    Variable (United States of America) CAT Therapeutic Mentor The CAT Team Therapeutic Mentor/Case Manager provides mentoring and supportive services for children ages 3-11 with mental health and/or co-occurring disorders as a part of the CAT Team. Responsible for developing positive relationships by providing guidance and facilitating interactions and services for children and their families. This position will also provide targeted case management services to children with serious emotional problems, their families; and caretakers and assist clients in maintaining their highest level of functioning in the environment, which best meet, their needs. Services include but are not limited to: medical, social, legal, educational and other support services. Must be willing to travel throughout Volusia and Flagler County. Refer child and family for additional services as identified. * Act as liaison and consult with the school system, community agencies and families to maintain coordination of the treatment process. * Advocate for child and family's rights and needs. * Provide support to child and family as a role model teaching life skills, positive discipline, parenting skills, financial responsibility and homemaking skills. * Assist families with developing natural support systems. * Provide on-call crisis intervention services during nighttime and weekend hours as assigned. * Provide transportation, respite and tutoring services as assigned. * Completes initial assessments on each new client and complete an initial service plan that assists clients in developing goals and addressing areas of need. * Completes periodic service plan reviews and assessments according to program guidelines. * Participates in client treatment planning and case review with other client care providers. * Attends meetings as scheduled. * Documents service activity in EMR, and ensures the medical record is up to date, accurate, and meets all documentation standards in accordance with program requirements. * Coordinates and provides care that is safe, timely, effective, efficient, equitable, and client-centered. * Executes linkages identified on the service plan and monitor progress of service delivery according to program guidelines. * Facilitates client access to community resources, including locating recreational activities, housing, food, clothing, school programs, vocational opportunities or services, providers to teach life skills, and relevant mental health and substance use services. * Assists client to develop natural resources and make contact with social support networks. * Helps clients make informed decisions by acting as their advocate regarding their clinical status and treatment options. * Communicates clients' progress by conducting interdisciplinary meetings and evaluations; disseminating results and obstacles to therapeutic team and family; and identifying treatment influences. * Provides limited crisis intervention under appropriate supervision. * Coordinates transportation for clients in order to execute linkages identified on the service plan. * Attends court hearings as required by program guidelines. * Maintains an awareness of proper safety procedures and guidelines and apply these in performing daily activities and tasks. * Adheres to professional standards as outlined by protocols, rules and regulations. * Additional duties may be required as per specific program guidelines. * Duties may be required as per specific program guidelines.
