As a Social Worker at US Renal Care, you will be an integral part of a cross-functional team, working to help patients living with kidney disease achieve maximum social functioning and psychological adjustment to dialysis treatment and rehabilitation.
What you will be doing
Advocate & Support. You will be part of an interdisciplinary team working to ensure patients receive the best care, including conducting all required patient assessment and care planning activities such as assessing new patient psychosocial needs and completing the KDQOL in accordance with company policy and all state/CMS regulations. You will identify and counsel psychosocial issues and provide patient and family education. As an advocate for your patients' needs, you will coordinate communities of support for patients and their families, identify social agencies and other resources (e.g., financial/funding), provide information and referrals, coordinate transient arrangements, and represent your patient as needed with appropriate local, state, and federal agencies.
Teamwork. As part of the interdisciplinary clinic team, promote teamwork, educate staff, and provide training around patient psychosocial care. You will participate in all required continuing education and staff meetings. You will collaborate with the Medical Director and physicians and maintain positive relationships with area hospitals, agencies, vendors, and the community.
Safety & Quality. You will help with clinical and operational processes to improve patient health and minimize missed treatments and hospitalizations, achieving target goals for patient outcomes. You will also participate in monthly Quality Assessment and Performance Improvement (QAPI) activities and ensure compliance with federal, state, and local laws and regulations.
What we're looking for
Master's Degree in Social Work accredited by the Council of Social Work Education (CSWE).
Current licensure (in good standing) in applicable state is required unless employed in the state of AZ, PA or Guam. Must meet any practice requirement(s) for the applicable state.
Demonstrated working knowledge of the English language and ability to communicate verbally and in writing.
Basic computer skills, including Microsoft Office (Word, Excel, Outlook).
Proficiency in all USRC clinical applications required within 90 days of hire.
Preferred
Previous experience in providing social services to dialysis patients preferred.
Other Requirements
Must meet any practice requirement(s) for the applicable state.
Additional license requirements may be applicable depending upon state.

We're looking for gospel-minded men and women (18+) to join our Skylark family this summer as a Christian Summer Day Camp Counselor! You'll serve kids, build deep community, grow through discipleship, and have an unforgettable summer.

What You'll Do
Lead and care for your campers - cheering them on, supporting them physically and emotionally, and helping them grow spiritually throughout the week.
Guide your crew through the daily rhythm of camp - from drop-off hype to rallies, rotations, outdoor play, and pickup high-fives.
Bring the energy during program activities - helping create a fun, safe, consistent environment where campers can thrive.
Jump in with setup and tear-down each day - because camp magic happens when everyone pitches in.
Teach activity classes with clarity, creativity, and strong safety practices (fun + safety = Skylark).
Prioritize health and safety, reporting any injuries, illnesses, or concerns immediately.
Use loving, consistent discipline and follow proper reporting channels - TBRI principles included.
Communicate what's going on with your campers - their wins, challenges, moods, and moments - so your Camp Director always has a full picture.
Jump in wherever needed to support your Camp Director and the overall ministry of Skylark. Camp life = all hands, all heart.

Position Overview: As a Youth Advocate you will promote the well-being of older youth in Permanency Services Program, participates in staff meetings and trainings. Coordinates with staff and other agencies involved in providing services. Facilitates and supervises the Youth Leadership Council.
Key Responsibilities:
Assist with Child Without Placement cases and coordinate independent living services for youth in the Permanency Program.
Facilitate Life Skills classes and ensure completion of assessments like the Ansell Casey Life Skills Assessment (ACSLA) for youth aged 14 and older.
Supervise and manage the Youth Leadership Council, including its projects and needs.
Assess placement needs, safety, and resources for youth, including eligibility for Extended Care and Return to Care programs.
Maintain accurate and complete documentation for youth receiving Preparation for Adult Living (PAL) services.
Collaborate with case managers and placement departments to secure appropriate placements for youth.
Ensure completion of the National Youth in Transition Database (NYTD) and guide youth in preparation for independent living and emancipation.
Plan and supervise youth at statewide and regional teen conferences, including Peaks Camp.
Monitor and coordinate PAL contract service delivery with contract management staff.
Coordinate independent living services with public and private agencies using best practices.
Prepare and submit reports with descriptive, analytical, and evaluative content, along with monthly statistical data.
Promote cultural sensitivity and diversity in all interactions and maintain regular attendance as per agency policy.
Drive personal or agency vehicles for transporting youth and conducting agency business, adhering to all driving requirements.
Qualifications:
High school diploma or GED required.
Two (2) years of relevant work experience.
Must be a former foster care youth

Who You Are:
Associate's or Bachelor's Degree preferred.
Familiarity or experience in a child care and/or adult care setting.
Top-notch customer service skills are required.
Strong attention to detail and adherence to call center metrics.
Outstanding analytical, organizational, and time management skills.
Excellent written and oral communication; demonstrated active listening and interpersonal skills.
Excellent computer literacy and keyboarding skills.
