The Case Manager collaborates with the Service Planning team, assists with school enrollment, medical and mental health provider transitions, and tracks aftercare outcomes to promote long-term stability and success. This role is based out of Abilene. Key Responsibilities Collaborate with the Service Planning team to create and monitor individualized aftercare plans. Manage a caseload of approximately 25 youth receiving aftercare services. Conduct and participate in aftercare check-ins to assess progress and needs. Assist with school enrollment and maintain educational portfolios. Coordinate medical and mental health service transitions to ensure no lapse in care. Document all services and outcomes accurately and timely in accordance with agency and state standards.
