The Behavioral Health Specialist provides short-term counseling, crisis intervention, and behavioral health assessments within an integrated primary care setting. They collaborate closely with medical providers, coordinate care with community resources, and offer education to patients, families, and staff. Essential Job Functions/Responsibilities:
Provides short term counseling for patients with Behavioral Health needs.
Orients patients to Behavioral Health Program.
Clinically assesses patients for appropriateness of behavioral health services. Establishes a psychiatric diagnosis.
Works with patients (and Primary Care Provider, if indicated) to establish and obtain treatment goals. Monitors follow-up.
Provides crisis intervention services to patients experiencing acute emotional distress.
Contacts may be face to face or by phone.
Contacts may include patients who are experiencing chronic pain/addiction issues.
Consults with medical providers and other professionals on patient treatment and/or services.
Communication with primary care providers, external psychiatrists, community therapists, community mental health centers, treatment facilities or other community resources.
Provides behavioral health education.
Instructs patients and families on management of behavioral health issues.
Provides patients and families with educational materials for behavioral health issues that are both paper and web based.
Educates staff on behavioral health issues.
Provides referral of patients
Identifies patient needs that may require referral to outside community services.
May refer patient to primary care provider for intervention and/or follow-up.
Assists in referral of patients to outside agencies, community therapists, psychiatrists and/or community resources.
Meets regularly with administrative staff, nursing and medical providers to identify, assess and support patient behavioral health needs.
Completes all EMR and other documentation as required; daily, weekly and monthly.
Records results of assessment and treatment goals through a progress note using behavioral health templates.
Uses messaging system/review bins for provider and/or nursing communications.
Documents patient contacts through EMR notes.

Position Qualifications:
Education
Master's Degree Social Work preferred, but will consider Master's in a related, license eligible field.
Licensure as a licensed mental health provider, or ability to obtain licensure is required.
Experience
At least one year of experience actively working with patients in the field.
Prior counseling experience, knowledge of DSM V-5.0 and knowledge of primary care behavioral health needs preferred.
Must provide continued proof of Professional Development as required by State professional licensure board.
Knowledge and Ability
Ability to provide triage, assessment crisis management and short term counseling interventions in an integrated primary care/behavioral health model.
Must maintain a high level of confidentiality.
Strong organizational skills are essential, as well as the ability to multi-task, respond to shifting priorities, and to work well under pressure while meeting all required deadlines.
Ability to work independently while demonstrating the skill to work positively within the framework of a team.
Must have proficiency in computer skills including, but not limited to email functions, spreadsheets, document processing and Electronic Medical Records.
Typical Physical Demands
Requires prolonged sitting, some bending, stooping and stretching.
Requires eye-hand coordination and manual dexterity sufficient to operate a keyboard, photocopier, telephone, calculator and other office equipment.

A Day in the Life as a Breastfeeding Peer Counselor:
As a Breastfeeding Peer Counselor, you'll be responsible for providing in-person basic breastfeeding information and encouragement to WIC pregnant and breastfeeding mothers.
Your day will involve:
Provides information, benefits, and reasons to breastfeed to pregnant and post-partum women in accordance with Agency policies and procedures and the USDA Breastfeeding Training Curriculum and counsels WIC prenatal and breastfeeding women at WIC clinics through face-to face counseling, telephone support, or through breastfeeding classes and support groups
Adheres to WIC USDA Breastfeeding Training Curriculum and NH Breastfeeding Peer Counseling Program requirements and refers breastfeeding problems, questions or concerns that are beyond existing level of knowledge, scope of practice or require immediate attention to the Breastfeeding Peer Counselor Program Coordinator (DBE) or nutritionist who is a CLC. Refers mothers to the local agency breastfeeding coordinator, DBE, lactation consultant, health care provider, or other community support program, according to local agency protocol and scope of practice.
Oversees a caseload of WIC participants and makes routine contact with all participants assigned by local agency Breastfeeding Coordinator and gives basic breastfeeding information and support to new mothers, including discussing the benefits of breastfeeding, overcoming common barriers, and how to establish early successful breastfeeding. Also helps women prevent and handle common breastfeeding concerns within scope of practice.
Attends clinics to provide in-person services to assist participants with breastfeeding needs as well as contacting participants by phone on days not scheduled for clinics.
Contacts postpartum women electing to breastfeed after delivery by phone or by office appointment to assist in breastfeeding initiating questions and concerns, provide information and support, encourage the continuance of breastfeeding and to respond to any breastfeeding concerns or problems and arranges follow-up contacts in accordance with schedule outlined in the NH Policy and Procedure Manual and as needed and indicated by WIC participants to provide necessary support and information to encourage a successful breastfeeding experience.
Maintains accurate records of all contacts and completes weekly activity worksheets documenting paper record information into the StarLINC System for WIC in a timely manner.
Will attend required Agency meetings with supervisor and other peers, WIC BFPC monthly staff meetings, as scheduled, and required in person clinic days. Attends state sponsored breastfeeding peer counseling meetings, special conferences or in-services on breastfeeding to increase knowledge and skills.

Lund's Regional Partnership Program (RPP) seeks a dedicated and driven Case Manager to support families, create meaningful change, and address substance use challenges in our communities. RPP Case Managers work out of local DCF Family Services offices to improve the well-being and permanency outcomes of children affected by parental substance misuse. This position is split between two DCF offices, working approximately 20 hours at the St. Johnsbury office and 20 hours at the Morrisville office.
In close collaboration with DCF staff, RPP Case Managers play a vital role at the front end of child protection, screening for problematic substance use, linking parents to appropriate treatment services, and addressing barriers to successful engagement. By increasing access to and participation in treatment, RPP Case Managers provide support at the individual level and across the broader care system, ensuring families receive the help they need to thrive.
This role conducts substance use screenings, coordinates referrals to assessment and treatment providers, and fosters treatment engagement by addressing transportation, childcare, or stigma barriers. Case managers provide consultation and training to DCF staff on substance use and treatment services while liaising between the treatment system and DCF Family Services. Services are delivered in various settings, including DCF offices, community spaces, and caregivers' homes, ensuring that families receive support where they need it most.

What You Will Bring:
Minimum of an associate degree required. A bachelor's degree in social work, counseling, or a human services field is preferred.

Background in Trauma Focused Cognitive Behavioral Therapy (TF-CBT) and Attachment Self-Regulation and Competency (ARC) principles and/or willingness to learn is imperative. Qualifications

The Becket Family of Services, Vermont Permanency Initiative is seeking a full time Milieu Clinician (MC) to join our team. The role of the MC is to provide individual and group therapy as well as day to day oversight and treatment for approximately 10 adolescent males between the ages of 11-21. The VPI Program, located in Campton New Hampshire, utilizes an outdoor education and wilderness therapy approach seeking to replace maladaptive behavioral patterns with lifelong pro-social skills. Many youth within the program have a history of co-occurring mental health disorders. Background in Trauma Focused Cognitive Behavioral Therapy (TF-CBT) and Attachment Self-Regulation and Competency (ARC) principles and/or willingness to learn is imperative. Common responsibilities include providing counseling to students while providing strengths based interventions and pro-social incentives, communicating treatment plan needs and assisting with general oversight for the duration of the time in treatment.
Qualifications
Master's Degree in social work, psychology, counseling, or related field in addition to two years experience counseling adolescents and/or families. Ideal applicants will be licensed or be working towards licensure.
