Job Title & Role Description:
The Care Specialist is a key member of the team responsible for delivering chronic care management to high-complexity patients. The Care Specialist primarily works in patients' homes and communities (90% of the time) and engages in virtual care (10% of the time). This role involves direct outreach to patients, guiding them through the enrollment process, facilitating virtual appointments with providers, and ensuring that patients adhere to care plans. By building relationships, coordinating care, and providing education, the Care Specialist plays a critical role in promoting health, preventing unnecessary hospitalizations, and improving patient outcomes.
Skills Required:
Minimum of 2 years of relevant work experience (e.g., Community Health Worker, Peer Support Specialist, Medical Assistant, etc.).
High school diploma or GED required.
A valid driver's license, auto liability insurance, and reliable transportation to travel within the assigned territory.
Experience in chronic care management or working with chronically ill/elderly patients.
Technologically proficient with basic computer skills (typing, using EMR systems).
Experience with motivational interviewing, trauma-informed care, and care coordination.
Strong interpersonal communication skills with the ability to engage patients and team members effectively.
Ability to prioritize tasks, manage schedules, and work independently in an unstructured environment.
Multi-lingual skills are a plus but not required.
Prior home care experience is beneficial.

Compensation
The hiring range for this position in Glendale, CA is $212,000.00 to $274,200.00 per year. The base pay actually offered will take into account internal equity and also may vary depending on the candidate's geographic region, job‑related knowledge, skills, and experience among other factors. A bonus and/or long‑term incentive units may be provided as part of the compensation package, in addition to the full range of medical, financial, and/or other benefits, dependent on the level and position offered.

About The Role:
As our Commercial Counsel, you will support our cross-functional partner teams and our growing sales team in the Americas. We are looking for a team member who is excited about our passionate user base and is interested in joining a dedicated cross-functional team that engages with our customers, community, and vendors.
What You\'ll Achieve:
Have responsibility for providing practical day-to-day commercial legal support to the Notion Sales team in the Americas, including negotiation of customer subscription agreements.

Ability to confidently and independently manage and negotiate SaaS agreements with a wide variety of customers
Strong understanding of cloud services and general SaaS business practices, and a willingness to dive into Notion\'s product offerings, operational processes, and security and privacy practices and policies in order to build a strong foundation for providing support across all relevant lines of business
Resilience, resourcefulness, the ability to directly interact and build meaningful relationships and trust with internal and external clients and leaders, and the ability to make recommendations and decisions with imperfect facts
Experience negotiating with technology and professional services vendors
Excellent drafting, written and verbal communication skills
Demonstrated interest in artificial intelligence solutions in a work setting.
Bonus Points:
Certified Information Privacy Professional or equivalent
Experience supporting channel and reseller partnerships
