services staff, to ensure timely intervention and coordinated follow-up.
• Assist members with appointment scheduling, transportation, referral coordination, and other care coordination needs.
• Gather clinical information from outside sources such as SNFs, PCPs, specialists, hospitals, and applicable electronic health record systems, and upload them in the case management (CM) system.
• Ensure relevant TOC Clinic records are shared with the member's PCP to help facilitate continuity of care.
• Verify member eligibility, demographics, benefits, and case management program eligibility.
• Ensure closed-loop referrals to community supports, housing, and social service agencies, with follow-up to confirm services were delivered, including services authorized by the organization.
• Participate in Interdisciplinary Care Team (ICT) meetings by gathering and presenting information and communicating member needs and preferences.
• Serve as an associate and resource to members, providers, staff, and external customers regarding policies, benefits, and care coordination.
• Provide administrative support, including answering phones, assisting with correspondence, and maintaining data systems.
• Conduct outreach to members to verify needs are being met and services delivered.
• May be assigned to conduct in-person meetings with members during clinic visits.
• Attend mandatory staff and departmental meetings.
• Assist with training and orientation of new staff, as requested.

We're looking for someone to join our team as a Youth Advisor-AFS who:
The youth advisor has an integral role in the AFS department, specifically in the Adolescent Family Life Program. The part-time youth advisor works 10 hours per week and is responsible for providing youth input for program implementation, including participation in local stakeholder meetings, MCAH/funder meetings, community outreach events, supporting program quality assurance efforts, and engaging AFLP participants in the program. The youth advisor may help with recruitment efforts, data collection, and event planning.
Essential Functions:
1. Shadow case managers and/or supervisor on participant visits to connect with program youth
2. Greet participants and engage in a friendly, positive tone
3. Participate with other AFLP staff in community and agency meetings and events
4. Participate in monthly MCAH meetings
5. Participate in the AFLP quality process
6. Comply with and support Clinica Sierra Vista and AFS policies and procedures
7. Maintain confidentiality and security of client data at all times
8. Reasonable and predictable in person attendance.
9. Other duties as assigned
Education, Experience, and Skills Required:
1. Recent high school graduate
2. Expectant or parenting
3. Current or former AFLP participant strongly preferred
4. English/Spanish bilingual strongly preferred
5. Flexible schedule for events and meetings

What You Will Do to Change Lives
Under direct supervision, the Case Manager II serves as a primary support and case manager for individuals with mental illness and other co-occurring disorders by assisting them to live as independently as possible in their chosen community.
Shifts Available:
Full-Time | AM | Shifts: 8:30 AM - 5:00 PM | Days: Monday - Friday
Expected starting wage range is $21.00 - $23.71. Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.
What You Bring to the Table (Must Have)
High School Graduation or G.E.D. equivalent and two (2) years of direct services in Mental Health or community services; or Associates degree and one (1) year of experience; or Bachelor's degree in Social Services
Valid and current driver's license, and personal vehicle insurance with your name listed as a driver.
Willingness to use your personal vehicle to drive clients to appointments and groups, etc.
