Integrated Care Managers advocate for, facilitate, and coordinate services that a person needs to pursue their goal of recovery. They are a central point of contact to ensure that people are receiving integrated care and services recommended for them that supports their personal recovery vision.

(3) current certification as an alcohol and drug counselor or alcohol and drug counselor reciprocal, through the evaluation process established by the International Certification and Reciprocity Consortium Alcohol and Other Drug Abuse, Inc.;
(4) completion of a bachelor's degree including 480 hours of alcohol and drug counseling education from an accredited school or educational program and 880 hours of alcohol and drug counseling practicum; or
(5) employment in a program formerly licensed under Minnesota Rules, parts 9530.5000 to 9530.6400, and successful completion of 6,000 hours of supervised work experience in a licensed program as an alcohol and drug counselor prior to January 1, 2005.
(c) An alcohol and drug counselor may not provide a treatment service that requires professional licensure unless the individual possesses the necessary license. For the purposes of enforcing this section, the commissioner has the authority to monitor a service provider's compliance with the relevant standards of the service provider's profession and may issue licensing actions against the license holder according to sections 245A.05, 245A.06, and 245A.07, based on the commissioner's determination of noncompliance.

POSITION OBJECTIVE: The Community Advocate is responsible to provide crisis intervention, personal and legal advocacy, court support and systems advocacy for individuals affected by domestic violence and/or sexual assault. POSITION RESPONSIBILITIES
Advocacy (75%)
Provide emotional support, advocacy, crisis intervention and information and referral to callers on the 24-hour crisis line.
Provide direct advocacy and support services to victims of domestic and sexual violence.
Assist clients in pursuing civil, criminal and family court remedies.
Notify victims regarding significant events relating to the investigation and prosecution of the victim's case.
Provide trauma-informed and empowerment model services to clients. Assume positive intent.
Effectively “triage” priorities and multi-task as needed.
Collaborate with agency staff to review client cases and make recommendations to aid in client success.
Develop and facilitate an active support group.
Provide transportation for clients when necessary and with approval.
Develop and maintain listings of available community resources/referrals and act as a liaison between those resources and clients as needed.
Maintain professional boundaries, confidentiality, and integrity of sensitive information always.
Role model positive behavior, interactions, conflict resolution etc. for clients and with other staff.
Offer support, assistance to and inclusion of volunteers.
Maintain a welcoming, peaceful, healing office environment for clients.
Attend and participate in all staff and supervision meetings, training and in-services as required.
Community (15%)
Greet visitors, callers, and donors, handling their inquiries or directing them to the appropriate person according to their needs.
Receive, record, thank and organize donations. Financial contributions must be given to the supervisor immediately.
Develop and maintain positive working relationships with community partners specifically victim services, county attorney, law enforcement, court services, and family services.
Participate in outreach and awareness efforts and public speaking engagements as directed.
Participate in local multi-disciplinary teams as directed.
Professional behavior, dress and conduct are expected in all aspects of work with clients as well as contact with the community.
Administration (10%)
Read and communicate through email, text etc. with team; manage and keep calendar current throughout workday.
Complete documentation to assure compliance with agency standards and funder expectations. Complete necessary electronic entries and all agency required forms in a timely fashion.
Comply with funder, state and federal rules, laws, and regulations.
Complete agency forms to document critical incidents or injury, notify supervisor immediately.
Adhere to and promote Family Pathways organizational, program and personnel policies.
Report needed repairs to supervisor. Assist in up-keep of the office and grounds. Complete minor repairs.
Recruit financial donations as opportunities arise.
Assist with organization and transportation of donations.
Perform other duties as assigned and work as a team member with staff, volunteers, and administration.
Required:
Associates Degree in human services or 2 years' experience in human service field preferably providing domestic/sexual violence advocacy
Experience working with people who are managing crisis.
Ability to work nights and weekends as needed and participate in an on-call rotation.
Must have a dependable vehicle, a current and valid MN state driver's license and proof of insurance.
Preferred:
BS degree in human services or related field

Ensure compliance with Department of Human Services (DHS) 245G Statutes including timely documentation completion, clinical thoroughness and accurate billing for services provided. Conduct chart audits as requested.
Understand the Grievance Policy to assist clients in navigating the process to resolve grievances at the lowest level in a timely manner and document accordingly.
Maintain knowledge of and apply American Society of Addiction Medicine (ASAM) Criteria. Appropriately diagnose clients using the DSM -5.Provide screening and assessment through SUD Evaluations/Assessments and required documentation.
Provide intake and orientation to programming and required documentation.
In conjunction with the client, create and monitor an individualized treatment plan based on client's needs, language, reading skills, cultural background and strengths.

Implement approved SUD Group Curriculum

Provide client education strategies to avoid inappropriate substance use and health problems related to substance use and the necessary lifestyle changes to regain and maintain health. Assist client to integrate gains made during treatment into daily living and to reduce the client's reliance on a staff member for support as progress is made.
Participate in consultation with other professionals by attending Weekly SUD Case Consultation. Present client cases monthly, implement feedback into client's treatment plan and document accordingly.
Address co-occurring disorders of clients, including client education on symptoms of mental illness, the possibility of comorbidity, and the need for continued medication compliance while recovering from substance use disorder.
Complete drug screens with clients based on clinical necessity. Therapeutically address drug screen results with clients.
Maintain the integrity of the SUD Program Structure including the need for weekly SUD Individual Sessions as part of ASAM Levels of Care.
Participate in scheduled individual clinical supervision meetings with supervisor.
Assist with training of professional program staff and provide supervision to assigned interns as requested.
