Responsibilities:
✅ Conducting Functional Behavior Assessments (FBAs) and develop individualized treatment plans

CASE MANAGERS, SPECIALISTS AND COUNSELORS (NON-RESIDENTIAL)
Case Managers provide an integrative approach to coordinating the care of their clients. Working with a defined caseload, case managers partner with the client to devise a treatment plan to meet their individual goals and objectives toward improving their lives. The heart of social work beats in the chest of every McNabb Case Manager and is the backbone of the organization, living out our mission of Improving the lives of the people we serve on a day-to-day basis.
Examples of case managers include HealthLink Care Coordinators, Safety Net Case Managers, Building Stable Lives Case Managers, CTT Case Managers, and several others.
Specialists and Counselors provide information and referral, agency linkage, advocacy, home visits and follow-up services as defined by the individualized goals of program participants. This includes a range of services from employment support, child development, family support, HIV education and wraparound support, child abuse prevention, juvenile justice reform and more.
Examples of Specialists and Counseling positions are OnTrack Peer Support Specialist, TMI Peer Recovery Specialist and Child Development Specialists.
An F-endorsement is required as many of the case management, specialist and counselor positions provide transportation as necessary.
Starting salary ranges from $15.93-$20.05 / hour depending on position, education and relevant experience. MENTAL HEALTH TECHS
Mental Health Techs are based in our Supportive Housing locations that utilize round-the-clock staff. Work schedules are shift-based and require evenings, overnights, weekends, and holidays. Shift differential supplemental pay is available for 2

and 3
rd
shift in our Supportive Housing locations. Mental Health Techs serve in a direct care role that involves supervising/monitoring clients in an inpatient or residential setting, performing intakes, inventorying belongings, performing safety checks, facilitating psychoeducational groups, and providing a supportive environment for the clients. Mental Health Techs also transport clients to and from the different appointments and activities, so an F-endorsement is required.
Starting salary for a bachelor's level clinician in Supportive Housing is $17.19/hour (Full time) and $16/hour (PRN/Part-time). Client population and acuity as well as position, education and relevant experience determine salary. HS positions also available (see HS positions post).

Responsibilities: The Family Centered Treatment Therapist provides quality psychotherapeutic services to children, adolescents, and families utilizing the Family Centered Treatment model.
Provide Family Centered Treatment (FCT) Clinical Services as per the service definition and model fidelity requirements.
FCT Certification is required and must be completed within one year of employment in order to continue to provide FCT services and maintain employment with Health Connect America (HCA).
Complete FCT Checkoffs and other internal trainings within designated deadlines. Trainings are assigned and monitored through an e-learning system.
Deliver FCT clinical services to children, adolescents, and their families, primarily in their living environments, with a family focus.
Provide 24/7 on-call coverage to a caseload of families and children. Service is regularly provided in the home and community and will require service provision at the convenience of the child and family requiring a flexible work schedule.
Complete all required Clinical Records and FCT documents with the child and family and ensure the documents are entered/uploaded timely in the Electronic Health Record, according to company standards.
Develop Service Plan in coordination with Treatment Team Process and maintain the plan per designated authorization processes in the clinician's area of service. Work collaboratively on behalf of the child and family with the identified Treatment Team members to ensure effective communication and coordinated service provision.
Provide psychoeducation as indicated in the Service Plan.
Conduct interventions with the family aligned with the FCT model to include facilitating the movement of the child and family through the four phases of treatment and completing all fidelity documents.
Assist the families and children to establish weekly goals and help them to identify the steps that need to be taken to reach the established goals.
Develop and coordinate discharge plan based on treatment, indicating all relevant aftercare needs and plans. Access community resources that would be beneficial to client aftercare needs.
Coordinate and oversee the initial and ongoing assessment activities.
Convene the Treatment Team for Service Plan.
Consult with identified medical (e.g., primary care and psychiatric) and non-medical (e.g., Dept. of Child Services, school, Dept. of Juvenile Justice) providers; engage community and natural supports and include their input in the person-centered planning process.
Provide and coordinate behavioral health services and other interventions for the youth or other family members with other behavioral health professionals and Treatment Team members.
Qualifications:
Minimum of a Master's Degree in human services or related field is required.
Two years' experience working with youth or families.
Current Family Centered Treatment Certification is preferred.

Conducts problem-solving in individual sessions to assist clients in addressing challenges (i.e., domestic violence, trauma, anger/stress management, enhanced behavior management for children/youth, grief counseling, separation issues, enhanced methods of appropriate disciplinary techniques.
Utilize various therapeutic modalities, such as cognitive-behavioral therapy (CBT), dialectical behavior therapy (DBT), and psychodynamic therapy.
Understands the key values and activities of intensive case management,

Qualifications
Requirements Family Support Counselor must:
Minimum:
Bachelor's degree in human services
3 years of human services experience
Master Level professional eligible for Georgia state licensure under supervision.
Provisional Licensed or Associate Licensed Professional in the state of Georgia with: Associate Marriage and Family Therapist (LAMFT), Associate Professional Counselor (LAPC), Licensed Master Social Worker (LMSW)
1 year of experience in diagnosis
1 year of experience in creating treatment plans
Preferred
Doctoral Degree in the Human Services field and licensed with one of the following:
Licensed Clinical Social Worker (LCSW)
Licensed Professional Counselor (LPC)
