Essential Functions:
Provides prescreening of patients in crisis and linkage case management services in a team-based environment. Provide and coordinate a continuum of tx services between the ED and the Welcoming Center. Meet productivity requirements as established by program director. Complete and submit all necessary reports and documentation accurately and on time, as required by the agency and funding source. Demonstrate sensitivity and responsiveness to cultural differences present in the organization and its service population. Must work one unscheduled shift per month and at least two scheduled shifts per month based on program needs.
Position Qualifications:
Bachelor's degree in Social Work or in a related human services field (psychology, counseling, or human services); Master's degree preferred. Preferred 1 year experience assessing patients with mental health diagnoses, counseling, crisis intervention, or case management. Preference for CADC or one year experience utilizing substance use interventions. Ability to work in a team environment with demonstrated empathy and maturity. Must pass the IM_CANS or IM-CATS training & test.

CASE MANAGER / CAREER COUNSELOR



EMPLOYMENT & TRAINING DEPARTMENT



FULL-TIME, FULL YEAR, 30 TO 40 HOURS PER WEEK, NORTH KINGSTOWN



GENERAL RESPONSIBILITY:
Responsible for successful completion of the Intake, Assessment & Individual Career Plan for clients primarily 14 to 24. Must possess strong leadership qualities and have experience identifying and addressing various barriers that may inhibit successful completion of approved service plan.


SPECIFIC RESPONSIBILITIES
:
Must be able to work independently to manage clients at varying stages of success that may need employment supports, conflict resolution, internal / external referrals, and long and short term follow up. The ideal candidate will possess the necessary skills to offer clients positive reinforcement that provides the motivation necessary to achieve individual career goals and realize their potential as members of the labor force.Responsible for managing & tracking client participation in all service plan activities via approved data base(s) for the purpose of ensuring compliance with mandated performance outcomes. Client service plans include: setting personal & professional goals; creation of an ideal household budget; exploring labor market information; participation in career & professional development workshops; work readiness & soft skills training; exploring career options which support ideal lifestyle; participating in paid and unpaid work experiences; facilitating enrollment in advanced training, military, apprenticeship or short / long term credential trainings; transition to college; financial & digital literacy; repairing credit & credit counseling; employment.


QUALIFICATIONS
:
Previous experience working with youth, understanding of agency vision and mission, knowledge of primary service area, ability to work independently to meet program goals, previous experience working with at risk, LD population, aged 16 to 24, workforce preparation experience preferred, Bachelor's degree required
