Training Details:
The CORE program includes seven weeks of classroom training within the 22-week program. Classroom sessions occur in one- to three-week blocks and are held in midtown Phoenix. Eligible hires receive hotel accommodations, as well as meal and mileage reimbursements during required in-person training.

Job Preview at a Glance:
The Case Manager serves as a central support within the interdisciplinary team, working primarily with adults. This staff member conducts assessments, assists members with resources within the community and provides coordination of care inside and outside of the agency. The case load will be working with SMI population.
Pay Rate: Starts $20.00 hourly
Responsibilities:
Intake screenings (Completing intake paperwork and referrals)
Completing annual behavioral health assessments, including service planning and support planning
Facilitating CFTs and ARTs
Facilitating groups
Identifying gaps in care for our members
Completing SMI and SED evaluation
Qualifications:
Minimum of 22-year of age
Bachelor's degree in the behavioral health field plus one year of relevant experience in behavioral health
A combination of experience and education totaling 2 years is acceptable.
Valid AZ driver's license & motor vehicle clearance.
Must have fingerprint clearance issued by the Arizona Department of Public Safety or be eligible to receive within eight (8) weeks of hire.
