The home base for this position will be the Buzz Westfall Justice Center in Clayton working with individuals in the County jail. This position will also work in all three of the Department of Public Health Clinics to include John C. Murphy in Berkeley, North Central Community Health Center In Pine Lawn and the South County Health Center in Sunset Hills. The annual starting salary will be $64,480. Work with health center staff to educate them on therapeutic communication and effective client interaction. Maintain excellent interpersonal skills, able to work independently as well as within a team and represent DPH professionally both internally and externally. Submit reports on patient outcomes and productivity. Complete all patient documentation in the Electronic Medical Record according to DPH standards. Maintain HIPAA compliance. Serve on DPH committees and work groups as assigned. Actively participate in patient huddles with other team members. Perform additional duties as determined needed for this job class. Minimum Qualifications Graduation from an accredited college or university with aMasters Degree in Social Work or Counseling and a minimum of one year of related counseling experience or any equivalent combination of experience and post graduate training. Must possess a valid driver's license. Licensure by the State of Missourias a Licensed Clinical Social Worker (LCSW)or Professional Counselor (LPC) must be obtained prior to completion of a six month probationary period. Prefer experience working with adult and pediatric patients. Additional Information SELECTION AND APPOINTMENT: A selection committee will evaluate the qualifications of each applicant which will include recentness and appropriateness of the applicant's education and experience, answers to the supplemental questions, results of employment references investigated and any other appropriate information available. The selection committee will admit to the interview only those persons who possess the most job-related qualifications and may establish admittance criteria beyond the minimum qualifications stated above. As a Family Counselor, you will meet with clients and families in their homes and other community-based locations, providing individualized services aimed at strengthening relationships, addressing trauma, and building positive community connections. The Family Counselor also coordinates services with other relevant providers and develop appropriate referral plans, completes thorough and timely documentation, including assessment and service notes, using a web-based client services platform, and provides after-hours support to remote field staff on an on-call rotation. Core Responsibilities and Essential Job Functions: Provides crisis intervention and conflict resolution with youth and their families as necessary. Provides individual and family counseling according to program model. Provides comprehensive case management services to a caseload of clients which include: needs assessment, service planning, case coordination, service brokerage, linkages with community resources, and monitoring progress toward client(s) goals and objectives and makes recommendations for on-going services when necessary. Establishes trusting, nurturing, therapeutic relationships with clients and their families. Arranges and/or provides services needed to meet the immediate needs of client(s). Assesses youth and family functioning for safety, risk and protective factors and determines appropriate interventions. Formulates service plans and goals with client(s), obtains supervisory approval of the plan, and is responsible for the implementation of the plan. Analyzes and evaluates progress in the service plan and modifies when necessary to meet the needs and goals of the client(s). Provides psychoeducation, including positive parent-youth interactions, problem-solving, adolescent development and positive parenting. Assists families to improve their support systems by providing families with information about community services and advocacy. Assists in locating and accessing community resources, including health care. Maintains positive working relationships with referring agencies and other community resources. Coordinates services and attends meetings with other agencies, systems and community providers to meet the needs of the client(s). Schedules appointments to the client(s) home or a setting most agreeable to the client(s) at a time that is convenient for involved parties, including regular evening appointments. Provides and/or arranges transportation for clients. Develops aftercare service/treatment plans with the client and ensures that any recommended services or providers are in place prior to client(s) being discharged. Participates in an on-call rotation, which may include providing after-hours crisis intervention. Attends regular supervision staffing's with designated program supervisor and attends clinical supervision with clinical team when deemed necessary. Meets the documenting, record-keeping and billing requirements of the program. Candidate qualifications: Master's Degree in Social Work, Counseling, or a related field, OR a Bachelor's degree in a human services field plus at least two years' related experience working with children and families. Valid driver's license, insurance, and a reliable vehicle. Effective listening and trust-focused relationship-building skills. Excellent verbal and written communication skills.
