Qualifications
Education and Experience: Bachelor's degree.
Substitution of Experience for Education: Qualifying full-time professional eligibility determination experience and/or a combination of para-professional experience as an Eligibility Assistant or a State contractor serving in this capacity for the Department of Human Services may be substituted for the required education on a year-for-year basis.
OR
Two years of full-time para-professional experience as an Eligibility Assistant or a State contractor serving in this capacity for the Department of Human Services.
Necessary Special Qualifications: Applicants for this class must:
Complete a criminal history disclosure form in a manner approved by the appointing authority;
Agree to release all records involving their criminal history to the appointing authority;
Supply a fingerprint sample prescribed by the TBI based criminal history records clerk.
Overview
This classification learns to perform eligibility determination work of average difficulty. An employee in this class is customer facing and learns to interview customers and determine eligibility for SNAP and TANF programs. This classification performs responsibilities at the learning level under direct supervision. This classification differs from the Eligibility Counselor 2 in that an incumbent of the latter functions at the working level.
*An applicant appointed to this classification will be reclassified to an Eligibility Counselor 2 after successful completion of a mandatory one-year training period; 'unacceptable' or 'needs improvement' performance ratings during the training period will result in automatic demotion or termination.
Responsibilities
Learns to manage a workload of initial and on-going eligibility determinations for SNAP and TANF programs based on specific demographic and economic criteria in accordance with Federal and State requirements through independently conducting probing interviews with customers by phone or in person.
Learns to provide case management for initial SNAP and TANF eligibility, ongoing eligibility, and customer changes to assess impact on continued eligibility through direct customer engagement via phone or in person.
Learns to calculate monthly income and expenses to ensure accurate budgeting. Documents information regarding verifications requested/received, benefits and services provided, and other case-related activity using a computerized case record and/or paper case files.
Learns to ensure accurate receipt of entitled benefits by reviewing, verifying and utilizing data provided by customers and multiple electronic systems while maintaining confidentiality.
Learns to assist customers with the application process and scheduling appointments.
Learns to review and interprets program policy and explains eligibility and ongoing responsibilities for programs/services to ensure customer understanding.
Learns to assess customer's strengths and considers their goals, desired outcomes, work activities, and supportive services to aid in self-sufficiency. Identifies benefits and services that may be available to customers within the Department or from external sources and makes referrals as appropriate.
Learns to develop and maintains partnerships with vendors and outside organizations to obtain information, promote programs and identify available services and benefits to effectively meet customer needs and promote self-sufficiency.

Essential Duties and Responsibilities: The PRN Community-Based Counselor: * Carries a small case loas as assigned by supervisory on an as-needed basis * Holds individual and family sessions with each family 3 times a week, scheduled at the convenience of the families * Conducts on-going assessment of youth to determine their needs from a strength-focused, solution-based perspective * Attends 3 weekly meetings (individual, team, and clinical consultation) for professional development to help enhance clinical skills * Drives up to 60 to 80 miles to meet with families in their homes and communities (additional travel may be required) * Collaborates with other providers, case workers, and courts to formulate a collaborative treatment plan * Provides on-call crisis support to the youth and family (schedules vary by location) * Completes accurate and timely documentation in an electronic medical record system (EMR) * Performs other duties as assigned Additional Information: * Schedule is flexible and non-traditional as it is based around the availability of youth and families served. * Applicants must possess a current, valid driver's license, an automobile for work purposes, and proof of auto insurance. * Community-based staff will be reimbursed for applicable mileage. Provides crisis intervention services, seeking to address the cause as well as the presenting complaint, coordinates family conferences and serves as liaison to family members.
Provides case management to Veterans and families as needed throughout the continuum of care.
Addresses issues such as advance directives and organ donation in accordance with acceptable standards.
Inputs all patient information into VISTA/CPRS.
Completes any and all clinical reminders “due” at the time of each patient visit.
Participates in program planning and development, making recommendations for changes in policies and procedures as needs and opportunities arise.
Collaborates in the performance improvement processes and complies with performance measures as required by Valor and the VA.
Participates in activities that obtain feedback from veterans and works to enhance services as needed.
Complies with all Valor and VA training requirements.
Other duties as assigned.
Compliance: Fulfill compliance requirements of the Office of Inspector General (OIG), Joint Commission (JC), Environment of Care (EOC) oversight, lab compliance and other related items.
Clinical Outcomes: Remain focused on achieving excellent clinical outcomes through the specified VA guidelines.
Customer Service: Must provide excellent customer service to each veteran and his/her family, both in person and over the phone, as well as to fellow colleagues and clinic visitors.
Collaborates with Clinic leadership to resolve veteran concerns or complaints.
Enrollment Growth: Participate in the clinic's outreach events to help support the clinic's enrollment initiatives.
Save & Recapture: Support patient enrollment and retention in appointments, follow-up visits or nurse visits.

Qualifications
Must have one of the following educations:
-- Doctoral degree in psychology from a graduate program in psychology accredited by the American Psychological Association (APA), the Psychological Clinical Science Accreditation System (PCSAS), or the Canadian Psychological Association (CPA) at the time the program was completed.
-- Master's degree, or higher, in Social Work from a program registered by the Department of Education is required, with specific training dealing with adult patients. Equivalent degrees (Counseling, Marriage and Family Therapy, etc..) will be considered and are subject to final approval from the local VA.
Must comply with and maintain all requirements for a valid, unrestricted LCSW (or equivalent) license in the state of desired employment and able to independently practice at the master's degree level.
Minimum of 1-3 years' post-master direct counseling experience with adult patients.
Must be credentialed and remain in good standing through the Veterans Health Administration (VA).
Current certification in Basic Life Support (must be renewed periodically as specified by the certifying agency - AHA valid for two years, e.g.)

Essential Duties & Responsibilities:
Provide Family Centered Treatment (FCT) Services as per the service definition and model fidelity requirements.

Complete FCT Checkoffs and other internal trainings within designated deadlines. Trainings are assigned and monitored through an e-learning system.
Deliver FCT services to children, adolescents and their families, primarily in their living environments, with a family focus.
Provide 24/7 on-call coverage to a caseload of families and children. Service is regularly provided in the home and community, and will require service provision at the convenience of the child and family requiring a flexible work schedule.
Complete all required Clinical Records and FCT documents with the child and family and ensure the documents are entered/uploaded timely in the Electronic Health Record, according to company standards.
Develop Service Plan in coordination with Treatment Team Process and maintain the plan per designated authorization processes in the clinician's area of service. Work collaboratively on behalf of the child and family with the identified Treatment Team members to ensure effective communication and coordinated service provision.
Conduct ongoing revisions of Service Plan and is responsible for implementation of Service Plan.
Provide psychoeducation as indicated in the Service Plan.
Conduct interventions with the family aligned with the FCT model to include facilitating the movement of the child and family through the four phases of treatment and completing all fidelity documents.
Assist the families and children to establish weekly goals and help them to identify the steps that need to be taken to reach the established goals.
Make referrals to services and resources on behalf of the families and children.
Develop and coordinate discharge plan based on treatment, indicating all relevant aftercare needs and plans. Access community resources that would be beneficial to client aftercare needs.
Coordinate and oversee the initial and ongoing assessment activities.
Convene the Treatment Team for Service Plan.
Consult with identified medical (e.g., primary care and psychiatric) and non-medical (e.g., Dept. of Child Services, school, Dept. of Juvenile Justice) providers; engage community and natural supports, and include their input in the person-centered planning process.
Ensure linkage to the most clinically appropriate and effective services including arranging for psychological and psychiatric evaluations.
Provide and coordinate behavioral health services and other interventions for the youth or other family members with other behavioral health professionals and Treatment Team members.
Monitor and document the status of the consumer's progress.
Participate in Quality Assurance/Quality Improvement activities to include audits, corrections and designated improvement plans and projects.
Qualifications
Education & Experience:
Minimum of a Bachelor's Degree in human services or related field is required.
Two years' experience working with youth or families.
Current Family Centered Treatment Certification is preferred.

QMHP or ability to register as a QMHP.
Mississippi

The goal of the WIC program is to improve the health of families and individuals during critical times of growth and development.
The WIC Breastfeeding Peer Counselor is responsible for providing evidence based breastfeeding education and support for WIC participants through one-on-one clinic visits, phone calls, group classes or support groups. The WIC Breastfeeding Peer Counselor will participate in WIC community outreach and breastfeeding promotion activities and events.
The WIC Peer Counselor is expected to be an active participant in supporting the mission, vision and values of the WIC program and the Tennessee Department of Health. They will provide consistent, quality services to the local WIC participants within their scope of practice. They will demonstrate enthusiasm, initiative and flexibility in assignments and maintain positive working relationships and communication with their co-workers.
Minimum Qualifications:
* Has breastfed at least one baby

* Current or previous WIC participation preferred but not mandatory

Requirements:
Education -Must be met programmatically, i.e., as a matriculated in-residence student in a formal graduate training program organized to provide graduate education and training in psychology, and whose stated purpose and design is to educate and train Psychologists, Senior Psychological Examiners, Psychological Examiners, and/or Certified Psychological Assistants work, psychology, rehabilitation, special education, or in a related human services field, is required. A candidate that has a masters degree in an appropriate field, the experience requirement is preferred, but not required.
Experience -Working with children with serious emotional disturbances or substance abuse problems. Experience with group work techniques is a plus.
Skills -Participates in both basic and applied research endeavors and contributes to the basic fund of psychological knowledge; Must be able to manage for results, work collaboratively, serve as a change agent, possess strong organizational and communication skills, and make quality, timely decisions. Must be self-motivated and able to work without direct supervision.
